The Critical Areas Checklist contained on this form is
to be filled out by any , .person preparing a
Development Permit Application for the City of
Edmonds prior to his/her submittal of a development

The purpose of the Checklist is to enable City staff to
determine whether any potential Critical Areas are, or,
may be, present on the subject property. The
information needed to complete the Checklist should
be easily available from observations of the site or
data available at City Hall (Critical Areas inventories,
maps, or soil surveys).
An applicant, or his/her representative, must fill out
the checklist, sign and date it, and submit it to the
City. The City will review the checklist, make a
precursory site visit, and make a determination of the
subsequent steps necessary to complete a development
permit application.
Please submit a vicinity map along with the signed
copy of this form to assist City staff in finding and
locating the specific piece of property described on
this form. In addition, the applicant shall include
other pertinent information (e.g., site plan, topography
map, etc.) or studies in conjunction with this Checklist
to assist staff in completing their preliminary
assessment of the site

A site inspection revealed that a stream runs through your property. The property is developed
with a single-family residence. The City of Edmonds identifies this a Class 2b stream on both
the City's Critical Areas Map and Environmentally Sensitive Areas Map.
The applicant submitted a survey with his Critical Areas Checklist that delineates the creek and
top of bank. The survey was created when the property was subdivided in May of 1991.
Because a copy of the survey was submitted with the subject application, the study
required has been completed.
Class 2b streams require a 25-foot buffer, and an additional 15-foot building setback. In addition
to the required setbacks, any additional development within or ' adjacent to the stream
requires an environmental review, St#te Environmental Policy Act (SEPA) Checklist.

Re: Regarding Installation of a 3/" Water Service
On March 29, 1999, the City of Edmonds Water Division installed a V water
service in front of your property with you intent to connect your water service to
this new point. This will eliminate your existing service and meter on North
Meadowdale Road.
The new meter location was determined best for meter reader access, and if the
need arises, a traffic rated meter box lid can be installed.

Subject: Water Leak Credit #3-07050
Dear Mr. Walsh:
I have reviewed your account and will allow a credit for the billing period between
October 23, 1998 through April 25, 1999 in accordance with our City policy. The policy
states that the customer will be billed at the retail rate based upon the average water
consumption for the same period during the previous year. In addition, the excess water
lost from the leak will be billed to customer at the City's wholesale rate with a 15%
surcharge added for administrative cost. Only one leak credit will be granted in any three-
year period.

Dear Mr. Waite
I am writing in regard to my unusually high water bill this month. I received
a notice on my door 2 months ago saying I might have a leak, I called the
water dept. and explained we had just purchased the house asked if they
thought the high consumption might be because there was four of us living
there now as compared to a retired couple previously, they told me that was
most likely the reason. Upon coming home from work last week I noticed I
had no water pressure and once again I walked the line route to my water
meter to find water pouring out of the ground and the meter spinning out of
control. I've now cured the problem and was hoping you might have mercy
on my misfortune, by re -adjusting my water bill.
