Executive Summary
This report provides the results of an RF power density analysis performed on T-
Mobile's utility pole site SE01 575A.
Current Site Confizuration:
The Site's current configuration is as follows: Three sector site operating with technology
including GSM in PCS band (1900 MHz), UNITS in PCS band (1900 MHz), and UMTS*
in AWS band (2 100 MHz).
Proposed Site Configuration:
The Site's proposed upgrade is as follows: Installation and operation of additional LTE
technology, Long Term Evolution (LTE) in the 700 and 2100 MHz bands. UNITS in the
AWS band (2100 MHz band) will be removed.
The proposed physical modifications will result in replacement of radio hardware and
antennas at this site.
Site Description:
Site Type and Height- Utility Pole, elevation of 93. V
Equipment Location- Adjacent to the utility pole, inside a shelter.
Other Antennas- This is a T_-Mobile only site that does not host 'any other
antennas.
Access- Access to the utility pole is open to the general public, the equipment is
inside a locked shelter.
Study Reference Location- This study examines the power density environment at
ground and rooftop level from ALL sources of RF. -
Existing Signage- There is blue RF signage at the base of the utility pole.
RF Environment- Since the area on the ground and the adjacent rooftops is open
to general public, for purposes of evaluating compliance with FCC OET Bulletin
65, the area is determined as a General Public environment.,
AnalyaLis:
The analysis of the RF power density at this site indicates that emissions on the rooftop of
adjacent buildings will not exceed the FCC's Maximum Permissible Exposure (MPE)
limit for general/public environments. Ground level exposure is calculated to be less than
5% of the FCC MPE limit for general public environments. No further mitigation is
required.

Conclusion and Recommendations:
0 The results of the analysis indicate that the power density levels at ground and rooftop
of the, nearby buildings will not exceed FCC MPE limit with the proposed antenna plan.
- The risk of accidental exposure from these antennas is being mitigated by an RF alert
sign placed at the base of the utility pole which is visible upon approaching to it.
- This site will be operating in general compliance with FCC OET Bulletin 65 following
the implementation of the changes outlined in this report. No further mitigation is
required.

Introduction
The purpose of this analysis and report is to evaluate the cumulative power density levels
of all antennas located on the utility pole and identify any areas of concern that require
mitigation. This report also assesses the site's compliance with FCC OET Bulletin 65;
"Guidelines for Human Exposure to Radio -frequency Electromagnetic Fields".
As part of this analysis, the site was visited on 3/23/2015, to perform a complete field
survey. The purpose of the survey was to verify antennas located on the utility pole and
identify co -located wireless facilities.
The power density simulation performed for this site utilized Roofviewo analysis
software. All antennas were assigned an operating frequency and transmit power and
were deemed to be operating at 100% of their rated output power.

Predictive Analysis Details
For purposes of this analysis, RoofView@ was configured to provide an output based on
the appropriate MPE limit(s) published in the FCC's guidelines. The antenna information
was loaded into RoofView@, an MPE predictive analysis tool by Richard Tell and
Associates. The analysis was run for both the ground and rooftop levels.
The map in Exhibit 3-1 is a graphical representation of the predictive emissions on the
ground. The map in Exhibit 3-2 is a graphical representation of the predictive emissions
at rooftop of the nearby buildings. The solid green shading indicates that power density
levels are expected to be less than 5% of the applicable maximum permissible exposure
(MPE) level in these areas. Other colors are defined in the legend.

Appendix A: FCC Compliance and RF Safety Policies
In August of 1997, the FCC published OET Bulletin 65 Edition 97-01 to regulate
methods for evaluating compliance with FCC guidelines for human exposure to
radiofrequency (RF) electromagnetic fields. The FCC guidelines for human exposure to
RF electromagnetic fields incorporate two categories of limits; namely "Controlled"
(a.k.a. Occupational) and "Uncontrolled" (a.k.a. General Public). The guidelines offer
suggested methods for evaluating fixed RF transmitters to insure that the controlled and
uncontrolled limits deemed safe by the FC for human exposure are not exceeded.
OET Bulletin 65 recommended guidelines are intended to allow an applicant to "make a
reasonably quick determination as to whether a proposed facility is in compliance with
the limits." In addition, the guidelines offer alternate supplementary considerations and
procedures such as field measurements and more detailed analysis that should be used for
multiple emitter situations.
These guidelines define RF as emissions in the frequency range of 300 kHz to 100 GHz.
The FCC define Maximum Permissible Exposure (MPE) limits within this frequency
range based on limits recommended by the National Council on Radiation Protection and
Measurement, the Institute of.Electrical and Electronics Engineers, LCC. (IEEE), and by
the American National Standards Institute (ANSI).

