This is in response to several notices we have received regarding a Permit
renewal for hazardous materials at our location of business. You will find
a copy of the most recent letter enclosed.
This letter is to inform you that the entity known as Veterinary Oncology
Services is no longer doing business.
We (Veterinary Radiology) are presently located at 22226 Highway 99,
Edmonds, WA. We have been in this building since July 22, 2000, at which
time we moved into this building and shared the space with Veterinary
Oncology Services. Since that time, the hazardous materials Iodine-131
and Technetium-99m have not been on site.
We hope this addresses your concerns.

The Department of Health, Division of Radiation Protection is required by law
(RCW 70.98.080(2)) to notify the appropriate local government officials whenever
a radioactive materials license application is received. Specifically, the
Division must notify either: the chief executive officer, if the applicant is
located in a city; or the county legislative authority, if located in the county.
This is your official notification of the specific use and pos session of
radioactive material as described in paragraph four below. As a courtesy we are
forwarding a copy to the director of your local jurisdictional health department/
district. We urge that you forward a copy to the appropriate fire department for
their review.
The Division of Radiation Protection will delay the issuance of a license, at
least 20 days from the date of this notice. During this time period, if you deem
appropriate, you may file written objections against. the applicant or the
activity for which the. license is sought. The law states that all written
objections shall include a statement of facts upon which such objections are
based. Upon filing written objections you -may request, and the. Division of
Radiation Protection may at its discretion hold, a formal hearing concerning the
applicant or activity.
On May 26, 1993, DENNIS WACKERBARTH DVM/CATS EXCLUSIVE VETERINARY HOSPITAL, PS
submitted an application requesting to be licensed to receive and use radioactive
materials at 22226 HIGHWAY 99, EDMONDS, WASHINGTON. The applicant stated that
the radioactive materials will be used for diagnosis and medical treatment for
cats.

Purpose for which Radioactive Material will be Used
The diagnosis and medical'treatment of hyperthyroid cats. Cats will be
injected with 1-131 in quantities sufficient to suppress thyroid func-
tion. They will be isolated at the hospital until their body burdens of
radioiodine have reached a predetermined level at which they may be
released to their owners. Technetium-99m will be used diagnose thyroid
disease and other conditions by conventional medical imaging techniques.

Exclusive Veterinary Hospital. This hospital plans to use injections of 1-131 for the
d' nosis and treatment of hyperthyroid cats. We have twenty days from June 16 to respond

Dr. Wackerbarth (771-2287) called the Mayor's office today asking about -the license as he
would like to begin treatments as soon as possible. He is hoping the City has no objections
and could respond to the Department of Health quickly, rather than use the full twenty
days.

Purpose for which Radioactive Material will be Used
The diagnosis and medical treatment of hyperthyroid cats. Cats will be
injected with 1-131 in quantities sufficient to suppress thyroid func-
tion. They will be isolated at the hospital until their body burdens of
radioiodine have reached a predetermined level at which they may, be
released to their owners. Technetium-99m will be used -diagnose thyroid
disease and other conditions by conventional medical imaging techniques.
