Kristy, thanks for your prompt response.
Concerning the key box, I saw a box on the North side of the building but was unable to open it. I attempted to lubricate
without success. Could be from the salt air. Anyway, I am more than willing to meet on site with a maintenance person
and attempt to gain access again, but if we are unsuccessful, we would need a new box.
I had one other item on my list, the fire extinguishers in the building have not been serviced in 3 years, tags state
serviced in 2/2010, expired 2/2011.

www.knoxbox.com. This form is site specific and ensures the box is keyed to Snohomish County Fire
District #1's key. When you receive the box and have it mounted, please provide a key to the main
access door and the fire alarm control panel room, I will come out and put your keys in the box.

A Fire Inspection of your property was completed a short time ago and the following
items were noted:
1) Two of the emergency lights in the North stairwell were inoperable.
2) The 3rd floor fire extinguishers need an annual inspection done. (This was probably
an oversight by the fire extinguisher company since the first and second floor fire
extinguishers were serviced June of 2004).
Please contact me if you have any questions or when the items are completed.
On a side note the general maintenance of the Life Safety items in the building have
improved tremendously.

Subject: Sprinkler Extinguishes Fire; Saves Building (2 pp.)
An automatic fire sprinkler system installed in a downtown building extinguished
an unintentional fire caused by a worker on August 22 before serious damage occurred.
The Bayshore is a mixed -use residential and commercial building at Olympic Beach on
W. Dayton Street. The building is protected with an automatic fire sprinkler system
throughout all 3 floors and basement. Automatic fire sprinklers aid in the detection and
control of commercial fires, providing improved protection against injury, life loss, and
property damage. Fire sprinklers activated by heat deliver water only onto the fire itself.
This was fortunate for tenants and the property owner early Thursday morning.
A little after 9;.00 a.m., a maintenance worker was brazing water pipes in the basement
of the building when the torch he was using malfunctioned. A ball of flame ignited along
the applicator and the fiery tool was dropped. A patch of scrap carpeting ignited on the
floor as fire continued to blow while the torch fuel fed a jet of flames. The worker left
quickly to sound the alarm and call 9-1-1.
The building manager arrived prior to Fire engine 17. He heard what sounded
like gas venting from an unknown source. In fact, he found water being applied to the
fire from a single sprinkler head and cooling the fire. Total fire damage came to a scrap
piece of carpeting. Fire engine 17 from downtown Edmonds arrived directly thereafter
to turn off the fire sprinkler sytem. A floor drain in the basement carried the water away
minimizing any damage from the water itself. The manager contacted his sprinkler

service contractor who brought the sprinkler system back into service within only a few
hours. The total property "saved" by the sprinkler is estimated to be in the hundreds of
thousands of dollars, as well as the safety of the residents and tenants themselves.
Fire sprinkler systems are required in certain buildings when they are
constructed. Residential fire sprinkler systems for apartments and dwellings add about
one percent or less to the cost of a new home. This is about the same cost as upgrading
carpeting. But carpets need to be replaced every ten years, while fire sprinklers last for
the life of the home.
Having the recommended number of smoke alarms can improve your chances of
surviving a fire at home by nearly 50 percent. Adding fire sprinklers and smoke alarms
can improve your chances to over 97 percent. Fire sprinklers will save lives. Contact

INVESTIGATORS CONCLUSION:
Unintentional fire caused by leaking 1p-gas open flame torch applicator. Given witness
statements and physical evidence at scene of fire, it is determined that gas leaked through a
faulty valve or threaded connection on a portable torch, ignited and created a ball of fire. Worker
dropped torch on floor which ignited carpeting. A wet sprinkler fused and extinguished fire.

Mixed use residential and commercial building in a commercial waterfront -zoned district.
Building is a three story with basement wood -framed structure currently occupied. Building is
protected with both UBC Standard 9-1 wet and dry fire sprinkler systems from a common supply.
EXTERNAL EXAMINATION:
No apparent problem in view. Sprinkler waterflow alarm sounding.
INTERIOR EXAMINATION:
Upon arrival I was directed to the basement utility/storage area.

