Please disregard the last Fire Access Plan SHEET AIA I sent you at 3PM Friday 2/22/13. Please find the new revised Fire Access Plan Sheet AIA
attached. The fire lane has been moved to be closer to the building to be located in the 24' wide drive lane. At the Northwest terminus of the fire lane,
the tuming radius from the 19' wide drive asile is equal to the turning radius of the 24' asile. Also the move closer to the building would allow us
more flexibility in the Northeast comer of the site for ftiture development. This adjustment from the plan we discussed earlier this week should be
fine because it 1. Moves the Fire Access Lane closer to the building and 2. provides a wider drive asile. Please let us know if this will work.

Sound/Noise originating from temporary construction sites as a result of construction activity are exempt from the noise limits
of ECC Chapter 5.30 only during the hours of 7:00am to 6:00prn on weekdays and 10:00am and 6:00pm on Saturdays,
excluding Sundays and Federal Holidays. At all other times the noise originating from construction sites/activities must
comply with the noise limits of Chapter 5.30, unless a variance has been granted pursuant to ECC 5.30.120.
Applicant, on behalf of his or her spouse, heirs, assigns, and successors in interests, agrees to indemnify defend and hold
harmless the City of Edmonds, Washington, its officials, employees, and agents from any and all claims for damages of

02/17/2009 Dug up, once dirt was off it stopped leaking, observed for some time,
backfilled hole, it started leaking again
02/20/2009 Vactor truck cleaned fittings, found fitting that was leaking, with dirt on
most of the pipe
02/23/2009 Dug up fitting to be fixed, City couldn't shut water supply off, so the
repair were made not to the standard we would like, but it seems to have
stopped the major leak that was causing the flooding of the
sidewalk per City of Edmonds Fire. If there is still a small leak in the
system, before anything can be done, the City will have to fix their valves,
so we can take the appropriate measures to fix the leak in the system.

Water supply for fire sprinkler system at 190 Sunset Ave has ruptured and is in the process of evaluation
and repair. Fire watch is imposed.
The problem is likely affecting entire building as there are 2 wet systems.
Please observe FDCs at following locations. Engine water will be the first extinguishing water in case of
fire.
-North building system FDC located at 184 Sunset Maytag laundromat.
-South building system FDC located at Sunset and W. Dayton on street corner.
Affected commercial addresses: 170 through 194 Sunset Ave S., and 111-123 through W. Dayton St.

1) West Side of Bldg. Back Room:
A rest room was added without sprinkler coverage. 1 sprinkler head needs to
be installed to ensure proper coverage and meet fire code.
2) Back Room:
Changes in the soffits require additional sprinkler coverage. 3 sprinkler heads
need to be installed to ensure proper coverage.
3) Back Room:
2 sprinkler heads are 3 feet apart. According to code, heads shall be no closer
than 6 feet from each other. 1 sprinkler head needs to be plugged or 1 head
relocated to avoid cold soldering (where the spray pattern from one sprinkler
head cools the- other enough that it does not activate).

Enclosed for your information and guidance is Ordinance #lS61,
which was passed by the City of Edmonds Council on June 1, 1971. This
Ordinance pertains to the installation, maintenance, and inspection of
Fire Protection Systems, and sets forth charges for routine maintenance
andpenalties for unauthorized usage and/or uncorrected leakage.
Many man-hours of maintenance services and.testing are performed
each year to insure that the Fire Suppression and Sprinkling Systems
are in working order. The monthly service charge indicated in Section
6.21.040., Paragraph (a) of the Ordinance is used to defray these costs. -
An inspection of your account indicates there has been unauthorized
usage of your System during the last billing period. Another inspection
will be made in thirty (30) days from the date of this letter. If at
that time the leak or unauthorized usage has not been corrected, a
minimum charge of $4S will be made as indicated in Section 6.21.040,
Paragraph (b) of the Ordinance.

Dear Mr. Wold:
Late in September this department was contacted by a representative
from C-K Color Lab stating that the sprinkler system at Parker
Plaza had shifted and was in eminent danger of breaking a main
supply pipe,
Contact at that time was made with your firm to initiate repairs
through a qualified sprinkler company.
Subsequent inspection of the complex indicates that the repairs
made are of a temporary nature with attitional work needed to
restore the system to acceptable standards.
The Fire Department will require that the two main supply pipes
entering the building from underground be raised back to their
original level. At present all of the drain lines run up -hill
and some of the operating shafts are in danger of being sheared
off.
Please advise this department when the above conditions are
cleared or have the sprinkler company call when they are working
in the building.

Give a complete account of the work of the fire company. The sequence of operations and where the fire company worked
(floor of building, for example) should be clearly recorded. To the best of your recollection show where each man worked and
what he did. Describe where the fire apparently started and from what cause, what material burned, progress of fire upon
arrival of company, and any dangerous condition of building. bustibles, or ob-
