MS-4(E)
Fire Alarm Control Panel
General
The Fire-Lite MS-4 and MS-4E Fire Alarm Control Panels
(FACPs) bring the latest in microprocessor technology to con-
ventional fire controls. The MS-4 is compatible with the PTO
smoke detectors from System Sensor, with drift compensation,
maintenance alert, and freeze warning. Automatic synchroniza-
tion of audio/visual devices with three selections for manufac-
turer protocol. The Notification Appliance Circuit (NAC) protocol
can silence audible devices while strobes continue to flash,
using a single pair of wires.
The MS-4 is compatible with conventional input devices such as
two- and four -wire smoke detectors, pull stations, waterflow
devices, tamper switches and other norm. l)y-open contact
 a
devices. Refer to the FireoLite Device Compatibility Document
for a complete list of compatible devices.
Note: Unless indicated otherwise, the term "MS-4"refers to both
MS-4 and MS-4E models.
Features
• Four Style B (Class B) Initiating Device Circuits (IDCs).
• Two Style Y (Class B) NACs.
• Optional module that converts all four IDCs and two NACs to
Class A operation.
• 24 VDC.
• i3Tm Technology features:
— Drift compensation automabcally adjusts detector sensitiv-
ity and increases resistance to false alarms caused by dust
accumulation.
— Maintenance Alert LEDs (per zone) warn of excessive dirt
accumulation, preventing false alarms (meets NFPA 72
requirements).
— Detector sensitivity is automatically measured by the
detector, which automatically adjusts its sensitivity back to
the factory settings when it becomes more sensitive due to
contaminants settling in the chamber.
— Wireless handheld sensitivity meter eliminates the need for
voltmeters, magnets, and a physical connection to the
detector. The reader displays sensitivity in terms of percent
per foot obscuration and provides text status indication.
— Supervisory LED (per zone) provides warning if a detector
senses temperature approaching freezing.
— Special test protocol and LED indication allows quick test of
all detectors without need for a ladder.
• NAC synchronization features:
— Synchronization of standard ANSI audible signals as
required by NFPA 72.
— Synchronization of ADA compliant strobes per NFPA 72.

Subject: Cintas Fire Alarm at Seabrook Condo's 221 3rd Ave S Edmonds
Mr. McGinness,
Thank you for your inquiry as to the permit in question that should have been issued for the replacement of your fire
alarm at your Condominium association's building. I do not show that Cintas Corporation has applied for or received a
permit from the City of Edmonds. If Cintas would have applied for one, at the time of installation, I would have had them
install on an emergency prevision as long as they came in the next business day and applied for a permit. The City of
Edmonds charges $165.00 for the permit and $35.00 administration fee for a total of $200.00. The installation
companies know not to do work without the necessary permits and for that reason we will be assessing a $240.00
additional fee for doing work without the permit. I believe that you should in no way be responsible for any additional
fees. It does appear that they did get an electrical permit months after the work was completed. ( L&I permit
#2798804E dated 1/31/2017 )
Just to be clear the Fire Alarm panel installation permit fee would have been $200.00 no matter if it was done as an
emergency or not.
Once this permit has been obtained, all additional fee's have been paid and the cut sheets for the system that they
installed have been submitted (dropped off at the time or emailed) they will need to call my office and request an
inspection and I will come out and have them test the system in my presents. This insures that what was installed is the
system that you paid for and that it is operational.
I am cc this to my Fire Marshal, Cintas Jeffery Hale and to our City of Edmonds Senior Permit Specialists so that we are all
on the same page.

I am sorry this has not gone as planned, I am involved now to help get it cleaned up. I do not
currently have the status on the city fire permit, as soon as I have any word I will let you know. This
initially was being handled through one of our engineers who is no longer with the company. That
being said, I will be helping on this to ensure it gets completed.
I could find no record of the electrical permit being pulled, that is not to say it wasn't, I just could not
locate it. It is pulled now, so we can schedule an inspector to come by anytime you say is fine. I
simply need contact information of who can give him access to the fire alarm panel.
Again, I am waiting to hear the status of the fire alarm permit. Once I have this we will schedule
with you and the fire dept to come do a function test on the panel. This has already been done by
Alex, it just needs a quick double check be the fire department. As soon as I have more information
on this I will let you know.
I apologize for all of the hassle on this, again I have just been brought into it recently, and I am
doing my best to make it right. Thank you for your cooperation through this.

