Dear Mr. Waite, October 7, 1999
A leak near our water meter occurred this spring, which resulted in a greater
than normal water usage showing on our Edmonds utility bill.
We had the same problem occur in 1998 due to our line leaking between the
water meter and the house. We replaced this line with new copper pipe. The
water usage returned to normal after this repair.
We wrote to you in June 1998 about -this problem. You gave an adjustment to
our water bill at that time.
I am again requesting an adjustment on our water bill for this last four months of
1999. Also we were gone for 2 months during this period and turned the water
valve off in the house.
The leak continued until August 20th when repairs were made. Since August
20th, the water usage has returned to. normal.
Our account number is: 2-14100.

Dear Mr. & Mrs. Dotsch:
I have reviewed your account and will allow a credit for the billing period between
February 11, 1998 through June 19, 1998 in accordance with our City policy. The policy
states that the customer will be billed at the retail rate based upon the average water
consumption for the same period during the previous year. In addition, the excess water
lost from the leak will be billed to customer at the City's wholesale rate with a 15%
surcharge added for administrative cost. Only one leak credit will be granted in any three-
year period.

We are pleased to do the following work:
1. Replace your waterline from the meter into the house with 1 %" Type L copper.
2. Provide and install new 1 %" ball valve in the water heater compartment. We are moving the shutoff
from the present location to the water heater compartment.
3. Asphalt removal and replacement is included. Asphalt will be repaired with cold patch.
4. Provide and install pressure reducing valve.
5. We will be digging thru the bushes and ground cover and they will not be put back.
6. . We will cut the sod, set it aside and put it back once the waterline is backfilled.
7. Provide and install 1 %" gate valve for future sprinkler system. ,
8. Install a weep hole by the water meter. q
Price: $3,699.00 + Tax'
PLEASE NOTE: 50% is due down at time of contract signing, and balance is due at time of completion.
The above proposal is based on complete and continuous access to all areas of the job site to allow the work to continue
without interruption once it is commenced. Dispute of any billing must be responded to in writing within ten days of the billing
date. Washington State Sales tax or current resale certificates is in addition to the bid price. For the work described above,
Gene Johnson Plumbing & Heating, Inc. will be paid 50% prior to acquiring permits or commencement for any work, balance
due upon completion of all work and inspection by the government entity if any. If payment of the entire amount due is not
received by the 10th of the month following completion of all and govem t inspecti , any balance owing Gene
Johnson Plumbing &Heating, Inc. will be assessed at 1.5% month on t outstan ' g ba ce. We (I) agree to accept a

work described above. The amount specked does not include y9 work or materials which may be necessary because
of obstacles unforeseen at the date of this col ract such as solid r f I
concrete, boulders, buried stumps or debris, extensive
ground water, heavy rainfall prior to or during the period of work, or any condition or object which necessitates additional
excavation or fill. In the event that these unforeseen obstacles or conditions are encountered, Gene Johnson Plumbing &
Heating, Inc. will be paid for all additional work and materials necessitated based on Gene Johnson Plumbing's current rate for
time, equipment and materials, as provided with this proposal. Gene Johnson Plumbing & Heating, Inc. is not responsible for
replacing or restoring any sidewalks, curbs, fences, lawns, unmarked or invisible pipes, or other improvements that may be
disturbed in the process of the work described above. Gene Johnson Plumbing & Heating, Inc. will compact backfill.
However, some settlement may occur, and Gene Johnson Plumbing & Heating, Inc. does not warrant against settlement and
will not be responsible for any damage caused by settlement not for restoration. Owner or owner's agent agree to accept and
pay immediately, without dispute to Gene Johnson Plumbing & Heating, Inc. for actual city backfill and restoration charges on
all jobs requiring street excavations. City invoice plus 15% processing and handling charges. Payment due immediately upon
receipt of invoice or late charge penalty of 5% per month will be accepted and paid. If the owner wishes to change or add to
the work listed in this agreement, Gene Johnson will perform this work based on it's current rate for time, equipment, and
materials, or an additional proposal will be prepared. There are no oral or written agreements affecting this contract or explicitly
referred herein. An express lien is hereby acknowledged for the above work. If Gene Johnson Plumbing & Heating, Inc. retains
an attorney to collect any obligation owned hereunder, Gene Johnson Plumbing will be entitled to recover its reasonable
attorneys fees, consultants fees, and any other costs or expensed related thereto.
