permit to the City.
'The purpose of the Checklist is to enable
City staff to determine whether any
potential Critical Areas are or may be
present on the subject property.' The
information needed to complete the
Checklist should be easily available from
observations of the site or data available at
City Hall (Critical Areas inventories, maps,
or soil surveys).
An applicant, or his/her representative,
must fill out the checklist, sign and date it,
and submitit to the City. ne City will
review the checklist, make a precursory site
visit, and make a determination of the ' -
subsequent steps necessary to complete a
development permit applicati on.
With a signed copy of this form, the
applicant should also submit a *vicinity map
or plot plan for individual lotsof the parcel
with enough detail that City staff can find
and identify the subject parcel(s). In

In response to a citizens complaint, the City Building Inspector made a site visit to
property you own at 21601 85th Place West in Edmonds onApril 30th, 1996. A deck
was observed under construction and a Correction Notice was subsequently posted
notifying you of the requirement to obtain a building permit by May 10th, 1996. As . of
today the City has not received a response from you regarding this matter. Therefore, as
required by Section 106.2 of the Uniform Building Code you must submit for the required
permit by May 24th, 1996. Another option is to completely remove the deck and secure
any opening violations and recall for inspection.
Failure to respond shall result in the start of the Civil Penalties process whereby fines of up
to $100 per day may be assessed against you until this matter is resolved. It is not the
City's intention to cause you undue hardship, however, compliance to the codes is
mandatory to -ensure the fair and equal treatment of all citizens. The City's applicable
handouts are enclosed for your convenience in making a complete permit submittal.