XPOINT OF ORIGIN:

,...,Xtanks are located. The tanks were installed on a plywood floor. In the.center of this space was a
carpet remnant placed over plywood. Building manager, Richard Wiggihs had dropped concrete
mix onto fire in an attempt to extinguish.
WITNESS STATEMENTS:
Worker and brazer Jack Welker explained that a new fuel cylinder had just been installed to his
1p-gas open flame torch. The torch was not getting hot enough. Mr. Welker was in the process
of shutting off the fuel gas at the applicator valve. Flames ignited fuel around the
cylinder/applicaior threads and control valve. Mr. Welker dropped the torch and left to get help.
ADDITIONAL INFORMATION:
Mr. Wiggins (Cell 206 799-4428) was informed to contact his fire sprinkler service company to
have both wet- and dry -sprinkler systems reset. The PIV was shut at the street by El 7 to stop
flow of water and waterflow alarm. Service company is American Sprinkler Corp of Snohomish,
WA 425 335-4645. System last serviced on 3/7/02.
No damage occurred as a result of the fire extinguished by the sprinkler. A floor drain allowed
the sprinkler water to flow out of the basement.
Mr. Wiggins called at 1515 hours to inform me that sprinkler system had been reset for $130
service fee.

2.9 Waterflow Alarms.
2-9.1 Waterflow alarm apParatu� shall be listed for the
service and so constructed and installed that any flow of
water from a sprinkler system equal to or greater than that
from a single automatic sprinkler of the smallest, orifice size
installed on the system will result in an audible alarm on
tile premises within 5 min after such flow begins.
2-9.2 Waterflow Detecting Devices.
2-9.2.1 Wet Pipe Systems- The alarm apparatus for a
wet pipe system shall consist of a listed alarm check valve
or Other listed waterflow detecting alarm device with tile
necessary attachnients required to give an alarm.
2-9.2.2 Dry Pipe Systems. -rhe alarm apparatus for a dry

Chapter 3 System Requirements
3-1 Wet Pipe Systems.
3-1.1 Pressure Gauges. A listed pi-cssure gauge con-

klers shall be located in the bays and not under the beams.
4-4.5.4 Obstruction to Discharge.
4-4.5.4.1 Obstructions Located at or near the Ceiling.
Where sprinkler deflectors are located above the bottom of
beams, girders, bottom chords of trusses, bottom chords of
joists, ducts, fluorescent lighting fixtures, or other obstruc-
tions located at the ceiling, the sprinklers shall be posi-
tioned so that the maximum distance from the bottom of
the obstruction to the deflector does not exceed the value
specified in 4-4.1.3.
4-4.5.4.2 Obstructions Located Entirely below the Sprin-
klers. Sprinklers shall be positioned with respect to any
fluorescent lighting fixtures, ducts, or any other obstruc-
tion more than 24 in. (610 mm) wide and located entirely
below the sprinklers so that the minimum horizontal dis-
tance from the near side of the obstruction to the center of
the sprinkler is not less than the value specified in Table
4-4.3.4.2. 1. (See Figure 4-4.3.4.2. 1.)
Exception: Sprinklers shall not be required to comply with spac-
ing limitations of Table 4-4.3.4.2.1 where additional sprinklers
are located beneath the obstruction and are included in the water
demand.
4-4.6 In -Rack Sprinklers.
4-4.6.1 System Size. The area protected by a single system
of sprinklers in racks shall not exceed 40,000 sq ft (3716 m)
of floor area occupied by the racks, including aisles, regard-
less of the number of levels of in -rack sprinklers.
4-4.6.2 Control Valves. Where sprinklers are installed in
racks, separate indicating control valves and drains shall be
provided for ceiling sprinklers and sprinklers in racks.
Exception No. 1: In -rack installations of 20 orfewer sprinklers.
Exception No. 2: The separate indicating valve shall be permit-
ted to be arranged as sectional control valves where the racks
occupy only a Portion of the arem-protected by ceiling sprinklers.
44.6.3 Type of Sprinklers. In -rack sprinklers shall be
ordinary temperature '/2- or 17/32-in. (12.7- or 13.5-mm)
sprinklers.
4-4.6.4 Location of In -Rack Sprinklers.
4-4.6.4.1 A minimum 6-in. (152-mm) vertical clear space
shall be maintained between the sprinkler deflector and
the top tier of storage.
4-4.6.4.2 The maximum spacing between sprinklers shall
be 10 ft (3.05 m).
4-4.6.4.3 Sprinklers shall be located in transverse flue
spaces.
4-4.6.4.4 In -rack sprinklers shall be located at the first
tier level at or above one-half of the storage height.
4-5 Special Situations.
4-5.1 Concealed Spaces.
4-5.1.1* All concealed spaces enclosed wholly or partly by
exposed combustible construction shall be protected by
sprinklers.
INSTALLATION
Exception No. 1: Concealed'spaces formed by studs orjoists with
less than 6 in. (152 mm) between the inside or near edies of the
studs orjoists. (See Figure 4-4.1.3.4.)
ExceptionNo.2: Concealed spaces formed by bar joists with less
than 6 in. (152 mm) between the roof orfloor deck and ceiling.
Exception No. 3: Concealed spaces formed by ceilings attached
directly to or within 6 in. (152 mm) of woodjoist construction.
Exception No. 4: Concealed spaces formed by ceilings attached
directly to the underside of composite woodjoist construction, pro-
vided the joist channels are resto ed into volumes each not
exceeding 160 cu ft (4.53 ml pp

