It was noted on March 22, 1991 that the fire inspections for
20909-70th Avenue West were somehow deleted from the inspec-
tion computer program for the year 1989 and 1990. No computer
record or hard copy exists. An inspection was given this date
with one violation (EL-1) found. This location was re-entered
into the inspection program with the next annual inspection
scheduled for July 1, 1991 which is the scheduled date for
this area.

At your request, I followed up with the Department of

Ecology in Redmond, Washington. I spoke to Bridget
Hoover Spill Response for the Department of Ecology.

Ms. Hoover states they responded on March 26, 1991 to

the site. Ms. Hoover states she made a physical

order. Ms. Hoover states she interviewed Mr. Mott and
he states that the substance was vaccumed up and taken
to Chempro. Ms. Hoover has requested that Mr. Mott send
the Department of Ecology a copy of the manifest from
Chempro. Ms. Hoover states that once the Department of
Ecology receives the manifest, the case is closed.

SUBJECT: REPORTED SPILL AT 20909-70TH AVENUE WEST
At 2:00 p.m. on Friday, March 22, 1991, two individuals
contacted my office and reported a discharge of Benzine at the
above address. The two persons making the report were asked
to complete written statements which they did, and I have
attached copies of those statements with this letter. Follow-
ing their report, Assistant Fire Chief Springer and myself
went to 20909-70th Avenue West - the site of the reported
spill. We made contact at the front office with a reception-
ist who identified herself as Mrs. Mott. We advised her of
our reason for being there and she indicated to us that she
was aware of the reported incident and at our request she
allowed us to walk through the facility. During our walk
through, we noted a generally clean operation within the build-
ing. There are two divisions within this building; one that
primarily works on removing the soot from heating units, while
the other reportedly works on commercial accounts ranging from
catch basins to unspecified situations. Our outside inspec-
tion indicated several questionable areas on the paved area
and in the green belt immediately east of the parking area.
On the southeast corner of the property there is a low spot in
the green belt that contained a brackish appearing liquid.
The area around a storm drain grate has staining that appears
to be petroleum in nature and adjacent to the fuel pumps in
the northeast corner of the property there is an overflow of
what appeared to be used motor oil. This concluded our inspec-
tion of the site. No samples were taken.
Since your agency was notified at 2:16 p.m. on March 22,
1991, nothing has significantly changed on the site. I am
turning this matter over to you for appropriate action.

Please refer to your on -site visit on March 26, 1991 regarding a complaint from
the City of Edmonds Fire Marshall's report on benezene spill. You looked over
our facilities and observed that the spill had been cleaned up. My operations
foreman, Erik Madsen and 1, reported to you all the steps taken to solve the
roblem. I stated I would get a written report from Mr. Madsen and a copy of the
ill of lading manifest on dumping of water at Chemical Processors, Kent, WA.
Both the copy of the written report and the copy of the B/L manifest are attached.
In summary, the facts are these:
1 ) The charcoal, which was generated by BP Oil, Tacoma, WA, was
transported to a designated receiving facility in Yakima, WA. All materials
were handled under a uniform hazardous waste manifest. Material was
also tested (T CLP) for concentrations, PQL and minimum concentration.
It is my understanding that the charcoal contained only traces of
benezene and was marginally hazardous.
2) The operator was instructed to thoroughly clean the truck at Yakima.
However, he did not completely clean the truck chutes resulting in receipt
of some residue at our site.
3) Both employees, Lennie Parsons and Tony Dupont were specifically
advised: 1) What the material was, 2) How to dress for handling the
material, 3) How to handle the material properly, and 4) How to clean the
truck. Both employees disobeyed instructions and, after bagging the
material, used a water hose to clean out the truck and the tarmac. They
told Mr. Madsen what they had done after they had completed the work..
4) Following this incident, we added approximately 1000 gallons of water to
the catch basin affected, the out flow line and the pond at the southeastern
edge of the property. We vacuumed the water, containing, negligible
amounts of benezene and transported the water and material to Chemical
Processors, Kent, WA.
5) Because these two employees failed to follow proper instructions, this
matter constituted partial grounds for their termination of employment.
Both employees were unsatisfactory workers and became disgruntled

because they were already warned to improve their work attitudes and
performances.
We deeply regret this incident. Our company took every possible precaution to
avoid this problem. After it occurred, we tooK the necessary steps to clean up the
mess and eliminate the problem.

