NOTE No. 5—Trenches in street must be water settled and surface of street restored to original condition. Contractors shall be responsible for failure due to Improper
work which may develop within one year of completion.
NOTE No. 6—It is unlawful to alter or do any other work than is provided for in the permit, or to do any work on the maln sewer or its appurtenances except to in-

MEMO FOR RECORD
SUBJECT: <199 22-MAPLtWOOD DRIVE
On October 18, 1984, Everett Akau and I received a call regarding
a plugged sewer at 19922 Maplewood Drive. Upon arriving at the
residence, we checked our manhole in the southwest corner of
the property. We found the stoppage to be in the 8" main
sewer line and notified the homeowner that we would be clearing
up the problem.
We picked up Unit #24 and returned to the job site. We ran
460' west of manhole 109-9 and stopped. We measured out the
distance and determined that we were in manhole 192-8. After
doing this, we figured out that the plug must be between man-
holes 190-8 and 192-8.
We moved Unit #24 and proceeded up the sewer line north of man-*
hole 190-8 and encountered an obstruction 87' in the line.
We backed up 5' and continued up the sewer line hitting and
breaking the plug. We then continued up to manhole 192-8,
twice, cleaning out the main and pulled back big roots.
We cleaned up the back patio of the subject residence and
dye tested the downspouts to make sure they were not connected
to sewer, which they were not. After determining this, we
talked to the homeowner And requested to inspect the damage
that had been done. She showed us the downstairs bathroom
and the carpet in the hallway that had backed up sewage on
it. She also indicated that she had called Rescue Rooter
out to rod out the sewer line determining that it was our
problem. Also, owner has illegal storm drain connection in
back cement patio under back deck of house.