4-5.1.2 Sprinklers in concealed spaces having no access
for storage or other use shall be installed in accordance
with the requirements for Light Hazard Occupancy.
4-5.1.3 Where heat -producing devices such as furnaces
or process equipment are located in the joist channels
above a ceiling attached directly to the underside of com-
posite wood joist construction that would not otherwise
require sprinkler protection of the spaces, the joist channel
containing the heat -producing devices shall be sprinklered
by installing sprinklers in each joist channel, on each side,
adjacent to the heat -producing device.
4-5.2 Vertical Shafts.
4-5.2.1 One sprinkler shall be installed at the top of
shafts.
Exception No. 1: Noncombustible, nonaccessible vertical duct
shafts.
Exception No. 2: No7icombustible, nonaccessible vertical electri-
cal shafts.
Exception No. 3: Noncombustible, nonaccessible vertical pipe
shafts.
4-5.2.2* Where vertical shafts have combustible surfaces,
one sprinkler shall be installed at each alternate floor level.

coverage (UFC 1001.5.1)
2) 2nd floor deck off south exceeds 4 feet in width and lacks sprinkler coverage (UBC Standard 4-4.10)
3) exterior bell will require relocation below drop ceiling to east.
Hydraulic calculations and plans submittal will be required to sprinkle the unprotected areas noted from existing or new dry
pipe system. Plans and calcs will require ICBO review prior to our review.