3. An extensive amount of grading is involved for this project. Of
particular concern is the tributary drainage from this site affecting the
adjacent property. Also of concern is the use of rockeries as retaining
structures. A soils engineer should show that the soil under the rockeries
is stable without them or a design should be submitted. Also, the rockeries
extend over the property line and affect the adjacent property owner.
Sec. 2903.
4. The type of fill material, compaction requirements, and steepness of
the slopes should also be shown. Sec. 7010.
5. You should check that this building complies with the state disabled
person requirements. As an assistance, I enclose a guide.
6. The warehouse is tall enough such that high piled stock could be
present. You should ascertain exactly what will be stored because the
requirements of UFC Article 81 should be installed now if required.
7. The area separation wall:
a. Even with this wall, two 25' yards should be reserved on the east
and west sides of the building for area justification of the larger
portion of the building. Sec. 506(a)l.
b. The exact composition of the 2 hour construction should be designated.
How is the exterior portion protected against the weather?
c. How is protection behind the electrical panels provided? Sec. 3304(e).
d. The wall should be shown extending to the roof sheathing. Sec.
505(e)3, Ex 2. But how will the gaps at the corrugation be filled?
8. The exact material should be designated on sheet 4 to provide the
waterproof wainscot in the toilet rooms. Sec. 510(b).
9. The material should also be designated to provide a similar finish in the
shower itself and it should be shown extending at least 70" above the drain
inlet.
10. Will plumbing be installed for the toilet room in the tenant space? Sec.
705.
11. The light and ventilation should be justified for this building. Sec. 705.

12. Exits:
a The exterior exit doors should all be openable from the inside
without the use of key special knowledge or effort. Sec. 3304(c).
b. No more than .2111 threshold should be specified at the main entrances
nor 11" elsewhere. Sec. 3304(h).
c. An 11" run should be provided on the mezzanine stairway. Sec. 3306.
d. The handrails should be rounded and grippable (1-21-211 in diameter)
and should be returned at walls at their ends. See also the
Disabled Persons Amendments.
e. A grippable handrail is also required on the open side of the
stairway.
f. This open side should either be solid or have a spaced intermediate
such that no 611 diameter object should pass through. Sec. 1711.
g. The method of attaching a stair guardrail in the mezzanine one
shown on sheet 8 should also be shown. Sec. 1711.
h. The method of attaching the 4 x 12 stringer to the double 2 x 12
headers should be shown.
13. The details of the textured finish over the plywood on the exterior
should be given in order to ascertain that weather protection is provided.
Sec. 1707.
14. Vertical fire blocking at 10' on center measured horizontally should be
used at the wainscot. Details are shown on sheet 8. Sec. 2516(f).
15. Labelled impact glazing should be specified for and provided in the
doors and windows immediately adjacent to the doors. Sec. 5406. If the
shower door is glass or plastic, it should also be impact glazing.
16. if the building is to be heated or cooled, the details of the mechanical
equipment should be shown and we will check it for compliance with the
Uniform Mechanical Code at that time.
17. The designer should show how the building complies with the state
energy code. Note that the vinyl insulation should have flame spread and
smoke develop ratings of 75 and 450 or less respectively. Sec. 1713.

STRUCTURAL COMMENTS
I - Quality control:
a. The tail specifications for the steel building and connectors should
be shown.
b. Special continuous inspection is required for the high strength
bolts and welding.
C. Welders both in the field and in the plant should be certified by a
third party quality control agency such as those licensed by the
Washington Association of Building Officials (WABO).
d. You should determine if Sun Steel Building Company is a plant
which complies with the minimum requirements of Sec. 306(f).
e. A soil report may be required to prove the assumed design soil
bearing of 2000 psf. Sec. 2905. At any rate, compaction reports
should be required.
2. A guide (showing what the symbols mean, what the input and output is,
what the design formulas are, etc.) should be provided for the computer
calculations in order for us to determine if the proper design values were used.
3. The exact details that apply to this building only should be circled on
the general detailed plans and/or be cross referenced to the 11 sheet set of
plans.
4. The wind pressure design does not seem to comply with the new 185
code requirements of Sec. 2311. A redesign may be required. The designer
should also prove that the building is stable at the rigid frame against a
1i times wind load. Sec. 2311(c).
S. The parts and components of the building should be studied for the
large localized values of wind pressure induced especially for cla*dding
connections. Sec. 2311(f).
6. The anchor bolt values appear to exceed the allowable ones given in
Table 26-G for shear and uplift. Attached weld plates to the bolts could be
used by compression against the concrete and could resist the loads.
7. The retaining wall between the two parts of the building, detail 3/5,
should be designed for a surcharge loading assuming that storage is provided
in the tenant space.

