Subject: Backflow Assembly Testing
The reduced pressure backflow assembly installed on your main water service line
for the required backflow prevention has been approved.
You are required to have the assembly tested by a state certified backflow
assembly tester. Attached is a test result form with a list of active certified testers
in the local area.
Please have the test co ' mpleted and the test results returned to my office within 30
days (7110- 210th St. SW., Edmonds, WA 98026).
Failure to comply according to the City of Edmonds Ordinance Number 2956 and
State WAC Code 246-290-490 will result in discontinuance of water service to
your building.

RE: DRINKING WATER SUPPLY CONTAMINATION
Dear Owner:
Recently, you were advised that it would be necessary to take certain steps in order to
minimize the danger of contamination to the drinking water supply sources at the above
premise. As required by State WAC 246.290.490, and City of Edmonds Ordinance
#2956, you must install an approved reduced pressure backflow assembly on your
make-up line to your boiler.
You will have 60 days to comply with this request. Please contact me within 10 days of
receiving this letter to apply for a permit (free of charge) and further information on
proper procedures and installation.
