ON -SCENE ACTIONS En route, SNOCOM advised Engine 17 a Port Security guard reported smoke coming from the building. Upon arrival, Engine 17 observed smoke issuing from the several eaves and heard the audible alarm sounding, but saw no smoke in the building. Engine 17 established Command, upgraded to a commercial response and began a walk -around. Engine 17 Driver provided water supply assisted by Medic 17 and Engine 16 crews. Carrying a pump can and irons Engine 17 and a Police Officer gained entry via the north door to investigate. Encountering some smoke conditions, the Officer was told to exit, the crew masked up and proceeded to search the second floor. Returning to the first floor, the crew encountered moderate smoke conditions in the kitchen -bar area where visible flames were extinguished with a pump can. Engine 17 participated in subsequent overhaul. B20 established Command Post on north side of structure, and assigned AC 15 as Safety. Engine 16 carried irons, helped with the water supply, pulled pre -connect, unsuccessfully searched for the water shut-off, assisted with overhaul, and used wet -vac to clear standing water. L20 placed the Ladder on the north side of the building, completed primary search on both floors, placed two fans and ventilated the structure. Engine 14 assisted with hose lines. R1 9 unlocked doors with master key. AC 17 served as an aide. The building is publicly -owned, a two-story, partially-sprinklered Senior Center. A maintenance person reported smelling something 30 minutes before the alarm sounded searched, but found nothing. The fire was caused by a commercial coffee maker that had been "on" for "n*ine years." The plywood under the stainless steel counter finally succumbed to pyrolysis. Estimated dam- age was $10,000 to the structure, $5,000 in content loss. LESSONS LEARNED AND REINFORCED • Rapid BC response allowed Command to be passed and crew integrity to be maintained. • By the time the scene approached 2-in 2-out requirements, Battalion 20 had arrived on scene to be the fourth person. • Leave room for Ladder 20. • Be thorough and complete in the initial size -up. Use benchmarks. This is especially valuable when radio communications are below standard as they frequently are in the Edmonds Downtown. • Pull and utilize the building pre -fire plan. • Crews carried a pump can and irons into the building. • Assess ventilation after placing fans. Do not hesitate to reposition, open and/or close windows as required. • Consider establishing Base on street. • For sprinklered buildings, direct an Engine to stand by the automatic sprinkler connection. • Clearly identify the type of fire as soon as it is determined. • Use DeWalt saws during overhaul. This Fire Department is advanced enough to employ force when necessary, but also finesse -- property conversation -- when appropriate. DISPOSITION: Closed WEATHER CONDITIONS: Cool, Upper 50s F. DESCRIPTION OF PREMISES: Two-story wood -framed and concrete ground floor structure. South Snohomish County Senior Citizen Center used for assembly, dining and retail for Edmonds community. The building is in a commercial waterfront zoned area. Tenants have been in place for 30 years or greater. Building has been remodeled throughout life. City of Edmonds owns building that is partially protected with a fire sprinkler system (remodeled area), and detected with manual fire alarms and some heat devices. EXTERNAL EXAMINATION: No apparent flame or smoke damage from exterior. INTERIOR EXAMINATION: Thrift shop at West end of "L" shaped building was not damaged. Smoke infiltrated throughout both floors at south end of building. The kitchen and adjoining dining area is located on the first floor of the south end of structure where fire occurred. Fire Origin and Cause Determination Report Edmonds Fire Department Fire Marshal 16 John-J. p I J % AREA/POINT OF ORIGIN: The dining/assembly area had a wooden cabinet / island wet bar configuration with a coffee maker on the counter behind the wet bar. A Bunn Omatic coffee maker was used on top of a stainless counter with a plywood cabinet underneath. Smoke found it's way from under the coffee maker to the drop ceiling tiles and spread out on the ceiling away from the Area of origin. Firefighters found no structural/wall damage behind the coffee maker. The point of origin was under the coffee maker in the cabinet given the alligatoring of plywood in the space directly under thecoffeemaker. Underneath the cabinet were located #10 coffee cans filled with a variety of supplies, including coffee filters and espresso coffee. No items were found in coffee cans or under coffee maker in cabinet to warrant fire ignition. The outer stainless case for the coffeemaker was very hot from the normally heated water inside. The coffeemaker had approximately 2 1/2" legs standoff to stainless counter. Upon inspection of underside of cabinet, the entire top plate of plywood cabinet had burned away under stainless steel counter. In my opinion, the cabinet top ignited due to the continuous heating from the coffee maker, reducing the ignition temperature of the plywood top under the metal