The FCC states that "Occupational/Controlled limits apply in situations in which persons
are exposed as a consequence of their employment provided those persons are fully
aware of the potential for exposure and can exercise control over their exposure. Limits
for occupational/controlled exposure also apply in situations when an individual is
transient through a location where occupational/controlled limits apply provided he or
she is made aware of the potential for exposure."

For General Population/Uncontrolled limits, the FCC states that "General
populatibn/uncontrolled exposures apply in situations in which the general public may be
exposed, or in which persons that are exposed as a consequence of their employment may
not fully be aware of the potential for exposure or cannot exercise control over their
exposure."
For purposes of this analysis, all limits are evaluated against the Power Density limits.
Typical guidelines for determining whether occupational/controlled limits can be applied
include insuring the environment (such as a rooftop) as limited/controlled access via
locked doors or physical barrier that are preferably controlled by a landlord that is aware
of the situation and can inform anyone going through the locked door of the. existence of
the RF emissions. Such notification/awareness is typically accomplished by means of
signage on the door, or other access to the area of concern, as well as signage on or near
the antennas. Examples of such signs include the following:

Appendix B: Overview of RoofView@ Functions and
Assumptions
RoofView@ is a tool develope'd and supported by Richard Tell Associates, LCC. to be
used for analysis of RF field levels at telecommunications sites produced by antennas of
the type commonly used in cellular, paging, SMR, PCS and two-way radio
communications services. Although its name suggests that the tool is only for use in
evaluating emissions for roof top applications, it can also be used to evaluate ground
level effects of tower facilities.
RoofView@ allows the user to apply near field, far field, or a combination of near and far
field computational methods as desired by the user. For this analysis, near field
computations are used for areas within the near field, and far field computations are used
beyond the near field. Specific break points are dynamic based on the aperture of the
antenna being analyzed.
The near field methodology is based on a cylindrical model that assumes the power into
an antenna is distributed as a cylinder�around the aperture of the antenna. Research by
Richard Tell Associates, LCC. found that using such a model, along with corrections for
height and antenna pattern, is very accurate, if not slightly conservative in estimating RF
exposure. FCC Bulletin 65 recognizes the use of the cylindrical model for near field

Each bin's results are then also adjusted by spatially averaging the portion of a 6 foot tall
human that intercepts the aperture over 6 feet. Once the antenna is completely above (or
below) the height that corresponds to a 6 foot tall human, the cylindrical results are

reduced to 10% of their results and then dissipated inversely in proportion to the square
of the distance.
Once bins being analyzed fall outside of the near field (as determined by a method and
variable that is user -selectable; see below for method and variable used in this analysis), a
far -field spatial average is calculated. Spatially averaged power density in the far -field is
calculated by reducing the spatially averaged power density inversely, by the square of
the distance from the antenna(s).

Structural Mount Analysis for Proposed Antenna and Equipment Installation
Dear Bryan,
Per your request, we have reviewed the construction drawings prepared by your office for the proposed removal of
(3) existing panel antennas and (3) existing tower mounted amplifiers and the addition of (3) new panel antennas
and (3) new remote radio units, attached to an existing monopole. Each of the (3) panel antennas is attached to a
crossover plate on an 8" standoff arm mounted to a universal ring mount installed at the top of the pole; similarly,
the RRUs are mounted with 8" standoff arms to a second universal ring mount a few feet below the top of the pole.
In addition to reviewing the proposed construction drawings, we have reviewed as -built construction drawings of the
existing antenna configuration prepared by B.J. Thomas, P.E. (Dated: October 5, 2012) and site walk photographs
(no time stamp) of the existing antenna mount assembly provided to us by your office. Please be advised as follows:
Based upon our calculations and review of the documents noted above, we conclude that the antenna mount
assembly, as detailed on construction drawings, is adequate to support the loads associated with the proposed
antennas and equipment. Supporting calculations for the gravity and lateral loading on the proposed antennas
and Valmont structural analysis letters verifying the capacity of the existing universal ring mount (Valmont
part#: LWRM) and 8" standoff arm (Valmont part#: MMO 1) are attached.
The recommendations above are provided based upon calculations prepared by our office and information the client
provided. A representative from Vector has not visited the site. The analysis and conclusions described above are
limited to the members included in this report only and are based on the assumption that the structure was properly
designed and constructed. Analysis of the capacity of the existing monopole and foundation are not included in our
scope and are to be completed by others prior to modification of the loading configuration. Vector Structural
Engineering makes no claim as to the correctness of the original design or the current condition of the structure,
which is assumed to be in good condition, free of damage or deterioration. The contractor shall verify the suitability
of the existing mounts and the condition of the existing structure and shall notify Vector immediately should any
damage, deterioration or discrepancies between the as -built condition of the existing structure and the assumed
condition described in this report and on the construction drawings be found.