8. It is important that the reinforcement in the 4" slab be actually placed
accurately in the center of the slab to resist lateral an.d uplift loads.
Therefore, special inspection may be necessary as wire mesh is usually just
stepped upon as they Ire pouring the concrete and only perfunctory attention
is paid to placing it somewhere in the slab.
An invoice for this plan checking service will be forwarded to you under
separate cover based on the valuation of $297,000.
I am retaining all data and plans in anticipation of telephone calls from the
designer and also that I will be doing the recheck. Please instruct the
designer to indicate on a separate sheet on which sheet or detail the

Description
The D8112 Control/Communicator
is an eight zone microprocessor
based control panel with an integral
digital communicator. Through pro-
grarnming and accessory modules,
the 08112 offers a degree of
flexibility rarely found in other fire
alarm control systems.
The D8112 is U.L. listed as power
limited, therefore (per Article 760 of
the National Electrical Code ) U.L.
listed fire cable can be used for ext-
emal circuits, rather than expensive
conduit. This feature alone can
eff ect substantial installation cost
savings.
All eight zones are individually
programmable and may be used to
monitor all types of fire alarm,
sprinkler supervision, fixed extin-
guishing systems, industrial pro-
cess supervision, and burglar alarm
devices. The eight master zones of
the D8112 may be expanded to
134 individually annunciated zones
or points of protection through the
addi&n of an active multiplex zone
expansion system ("ZONEX").
Zone Expansion: With the
ZONEX system, alarm initiating
devices or other sensors are
connected to remotely located
transponder modules (D81 26U
POPIT). Using a two wire circuit, the
POPITs are in continuous two-way
communicaton with an expansion
module (D8125 POPEX) connect-
ed to the 08112. Each POPIT is
individually programmed to report to
a master zone on the D8112 allow-
ing it to be connected at any point in
the circuit without regard to a
specific sequence. This format pro-
vides virtually unlimited flexibility
and permits additions, deletions,
and changes in types of sensing
devices without requiring expen-
sive circuit revisions.

ations. The Alpha 11 is streamlined in appearance and power packed with all the functionality of the original
Alpha Command Module ... and much more.
Command Control
The Alpha 11 Command Module does more than just arm and disarm the security systerii.,SeVeral system
commands can be controlled by the Alpha 11. The Command Key on the Alpha I I can initiate master arming,
instant or delayed perimeter arming, selective zone arming, force arming, local system test, and more. Justby
combining the Command Key with one or two other keys, the Alpha 11 controls theentire Omegalarm security
system. The Alpha 11 can also allow the user to transmit test reports to the central station. Two programmable
"alarm" keys can signal for medical attention or police, and the user can initiate a silent duress alarm when
disarming the system.
Programmable Function Keys
The Alpha 11 has four function keys that can be programmed to perform any system command function
(other than arming commands) with the touch of one key. "One -touch" keystroke entry is ideal for the more
common commands such as sensor reset, system test, buzzer reset, or perimeter watch mode. The function
keys may also be programmed for limited access, allowing them to be used only by authorized personnel who

