Please take notice that PATRICIA J. BOULANGER who now resides at 6229
Northeast 202nd, Seattle, King County, Washington 98155, adn who for six months
prior to date of accident has resided at the same address, claims damages of and
from the City of Edmonds in the sum of $25,000 p,lus special damages arising out of
the following circumstances:

July 9, 1984 at approximately I-1:00 a.m.
While carrying items from residence at 929
Daley, Edmonds, Washington to truck parked. in front of said
residence address, claimant stepped off curb into a hole in the
street, which hole was diamond shaped measuring
approximately 18 inches on each. side of said diamond, with a
depth of approximately six inches. That the City of Edmonds
failed to repair the hole nor to give notice of its existence.
Injuries: Claimant received a sprain, strain of her right foot and ankle,
as well as a back injury, tendonitis, and other such injuries as
may be determined by treating physicians.
Damages: Medical expenses have been incurred to date and will be
incurred in the future, the full nature and extent of which
claimant does hot have fully knowledge of. Medical expenses
have been incurred to date with the following: Totem Sports
and Rehabilitation Clinic; Northwest Orthopaedic Clinic;
College Place Chiropractic Clinic; Kenmore General Medical
.Clinic; Stanley G. Newell, DPM; Birth and Family Clinic.
Loss of earnings to date and to be incurred in the future, as
yet undetermined.
Witnesses: Other persons having knowledge with respect
to the incident herein set forth are: Mr. and Mrs. David F.
Pittsford, 929'Daley, Edmonds, Washington. .

We received your letter informing us of the Breakwater Townhouse
Condominiums request for under�'�odnding a portion of our.aerial
cables on 2nd Avenue North.
We reviewed the conditions in the field and have prepared an es-
timate of cost based upon the P.U.D. permitting us to use their
trench at no charge. Should it be necessary for the Telephone
Company to dig trenches such cost will be added to this estimate.

The following is in response to your letter of August 25, re-
garding a preliminary cost estimate for the above conversion.
Our Engineering Department has investigated the site and worked
with your associate Mr. Allen to establish feasible boundary limits for
this project. Attached are two copies of drawings of the proposed in-
stallation. It should be noted that two services on private property are
involved.
Our preliminary project cost estimate for conversion of the
electrical distribution system is $3,474.00. As you know, the District
will share one-third of the project cost and that telephone, etc., and
the private property services are over and above this figure.
If detailed costs for contractual arrangements will be required,
we will gladly provide same upon authorization of $200.00 for the design
work involved. This, of course, would not be billed unless the project
failed.

I am the Secretary -Treasurer of the Breakwater Townhouse, a Condominium
Apartment located at 300 2nd Ave. N. in Edmonds.
I have made a survey of our twenty three owners living in this building
and'have found that they are very much in favor of undergrounding the

approximately 400 feet North on Second Ave. No. or to the pole at 400t
North of the intersection of Edmonds Street and Second Ave. No.
We are anxious to know if this can be done and approximately how much
