Average annual consumption for the last 5- year period was 25.5 million gallons. Existing deficiencies: Meter is not testable for accuracy. This meter has not been tested or had the measuring chamber calibrated or replaced. Meter is not UL/FM fire rated. This meter supplies three fire hydrants and a two firelines. Meter has a 6" swing check valve immediately downstream from meter, this affects meter accuracy. A five pipe diameter distance of straight pipe is manufactures' recommendations for meter accuracy and performance. Meter flow range is 30 — 2000 gpm. This means poor low flow accuracy. Proposed Improvements: Replace meter, valves, and piping components. Install one 6" Sensus Omni meter. This meter will provide a UL/FM rating AW WA C-703 for fire protection and domestic use. Meter has a greater flow range of 3 - 2000 gpm. Meter will be testable for future accuracy verification, and has data logging capability. Other notes: Current mag meters on the market do not have UL/FM approval. The tape contained the complete communications of the incident, however, the only portion played.was from receipt of the initial AFA call from Washington Alarm Company until the first unit gave a scene size -up. Each person at the analysis was also provided with a copy of the dispatch printout that included a record of all communications for the incident and the Fire Incident Report, including all narratives. Also incorporated in the analysis were overheads of the grid map (456), a footprint of the building, and a prefire of the portion of the building involved in the incident. The prefire was created for this analysis. Additionally, photographs from the scene taken by Inspector McNulty, were presented, as was a slide indicating a representation of the Incident Management assignments. Dispatch: Following the playing of the incident tape a discussion ensued concerning the response by the SnoCom dispatcher. Two areas of concern were addressed. The first concerned the length of time from the initial AFA call until the first unit was dispatched. This time was 1 minute 18 seconds. At the time of the alarm E14 was out of their response area on inspections and their acknowledgement of responding was another 1 minute 37 seconds. The second concern dealt with the fact that prior to the response by E14, SnoCom had received three calls on this incident and did not immediately upgrade to a commercial fire response. Following the initial AFA, a call was received from a neighbor expressing concern for an odor from the site. The third call was a report from the scene by a City of Lynnwood employee reporting heavy black smoke was coming from the lab. With this final piece of information it was still 3 minutes 11 seconds before the alarm was upgraded to a commercial response. Scene Operations: The scene presented an unusual situation because of the topography involved. The Incident Command System was employed and there appeared to be no major problems encountered. Improvement of decontamination procedures was discussed. More manpower and earlier recognition of decontamination needs were recognized as areas to be improved during future incidents. Hot Zone and Warm Zone should have been determined earlier. This problem lead to early movement of firefighters between the Hot Zone and safe areas Scene Communications: Radio traffic was recognized as an area that went well. It was felt that communications were brief, clear and concise. There was no noticeable unnecessary radio traffic. s► Support Functions: Rapid Intervention Team was set up too close to the Hot Zone. Safety. Two items. For a time, two employees at the plant were walking around the site freely. Although there was some communication lapses in their location, apparently they were to provide information to the Operations Chief. They were unescorted and their location was not known for several minutes. There was also some concern that the Safety Officer placed himself in a position that necessitated his need for decontamination. One firefighter was injured when he received cuts to his fingers. Three firefighters were exposed to fiberglass insulation. Fire Cause: It was determined that a three-ring binder placed on the chimney opening of a furnace, started the fire. Clearance for hydrant stem valve operation will be contracted for with a fence company within the next few days according to Plant Technician, John. Chlorine containers shall be removed from a heat source (fire, etc.) because the fusible plug will melt at 158 ° F. Refrain from using water to control the spread of chlorine. The resulting solution will be an acid that may prove more hazardous than not spraying with water. When a chlorine leak is detected and/or an alarm has been given, the following events should occur: 1. Determine the source and severity of the leak: a. Notify all plant personnel. b. Chlorine leak too large to contain - call 911. C. Leak identified and workable - go to step 2. 2. Two (2) people shall don the SCBA units and proceed as follows: a. Shut off the valves at the one ton containers. b. Locate the leak by passing the ammonia swab adjacent to the chlorine feed system. 3. If the leak is located at the one ton container and requires equipment from the chlorine institute emergency kit "B" to contain, evacuate the building and proceed to the blower control room.