English Language Display
The Alpha 11's display is a 16-character position English language readout, giving the end -user instant
identification of system status. No guesswork involved. Programmable displays for each protective zone (and
each point of protection for ZONEX systems) give your company's installations that special customized touch.
The display used by the Alpha 11 is a power conserving Vacuum Fluorescent Display (VFD). Each display can
represent any character of the alphabet, numerals from 0 to 9, and several punctuation marks.
Because the Alpha 11 is compatible with several different Ornegalarm Control/Communicator models, the
display can be switched to either an 8-character mode or a 16-character mode. This isdonewhile programming
the Control.
Electronic Tones
Response tones from the Alpha 11 Command Module are emitted from a micro -speaker capable of five dis-
tinctive tones: 1) a muted "beep" tone which sounds for each key touch received by the control/communicator,
2) a prewarning "beeper" to remind the user to disarm the system during entry delay, 3) a short "chirp" tone
sounds when the system is monitoring the perimeter of the building, to alert of perimeter activity, 4) a pulsing,
high pitched "bell" tone to warn of fire, and 5) a steady, high pitched "bell" tone to warn of non -fire (burglary)
conditions. Interior slide switches allow certain response tones and the Command Keyto be disabled at indivi-
dual arming stations.
Installing the Alpha 11 is simple, quick and cost effective. A four -conductor cable is all that is required
between the control panel and the Alpha 11 Command Module. Since the Alpha 11 is a low profile surface mount
unit, it is adaptable to virtually any application; giving -you the look of a flush mount installation without the
labor.

D192 Bell Circult.Supervision Module

Description
The D192 Bell Circuit Supervision Module is
used with the Omegalarm D6112 Control/
Communicator to supervise wiring from the
08112 to remote fire alarm signaling device.
The wiring is supervised for open circuit,
shorted circuit or grounded circuit. The D1 92
Module is also used as a filter to stabilize the
bell output voltage of the D8112 in the event
that battery power is interrupted.
The D192 Bell Circuit Supervision Module is
designed to use only low -current polarized
(DC) signaling devices. See "Specifications"
for Omegalarm fire bell part numbers. Other
types of fire alarm signaling devices used with
the D129 Module must concur with the fol-
lowing parameters:
A) Must be polarlized (DC) equipment.
B) Must operate using 12 VDC.
C) Current draw of device(s) must be limited
to D8112 alarm output power parameters.
Operation
During normal operation the bell circuit is wired in series with the end -of -line resistor (part #15-03130-001) provided with
the 0192 Module. The 08112 Control/Communicator supervises the bell circuit with a protective zone dedicated to
reporting bell circuit trouble. The zone used for supervision must be programmed to report TROUBLE when the circuit is
shorted or opened (see "8112 Programming Checklist").
When the D61 12 Cohtrol/Communicator detects a fire alarm condition, the D1 92 Module provides 12VDC to fire alarm
bells and/or other fire alarm signaling devices. The voltage provided to the fire alarm signaling devices is supplied by the
D811 2 alarm- output power supply.
Silence Switch
The D1 92 Module has a toggle switch used to silence the fire alarm bell while testing the unit. When the toggle switch is
put in the silence mode, the D192 presents an open circuit to the zone input of the D8112 Control/Communicator,
causing the D8112to transmit a TROUBLE report (if programmed forthis response). When the D192 Module is mounted
in the D8112 enclosure the toggle switch cannot be used without access to the interior of the enclosure.

Installation Checklist
1. Mount the D192 Module inside the Omegalarm D8112 enclosure using the mounting screws provided.
2. Connect the terminal 1 on the 0192 Module to terminal 3 on the D8112 Control/Communicator.
3. Connect terminal 3 on the 0192 Module to terminal 4 on the D8112 Control/Communicator.
4. Connect terminal 5 on the D192 Module to a protective zone input terminal on the D8112 Control/Communicator.

