The purpose of the Checklist is to enable City staff to determine whether any potential Critical Areas are or may be present on the subject property. The information needed to complete the Checklist should be easily available from observations of the site or data available at City Hall (Critical Areas inventories, maps, or soil surveys). An applicant, or his/her representative, must fill out the checklist, sign and date it, RECEIVED SEP 10 1993 PERMIT COUNTER and submit it to the City. The City will review the checklist, make a precursory site visit, and make a determination of the subsequent steps necessary to complete a development permit application. With a signed copy of this form, the applicant should also submit a vicinity'map or plot plan for individual lots of the parcel with enough detail that City staff can find and identify the subject parcel(s). In addition, the applicant shall include other pertinent information (e.g. site plan, topography map, etc.) or studies in conjunction with this Checklist to assist staff in completing their preliminary assessment of the site. I have completed the attached Critical Area Checklist and attest that the answers provided are factual, to the best of my knowledge (fill out the appropriate column below). HAHN DIRECTOR We received your May 8, 1990 letter and concur with your recommendation that a new waterline be installed. We will consider your letter as permission to work within the private drive. The City crews will schedule the installation of a new pipeline this year. The exact schedule is not known at this time. The City's contact persons for this project will be Bobby Mills and Archie Brena. Please feel free to contact them at your convenience. Sincerely, ROBERT J. ALBERTS, P.E. We reside on 86th Place West, which we are aware of is a private street. Our water problem has been an ongoing dilemma for us. The severely rusted and corroded main water line has reached a point where your city crews are spending more and more time flushing out the lines to help clean it out. It hasn't been helping the water quality however. We find our white landry coming out with rust stains. We are forced to buy bottled water to avoid drinking the "brown" water, which is an expensive substitute. Our bathroom fixtures are water stained with no hope in sight i . A few of us who are looking at home additions and remodeling face the inevitable staining when our new fixtures are installed. We are discouraged to invest money in our homes with this in mind. Our fear is that we would not be able to recover our investment due to poor water quality. Our request is that you take into consideration the replacing of our water main for our 6 homes on 86th Place West. This would eliminate the necessity of having city crews come out to continuously maintain the pipes. It would also enable us to feel safe using our water for drinking purposes once again. We look forward to hearing from you. Please contact Doug Reilly at 778-5993 or 344-6688(work).