counter, eventuall . y igniting the board. WITNESS STATEMENTS: Mike, the maintenance person had worked here for about nine years and had not ever turned off the coffeemaker. He had been in the dining area until approximately 11:45, thirty minutes prior to alarm. He said he smelled something burning, but was unable to identify the source. Regarding the coffee maker appliance being always ON, the distributor evidently had recommended that the appliance remain on at all times to reduce wear and tear upon the coffee maker. Ferrell Fleming, the manager of the South Snohomish County Senior Center also acknowledged the practice of keeping the coffee maker energized at all times. There is a notation of the plan, Architectural, Sheet A-2, "Note, provide new fire detection method for existing kitchen hood and interconnect to fire alarm system (see Specifications) ... and connect fire alarm system directly to City Fire Department alarm headquarters."- This is consistent with the base bid as pro- vided: "Kitchen hood fire detection method and connect to fire alarm system allowance $500.00." This kitchen system was deleted from the contract, and therefore, no connection was made. In each case, recommendations have been made to have the premises protected bya complete system of automatic fire sprinklers. The Center is a unique type of occupancy that does not readily fit.into any standard category as outlined in building codes, fire codes or life safety codes. Therefore, to properly safeguard this unique segment of our community, it is.the Fire Marshalls considered opinion to again, and strongly, urge that all posibilities and avenues be explored towards protecting our Senior Citizens with the proven capabilities of fire sprinklers. Statistics from the National Fire Protection Association indicate that automatic sprinklers have either controlled or extinguished 96 percent of all fires in buildings for the past forty years. . In conclusion, it is felt that a fire occurring in any building is a -tragedy, but one in our Senior Citizen's building could be a community disaster if.all possible means available are not taken to protect it. At that time there were some problems that were noted with both buildings, but through some confusion on our part, - no recommendations were made. Since then, we have again looked at these two buildings; and have found the same situations exist now that existed then. The building at 254 Railroad Avenue is supposed to be undergoing some cleaning up, with the building eventually being occupied as offices. From what we were ,able to determine, this cleanup has not been accomplished, as there is still a heavy residue of fiberglass dust within the building, and there was no one on premises at the time that our Loss Control Representative made his visit. We.would like to recommend that the fiberglass dust be cleaned up, as it does present a fire hazard should someone go in there and carelessly discard a match W,cigarette. We would also like to know what the definite plans are for this building. The building located at 220 Railroad Avenue needs some attention, and the follow- ing recommendations would apply. 1. A 5-pound dry chemical fire extinguisher should be placed in the kitchen, to be available in caseof a small fire getting started. 2. The plastic waste paper and garbage cans should be replaced with metal cans and provided with lids. 3. The practice of putting cigarette butts into the garbage cans should be discontinued, and metal sand buckets should be made available for the cigarette ' butts. An alternative would be to empty the ash trays into a can containing water at closing time each day. Many fires get started in different operations after closing hours. Those recommendations would take care of the fire problems associated with this risk, and the following would be applicable to the liability exposure. 1. Install approved panic type hardware on the exit door of the second floor. This is the door to the rear of the building. 2. A strong railing should be installed on the north side of the second floor exit door at a right angle to join the railing that is situated at the top of the stair. 3. 4. 7. Prepare the outside wood stairs from the second floor exit to the ground so that the treads are in good condition and securely nailed. Remove the debris and trip hazards on all the property ' including the material in the former "geodesic" dome roof structure north side of the building. Paint the auto parking spaces on the parking lot surface. Post DANGER -WARNING signs indicating railroad siding. Install railings on the second floor of 254 Railroad around the former boat hoist opening. The above recommendations except for the last one would be applicable to 220 Railroad Avenue. I realize that this looks like a pretty impressive list of recommendations, Larry, but they are of great importance to the insured in attempting to make these premises relatively safe from both fire and a liability standpoint. We would appreciate your passing thene along to the City of Edmonds, and then advise us in a couple of weeks as to what the results of these recommendations will bring.