D128 Dual Phone Line Switcher
Operation and Installation Instructions
Description
The Omegalarm D8112 Control/Communicator uses the Model D128 Dual Phone Line Switcher to transmit over a
second phone line when the primary phone line is determined to be inoperable. The D128 module allows the 08112 to
periodically test the phone lines for usage and integrity and to report to the central station (via the working phone line)
when a phone line is faulty. As the D1 28 switches between telephone lines during testing, a slight clicking sound will be
emitted from the D128. This sound is generated from the relays switching the phone lines. The D128 is protected from
high voltage transients on the phone lines by gas discharge tubes.
Operation
The D128always usesthe primary phone lineto initiatecalls (except in AutomaticTest Reports, see8112 Program Entry
Guide for details). If the primary line is used for two attempted calls and fails to reach the central station; or if the primary
line is detected by the D8112 to be faulty, the D8112 switches to the second line and adds to the original report a
telephone line trouble report. The primary line rep
,prts as ZONE B and the secondary line reports as ZONE C.
Program Execution Failure Annunciation
The D128 module has a circuit that monitors the D8112 Control/Communicator's CPU (Central Processing Unit). If the
D8112's CPU fails to execute its program correctly, the D128 will sound an audible trouble signal. This signal cannot be
reset while the CPU is failed.
Status LEDs
Four LEDs mounted on the front edge of the 0128 module display primary phone line failure, secondary phone line
failure, failure to communicate, and AC power status. The D8112 must be programmed to drive the LED display of the
D128 module. See program item #14 PhLite. When programmed and operating normally, only the green AC power status
LED should be lighted.
Phone Line Failure LEDs
An amber phone line status LED lights on the D128 module when the D8112 Control/Communicator detects less than
25VOC or 10mA of current across tip and ring of the telphone line. The 08112 monitors the faulty telephone line for a
programmable interval (program item #11 PhSupv) before indicating a trouble condition.
Failure to Communicate LED
An amber LED lights -when the D8112 Control/Communicator has attempted ten times to communicate with the central
station, but has failed. The LED is cleared by entering the arm/disarm combination of the 08112 Control/Communicator.
AC Power Status LED
The green AC power status LEO is lighted when the D8112 Control/Communicator is running on AC power. When the
D8112 is not running on AC power the LED is not lighted.
Phone Cord Supervision
The three phone cords used to connect the 0128 module to the D8112 Control/Communicator and to the telephone
system can be supervised for proper connection when using RJ38X Telephone Jacks. With the RJ38X jacks and the
phone cords installed, terminal 10 of the D8112 Control is a continuity referencewhich can be wired in series with a 1000
ohm resistor and connected to a protective zone input. When any of the three phone cords are removed from the jacks,
the protective zone is faulted (opened). The protective zone used for phone cord supervision should be programmed
with a zone code as follows-

Installation Checklist
M 1. The Model D1 28 Dual Phone Line Switcher mounts inside the D8112 universal enclosure on the lower right hand
side. Fasten the module to the inside of the enclosure with the screws provided.
M 2. The D1 28 ha� two flying leads. Connect the green lead to either Ter minal 1 pr Terminal 2 on the D8112 for AC
power.
EJ 3. Connect the black lead on the D128 to Terminal 4 on the D8112. This provides transient protection for the two
incoming phone lines. This is also ground reference for the AC LED.
M 4. Connect the ribbon cable to the connecting jack (J-4) on the D128 and to the D8112 accessory connector jack
(J-26) on the lower right side of the D8112 faceplate. The cable only fits one way.
M 5. Connect the primary phone line to the J-1 phone jack on the D128 module.
6. Plug the secondary phone line into the J-2 jack on the D128.
7. Plug the short (2 ft.) phone connector cord (provided with the D128 package) into J-3 on the D128 and into the
telephone cord connector jack on the lower left corner of the D8112 faceplate.

Description
This series of heat detectors
provides high quality, reliability, and the
ultimate in design and decor. The low
silhouette and pure white finish blend with
practically all ceiling designs to provide an
unobtrusive unit.
IMPORTANT NOTE: Independent studies
indicate that heat detectors should be
used when property protection alone is
involved. In cases where life safety is a
factor, smoke detectors should be used.
Heat Detectors are available in the 135
and 194 degree Fahrenheit ratings; in
fixed, or fixed and rate -of -rise categories;
single or two circuit configurations; and
with normally open or normally closed, or
one normally open and one normally
c osed contacts
The Heat Detector is available with either a
metal or a white plastic mounting plate.

The metal plate can be used for either
flush or surface mounting whereas the
plastic plate is designed specifically for
surface mounting of the detector. The
plastic plate mold includes the white skirt
which covers the space between detector
base and the mounting surface.
The Specification Table lists those
detectors whirh are available with a 1 OK
ohm or 560 ohm resistor in series with the
alarm contact.
IMPORTANT NOTE: The rate -of -rise
feature on heat detectors may be subject
to reduced sensitivity over time. There-
fore, the manufacturer recommends
testing the rate -of -rise feature at least
once per year by qualified fire protection
specialists.
Contact ratings on all numbers are 3 amps