I am not certain why it is saying 3 pages. I did not pull this permit and the person who did is gone
for the day. I am assuming the second page is blank. The third page (which usually is the second
page) is attached. This is used for call in purposes. If you or someone is on site to let the electrical
inspector in this can be done at any time. This is simply the log in info to do so.
The permit cost of $400 was an estimate as this was done on an emergency basis. We would
typically adjust for this at $600. A fire permit may also be required which requires design and
engineering labor plus the amount of the permit. This is a second permit in addition to the one
attached.
The answer to the question, is yes Alex or another Cintas representative would be onsite to go over
the panel with the Fire Marshal.

Jeffrey — Thank you for your help in getting this issue completed. I can be the contact person for the inspector to
check out the panel. The inspector can call me to set up a time to meet. My phone number is 425-776-9045. The
panel is located in the breezeway carport midway down on the north side of the building. It is mounted on an outside
wall in the carport, so access to it is always available.
Thanks again for your help.

Hi Jodi — Would you please email me copies of the following information regarding the fire alarm panel installation
that took place on Wednesday, 9/28/16 at the Seabrook Condominiums.
1. A copy of the service order. Alex had to leave for another appointment and did not leave me a copy of the work
performed for this job.
2. A copy of the permit issued by the City of Edmonds (referred to in the invoice)
3. A justification for the labor charge of $1040. How many hours of work is this based on and at what rate/hour?
4. A justification for a 390% markup on the retail cost of the Honeywell MS 4 panel. (Amazon lists it at $450)
5. Why were we charged for 2 batteries when Alex only replaced one of them. I mentioned this to you by phone less
than an hour after Alex had left that day.
Thank you very much in advance for your cooperation in supplying us with the above requested information.
Sincerely,
Jay McGinness

Good Morning Jody — Were you able to come up with answers to my 5 concerns mentioned below? I am pretty sure
that Alex was installing the panel from 10AM to no later than 12:30PM, so that would make the labor charge of the
invoice ($1040) figured at $416 per hour, which to me seems a bit unreasonable.
Looking forward to your reply, before I submit my payment.
Thank you,
Jay

No. I have not spoken to Alex since the install, and don't believe he has answers for the other items that we have
requested. He said that his paperwork would be sent in and that we would be billed for only one battery.
Looking forward to the requested documentation.

Hi Jody — I finally received a corrected invoice on 12/22/16 for service visit # 4118432 that shows that only one
battery was installed in our new panel on 9/28/16. 1 am still waiting for the PERMIT and the walk thru with the Fire
Marshal & Alex. I spoke to Alex last Monday 1/2/16 (which he said was his day off) and asked about the status of the
permit and walk thru. He did not know and suggested contacting you. I would like to pay this invoice and settle this
account, but will not do so until I have the $400 PERMIT and walk thru with the Fire Marshal & Alex that Alex had
mentioned was the justification for the large labor charge for the panel installation. After I received the corrected
invoice also I called Nancy (1-570-891-1888) onl/2/16 and left a voice mail expressing these same concerns but have
not heard back from her.
What is the current status of the PERMIT and scheduled meeting with Fire Marhal?
Thanks for your help.

Hi Jody — Alex did leave me a message to call him last week. I just figured he was not the one that determines the
per hour rate charge. If he had turned a work order or invoice in to you regarding the work he performed that day,
you'd be able to answer the questions about hours, permits, and the number of batteries that he installed. I doubt
that he has the PERMIT in his possession since I would assume that would be on file in your office. I am beginning to
wonder if the company's review on the BBB isn't correct. httr)://www.bbb.org/cincinnati/businesa-

Hi Jeffrey — The pdf you attached says there are 3 pages, but I only received page one of the permit request. Can I
get pages 2 and 3? Will the Cintas technician (Alex) be present when the Fire Marshal does his inspection? Since this
permit looks to be $54.30, how do you justify the $400 permit fee that was included on our invoice #OF93059637?
Thanks for clarifying this issue.
