February 15, 1996
On Thursday, February 15, 1996, 1 received a call from Mary Ann Hayslip who resides
at 18230 - 91st Avenue West. She stated that two city vehicles were in her area this
day and had ran over her garden shrubs and some rocks while turning around.
I investigated and found out that unit #80 and #68 had been in this area and therefore,
dispatched Everett Akau to take pictures of the site and meet with the owner
concerning her claim. I
It was Everett's conclusion that this was our responsibility and a cost for plant damage
was assessed by the homeowner. This valued at approximately $60 in plants per A &
D Nursery in Clearview. I did call to confirm the costs.
The crew did state this had happened and that they had attempted to notify the
customer but there were dogs in the yard.
I feel the cost is warranted and must be paid.

period during the previous year charged at normal customer rates, plus
the excess usage charged at the City's cost, plus a surcharge of 15%

Please issue new billing to this account as they were notified of a
leak by our meter reader on January 9, 1991. The homeowner found the
leak and made the necessary repairs. I have talked with the homeowner
and she has no receipts for those repairs. The leak was repaired and
checked by City crews. Please allow a credit according to our City
policy.