The mount was analyzed for two (2) antenna loads evenly spaced across each face of the
mount, centered on the centerline of the mount (i.e. no vertical eccentricity). Based on the
Design Criteria above, the mount capacity is 2,140 lbs (67.43 Sq-ft). The maximum normal
force per antenna pipe is 1,070lbs (33.71 Sq-ft) with a maximum tangential force of 532 lbs
(16.76 Sq-ft). The weight of each antenna was considered to be a maximum of 250 lbs. Self -
weight of the mount was also considered.
The mount will also support a nominal live load of 250 lbs at one (1) location to provide
access for a climber. This condition assumes no wind.

Modeling & Applied Upurtenance Loading ScenArio 1
The Ring Mount Assembly (LWRM) was analyzed with an evenly distributed load on
each Ring Mount weldment (X-LvVW with an 18" moment arm. Based on the Des' ign
Criteria above, the maximum acceptable normal force is 5,060 lbs (159 sq ft) and the
maximum acceptable tangential force is 3,200 lbs (100 sq ft) per weldment. The allowable
applied weight (mount plus antenna) was considered to be a maximum of 1,700 lbs per
weldment. Self -weight of the assembly was also considered.
Modeling & Applied Appurtenance Loading Scen rio 2
The Ring Mount Assembly (LXVW was analyzed with an evenly distributed load on
each Ring Mount weldment (X-LVVW with a 36" moment arm. Based on the Design
Criteria above, the maximum acceptable normal force is 2,530 lbs (79 sq ft) and the maximum
acceptable tangential force is 1,600 (50 sq ft) lbs per weldment. The allowable applied weight
(mount plus antenna) was considered to be a maximum of 860 lbs per weldment. Self -weight
of the assembly was also considered.
Modeling & Applied Upurtenance Loading Scen rio 3
The Ring Mount Assembly (LXXTM) was analyzed with an evenly distributed load on
each Ring Mount weldment (X-LVVTd\/I) with a 48" moment arm. Based on the Design
Criteria above, the maximum acceptable normal force is 1,900 lbs (59 sq ft) and the maximum
acceptable tangential force is 1,200 lbs (37 sq ft) per weldment. The allowable applied weight
(mount plus antenna) was considered to be a maximum of 650 lbs per weldment. Self -weight
of the assembly was also considered.
Live load Rating:
The mount will also support a nominal load of 5,000 lbs at a position less than 6"
from the mounting surface (climber anchorage or safety climb). This condition assumes
no wind without additional loading from mounts or antennas.
This mount will also support a norruinal. load of 500 lbs located at two (2) locations
simultaneously (500 lbs total) to provide access for climbers. This condition assumes no
wind, but does include the self weight of the mount, platform, and antennas.

April 4, 1977
Following a petition by a majority of the residents in your neigh-
borhood, the City Council of Edmonds accepted the annexation in-
corporating your property, located at 21909 96th Avenue West,
within the City limits. Officially, the annexation became effective
November 6, 1976. The City has now transferred accounts from the
Alderwood Water District to the Edmonds water system. Billing has
been converted to the Edmonds water rate and utility fee.
The major motivation for annexing to the City was to obtain avail-
ability for sewer hookups. Our records indicate that your property
abuts an existing sewer main and you are, therefore, required to
make connection. City Ordinance normally requires connection within
60 days. However, on annexation we allow for inclement weather con-
ditions and additional time for the home owners to make necessary
arrangements. We are establishing a date of July 1, 1977, as the
time in which you must have your'residence connected to sewers.
Requirements for obtaining a Side -Sewer Permit are as follows:
1. Payment of connection fee. Please bring the legal
description of your property to the Public Works De-
partment prior to your request for a Permit so that
the connection fee can be computed.
2. Payment of $35 Permit fee.
3. Copies of all recorded easements (where applicable).
4. A plot -plan drawing of the proposed connection.
Side -Sewer Permits may be obtained at the Public Works Department,
200 Dayton Street, Edmonds, between the hours of 8:00 a.m. and
5:00 p.m., Monday through Friday.