The 1800 series are system type
products of combustion detectors
designed for use with two -wire UL Listed
control panels or 4/6 wire systems.
Drawing less than 100 microamps
during normal operation, the detectors
can operate over a broad voltage range
of 10 to 32 VDC. The dual chamber uni-
polar ionization sensor detects both
visible and invisible smoke. The sensitiv-
ity of the 1800 is preset at the factory
and does not require adjustment in the
field. The 1851 can be field adjusted. A
solid state voltage regulator maintains
detector sensitivity over a wide range of
input voltages. A visual indication of an
alarm is given by an LED on the detec-
tor cover. The visible alarm signal may
be remoted by using the 669 series
remote alarm lamp assemblies. Incorpo-
rated within the detector is a sensitivity
test switch which makes troubleshooting
easy witHbut sophisticated test equip-
ment. Field wiring connects to easy-to-
wire terminals or mounting base, simpli-
fying installation. Easy to install, clean
and maintain, the 1800 is designed for
direct surface mounting (mounting
bracket included), or mounting to a
4' octagon or smaller box. The 1851
plugs into a series of mounting bases.
Installation
Units are mounted directly to a
4 ' octagon box or plaster ring; or with
adapter strap to 4 ' square box or
plaster ring.

matic Fire Detectors" for spacing, loca-
tion of detectors and other guidelines.

Maintenance
The new 1800 series has been
designed for minimum maintenance. An-
nual cleaning of dust from the detector
head with a vacUum cleaner is recom-
mended. Individual environments may
require a more frequent cleaning pro-
gram. Installation instructions furnished
with the detector provide detailed
guidance.

Specifications
The detectors shall have a dual
chamber ionization sensor of the uni-
polar type. The sensor shall have a
nominal sensitivity of 1.5 % ft. as mea-
sured in a UL smoke box. It shall be pos-
sible to perform a calibrated sensitivity
and performance test on the detector
without the need of generating smoke.
The test method shall test all detector
circuits.
The detector shall incorporate a
solid state voltage regulator which can
maintain detection sensitivity over an
input voltage range of 10-32 VDC. Stand-

by current shall be no more than 100
microamps. Current limiting shall protect
the detector against power surges and
noise protection circuitry shall protect
the detector so it can be wired without
conduit, where codes allow.
The detector shall have a mounting
bracket or base that allows for direct
surface mounting or mounting to a
4 * octagon box.
A visual indication of an alarm shall
be provided by a latching light emitting
diode (LED) on the detector which may
be seen at ground level. The visible
alarm signal shall be capable of remote
LED annunciation.
A special test meter shall be avail-
able to precisely check the calibration
and sensitivity of the detector. Metering
points for the test meter shall be acces-
sible on the exterior of the detector.
The detector shall not alarm when
exposed to wind velocities up to 2500
feet per minute. The detector's sensing
chamber shall incorporate less than one
microcurie of AM241.

APPLICATION RESTRICTIONS. All fire
detectors/ sensors, regardless of the
principle of operation (heat, ionization,
photoelectric, ultraviolet, infrared) have
their own unique operating characteristics
and application restrictions. All detectors
interact with and respond to the environ-
ment in which they are installed. Failure to
be aware of these operating characteristics
can result in mis-application and unwanted
alarms. For example, ultraviolet flame

detectors can be triggered by sunlight and
smoke detectors can be triggered by
aerosols other than from a fire.
With proper and experienced review of the
area to be protected and the selection of
the proper sensor for the application the
user will have years of satisfactory and

for operational characteristics and restric-

Operation
The Gentex GX-90S is a high -quality remote
signaling device that combines both an audible
and visual alarm in a modular package. The solid-
state piezo horn is rated 90 d8 at 10 feet, and
the high -intensity strobe utilizes a Xenon flash
tube which generates a high -intensity light visible
from all sides of the lens.
Available in select colors, the GX-90S is
intended to provide visual as well as an audible
evacuation signal for all personnel, including
those with hearing impairments, making it ideal
for hotels, apartments, hospitals, and rest homes.
The GX-90S is polarized for use on supervised
systems, and the high -impact, grain -finished
plastic housing permits attractive flush mounting.
Unlike expensive electromechanical horn/
strobe combinations, the GX-90S draws only
60 mA at 24 VDC. The device meets Underwriters'
Laboratories requirements for use with Fire
Protective Signaling systems and is warranted
for one (1) year from date of purchase.
