CRITICAL AREAS CHECKLIST
The Critical Areas Checklist contained on this form is
to be filled out by any person preparing a
Development Permit Application for the City of
Edmonds prior to his/her submittal of a development
permit to the City.
The purpose of the Checklist is to enable City staff to
determine whether any potential Critical Areas are, "or
may be, present on the subject property. The
information needed to complete the Checklist should
be easily available from observations of the site or
data available at City Hall (Critical Areas inventories,
maps, or soil surveys).
An applicant, or his/her representative, must fill out
the checklist, sign and date it, and submit it to the
City. The City will review the checklist, make a
precursory site visit, and make a determination of the
subsequent steps necessary to complete a development
permit application.
Please submit a vicinity map along with the signed
copy of this form to assist City staff in finding and
locating the specific piece of property described on
this form. In addition, the applicant shall include
other pertinent information (e.g., site plan, topography
map, etc.) or studies in conjunction with this Checklist
to assist staff in completing their preliminary
assessment of the site

ADMINISTRATIVE APPROVAL FOR A REPLACEMENT ROOF FOR
THE MURKIN TOWNHOMES.
The Murkin Townhomes are proposing to replace their existing flat pitched roof with a slightly
pitched roof. Similar materials will be used and little if any change will be apparent by
neighboring properties.
For that reason I am making the determination that the application is complete and that the
proposed change to the roof is consitant with existing approvals on the site and the City's Design
Review Criteria. With that, I am giving the project an administrative approval, waiving the
requirement to go before the ADB.

I will be examining the parking situation and traffic flow needs from
196th St. S.W. to 220th St. S.W., along the 76th Avenue W. corridor. -
As time allows, this review will result in recommendations and related
topics.
The City is pursuing a traffic signal installation in your -immediate
area (208th St. S.W. and 76th Ave. W.), which will undoubtedly result
in a change in lane configuration along your frontage.
I will endeavor to keep you informed as this situation is investigated
and changes are instituted. Should you have particular questions,

Gentlemen:
Due to our location directly across the street from College Place Elementary
school and approxiffately �4 mile North of Edmonds High School, the sidewalk
in front of Dtirkin Townhouses Condominium,�20714 76th Avenue West gets con-
siderable use frcm school children.
Between the sidewalk and the curb, we eliminated all the weeds and have
maintained this strip with bark which gives a quite pleasant appearance
when it is left in place --unfortunately, the school kids kick it and track
it onto the sidewalk which makes a continuing maintenance problem -- and
on the occasion of very heavy rains, the bark has "floated" over the curb
and into the street.
Ile would like to fill in this area with concrete bricks (red bricks being
prohibitively expenside) set in sufficient mortar to keep them from, comi.ng
loose or easily removed.
Please advise if we may have this work done by a qualified individual.

This will confirm our telephone conversation and agree-
ment on June 11,�1979, regarding resolution of,the amounts of
money you owe to the City of Edmonds..
You had previously been billed $2,947.08 for work that
the City accomplished at your request. The original estimate
for that work was $2,322.94. This left an additional amount
of $624.14 owing from you . to the City. -
Under the circumstances that occurred the City is willing
to compromise this claim'and accordingly we have agreed with
you to settle the claim on the basis that'you will pay the
City $324.14 in full and complete payment. This will resolve
all claims between you and the City with respect to monies
owing from this particular project.
. ' By carbon copy of this letter I am advising the Public
Works Director of the resolution of this dispute and I am
also advising the Fi,nance-Director that only the sum of $324.14

Your letter of March 4, 1979 suggested converting 76th Ave.
West to a through arterial at State Route 99'.
The City of Edmonds and the Washington State Department of
Transportation solution to the traffic flow in the Highway 99
corridor was completed in 1974.
The City Council approved final plans for this project
with 76th Avenue South of Highway 99,designed and designated
J
as a collector, not as a through arterial.
You might consider an alternate route which consists of
the following:
Southbound on 76th Avenue West left turn to 224th Street S.W.
to 73rd Place West, right turn on 73rd Place West to 228th
Street S.W. left turn on 228th*Street S.W.
Also a southbound lane to 1-5 is being planned from 220th
Street S.W. which will be a shorter route for you to utilize
in the future.

