Description of Violation.:. 1) As noted in the letter dated September 23, 2010, the mobile homes at
space O's 19 and 20 were vandalized and possibly used by vagrants. This is considered a public
nuisance. The required fire extinguishers in the park had been removed. Cut brush had been dumped
near the western sidewalk, representing a fire hazard
Corrective Action Required: 1) As noted in the field to propelly managers and in the letter dated
September 23, 2010 - Please remove the brush from the site. Please ensure that the mobile homes
currently at sites 19 and 20 are maintained as secure with all walls, doors and windows closed and
locked or boarded up. Another option would be to obtain a demolition permit and de'molish or haul the
mobile homes from the site. Fire extinguishers are required per the 2009 International Fire Code and
must be installed inunediately.
Correction is Required no later than: 5:00pm (time) October 12, 2010 (date)
If correction is not made by the date and time specified in the Order, a Notice of
Civil Violation shall be issued. The Notice of Civil Violation shall assess fines of
$100.00 per day, or portion of a day, during which the violation continues.

Narrative
E16 arrived on and was met by Mr. Sizemore, the night manager for the trailer park. He
showed us the power pole that had broken off at just above ground level. There were several
service lines connected to this pole. The pole was leaning against a shed. The wires were
draped through some! trees on the roperty. There was no arcing or sparking. I advised Mr
Sizemore to notify all residents of t�e property to stay away from this area. E16 crew strung
out scene tape in the area where the wires were low and/or on the ground. I requested PUD be
dispatched "priority". They were already in route. I reconfirmed with Mr Sizemore the potential
danger at the trailer park. E16 then returned.

Osmose presents a new specification for FLAME PROOF wood.
FR-S Lumber & Plywood
FR-S is fire retardant wood that has been tested by Underwriters' Laboratories, In-
corporated, for fire hazard classification of building materials. Pressure impregnated
lumber and plywood is subjected to extreme flame and heat in a 25 foot tunnel for 30
minutes test duration. The material so tested must meet the following limitation for
acceptance for U. L. FR-S labels and only FR-S material must meet all these re-
quirements.
Flame Spread 25 or less
Fuel Contributed 25 or less
Smoke Developed 25 or less
Recognition of Osmose FLAME PROOF FR-S by building codes and in ' surance rating
bureaus is based on these stringent requirements and all Osmose FLAME PROOF
products meet these requirements.
Codes and insurance rating bureaus accept FR-S studs and FR-S roof systems for use
in fire proof and fire resistive construction. Insurance rating bureaus rate FR-S
wood the same as non-combustible in most types of occupancies for non -load bearing
partitions and roof truss and decking.
The new specification for FR-S is included on the reverse side to guide the specifier
in obtaining FR-S wood that will meet building code and insurance bureau acceptance.

and national requirements.

SPECIFICATIONS
OSMOSE FLAME PROOF FIRE -PROTECTED
LUMBER AND PLYWOOD
All lumber and plywood ' so designated shall be pressure impregnated
with Osmose Flame Proof fire retardant chemicals. Material shall
bear the FR-S label of —Underwriters'- Laboratory showing conformance
to specifications of national and local building codes as well as fire
insurance underwriters. Each piece of lumber or plywood shall be
further identified with the names "Flame Proof" and "Osmose". This
treatment shall, also provide preservative chemicals to resist decay
and termite attack under conditions to which fire retardant treated
lumber is normally exposed.
After treatment, I inch and 2 inch lumber and all thicknesses of ply-
wood shall be dried to an average moisture content of 19% prior to
installation. Lumber thicker than 2 inches shall be surface -dried.
— CAUTION —
FLAME PROOF -treated wood should be limited to interior use
on I y. FLAME PROOF -treated wood should be stored indoors,
under dry conditions. Do not store or use i.n combination with
large areas of wet plaster, concrete, or terrazzo which contribute
moisture. Wait until these materials are dry and the moisture is
dissipated.