Confirming our phone conversation last week, I would like to recommend that
a study be made to determine the*feasibility of making 76th Avenue West
where it crosses -Highway 99*Aurora into a through arterial as oppbsed to
the present yellow barrier set-up.
I have resided at 20714 76th Avenue West since November 1976, and use the
76th Avenue to a LEFT on 224th - RIGHT on Hwy * 99 - LEFT back onto 76th
and LEFT onto 228th (around past Lake Ballinger Jonto the 236th/Mountlake
Terrace 1-5 on and off ramps) daily. Traffic has increased so significantly
during the*past 6 months along this..route - in both directions; i.e. coming
an . d going - that traffic on Hwy 99 is frequently backed up so extensively
in the left turn lanes back onto 76th from 99-and in the left turn lane off
99 onto 224th that it restricts the traffic flow on 99.
I recommend that the existing blinkingred light at 224th & 76th be eliminated
and STOP signs control the traffic on 224th at the 76th intersection -- i.e.
STOP signs only on 224th with 76th being a through street. The existing
yellow barrier on 99 at 76th should be removed and traffic signals installed
(sychronized with the signals on 99--particularly the one at 224th & 99)
so that traffic on 76th can move straight across 99 and connect with 1-5
either via the 228th (around Lake Ballinger) or 205th accesses.
Traffic in this area will increase - not decrease - in the months ahead with
the completion of numerous.condominiums and by making 76th a through arterial
(i.e. crossing over 99) traffic will flow much better as well as providing the
most direct access to 1-5 (and thereby conserve gasoline).
If I can furnish you with any additional data concerning this request, please
let me hear from you.

your account will be turned over to our Attorney for
collection.

SUBJECT: PAST DUE INVOICE #RW-60033 (JACK KOSTELYK)
Mr. Kostelyk was in my office on March 28, 1977, to dis-
cuss the unpaid balance of Work Order #4718 (copy attached)
in the amount of $624.14 for the installation of an 8-inch
fire line.
The balance is disputed by Mr. Kostelyk since he states
he was given a "firm quote" of $2,322.94 for the complete
installation. However, this amount appears to be an estimate
for materials only, which includes the cost of a meter vault
that was later provided by Mr. Kostelyk.
Since the quote was so underestimated, it prompted me
to contact Larry Billings, a former employee who was Acting
Water/Sewer Foreman at the time. Larry could not recall
exactiv what amount he quoted Mr. Kostelyk, but Larry was
aware that all estimates include time and materials.
Although the total amount for the completed job was
$2,947.08, Mr. Kostelyk refuses to pay any more than was
quoted to him. I attempted to reach a compromise, but without
success.
At this time, I am at a loss as to how we should collect
the unpaid balance. I cannot prove this wasn't the estimated
figure given to Mr. Kostelyk, and I feel we don't have
sufficient information to substantiate our referring this to
the City Attorney.
What are your thoughts?

SUBJECT: PAST DUE INVOICE #RW-60033
On December 17, 1975, Water -personnel began the instal-
lation of a fire line detector check at 20714 76th Avenue
West. On that day, a total of 32 hours straight time and 6
hours overtime was charged to this project. The overtime was
required due to problems which caused a need for a flagman
plus the need to break out and dispose of 12-inch thick
asphalt.
The crew returned on January 6, 1976, to'install the
detector check, valves, and related equipment but was unable
to because the contractor, Mr. Kostelyk, had not yet installed
the meter vault. On January 7, the crew returned and completed
the job. However, in reviewing Work Order #4718 (copy attached),
it appears that the 1.5 hours each for 3 personnel and Unit
#10 on January 6, was not included.
Mr. Kostelyk was in the office several times with Russ
Johnson and was informed the prices received from the City
were only estimates and that the actual co'st could be more or
le . ss, depending on difficulties encountered.
I hope this sheds some light on the subject and hope we
can now collect the outstanding bill from Mr. Kostelyk.
Archie, Ken, and I will meet with Mr. Kostelyk at his conve-
nip-nce on this matter.