E7 WAS DISPATCHED TO A FIRE ALARM AT 51 WEST DAYTON ST. E7 ON SCENE, MADE
CONTACT WITH THE RESTAURANT EMPLOYEES WHO WERE LEAVING THE BUILDING BECAUSE OF
CLOSING TIME. THERE WAS WATER FLOWING FROM THE SPRINKLER FLOW DRAIN AND THE
AUDIBLE ALARM WAS ACTIVATED. WE GAINED ACCESS TO THE KNOX BOX AND USED THE
MASTER KEYS TO CHECK THE ENTIRE BUILDING FOR A SPRINKLER HEAD THAT HAD BEEN
FUSED. WE FOUND NONE. WE GAINED ACCESS TO THE WET SYSTEM RISER AND CHECKED
FOR WATER FLOW. ALL VALVES WERE TURNED OFF AND THE ALARM WAS STILL ACTIVATED
AND WATER STILL FLOWING. THE DRY SYSTEM WAS CHECKED AND APPEARED IN ORDER.
THE COMPRESSOR WAS NOT OPERATING AND THERE WAS 40 PSI ON THE AIR GAUGE. WE
COULD NOT FIND A BUILDING MANAGER, MAINTENANCE PERSON NOR COULD SNOCOM GET
POSITIVE RESULTS FROM THE AGENCY THAT RAN THE LAST ACCEPTANCE TEST OF THE
SYSTEM. WE SHUT DOWN THE SYSTEM AT THE VERTICAL PI VALVE AND LEFT FF NESS
STANDING BY WHILE CAPTAIN ANGEL AND FF ERICKSON WENT BACK TO STA. 7 TO CHECK
THE FILES FOR A PHONE NUMBER OR A PERSON WHO COULD TAKE OVER THE BUILDING.
AFTER NUMEROUS PHONE CALLS IT WAS DETERMINED WE COULD NOT CONTACT ANYONE
CONNECTED TO THE BUILDING EXCEPT AAA FIRE SAFETY. AAA HAD COMPLETED A
SPRINKLER ACCEPTANCE TEST IN FEBRUARY OF 1994 BUT COULD NOT*COME OUT TO THE
BUILDING TO CHECK THE SPRINKLER SYSTEM UNTIL INSTRUCTIONS WERE GIVEN BY THE
OWNER. E7 WENT BACK TO 51 W.DAYTON. IT WAS THEN WE NOTICED THAT THE DRY
SPRINKLER HAD GONE WET AND THE AIR GAUGE WAS SHOWING A UNUSUALLY HIGH NUMBER.
THE WATER MOTOR LINE FROM THE DRY SYSTEM TO THE WATER MOTOR GONG WAS SHUT OFF
AND THE ENTIRE SPRINKLER SYSTEM WAS RESTORED.
THE WET SYSTEM WAS BACK IN SERVICE INCLUDING THE AUDIBLE ALARM. THE DRY SYSTEM
WAS IN SERVICE BUT WAS TRIPPED WET. THE AUDIBLE ALARM FOR THE DRY SYSTEM WAS
DEACTIVATED. THE VERTICAL PI VALVE WRENCH WAS PLACED IN THE WET SPRINKLER

I was paid a visit by one of the patrons of T.J.'s Bayshore Restaurant. This citizen, Mr.
Lewin was concetned about the safety of the elevator for the Edmonds Bay Building. Mr.
Lewin had shared a ride on the elevator with an unknown person proclaiming to be an
elevator service technician. This "technician" had an unsettling diagnosis of the operating
sounds the elevator was making. The citizen, Mr. Lenin, looked to identify a WA State
elevator inspection tag. This had been torn off; apparently vandalized.
I paid a visit to the elevator and could find no tag myself. My untrained ears did not identify
any disconcerting noises upon the elevators use.
This is for your information only. My concern lies in the continued patronage of your
tenant's customers, in this case. I do not perceive this as a life safety issue.

On may 24, 1985 1 received a call from one of the tenants who
resides on the second floor of your building. The call was to report
that water was leaking through the ceiling and running into light
fixtures and electrical baseboard heaters. A call was made to Ms.
Kathy Allen, Property Manager, but because of the long weekend I was
not able to contact her.
On May 29, 1985 1 met with Ms. Allen at your building and ob-
served the leaks. They are real and should be repaired as soon as
possible. If the present condition is allowed to continue a possi-
bility exists for either an electrical fire to start or an electrical
shock to be inflicted on one of your tenants.
I understand that the building is up for sale and there is no
desire to expend funds but, in the interest of safety, the problem
should be corrected as soon as possible. I

SUBJECT: UPDATE ON THE REGENCY BUILDING SAFETY PROBLEMS
Despite our efforts to obtain compliance with the City's life safety codes,
Mr. Pantley continues to operate the Regency Building at 144 Railroad Avenue
with occupancy and exiting code violations which.threaten the safety of the
public. The following is my analysis of the current situation.
The plans were submitted and approved for one hour fire resistive.
cons.truction between the parking garage (B-1 occupancy) and the
restaurant (A-2.1 occupancy). However, I am in receipt of a recent
opinion by the International Conference of Building Officials which
holds that the required separation should be 3 hour instead of one
hour. To make up the difference would require the addition of -more
fireproofing to the structural steel and the installation of a 3
hour fire door assembly. One hour construction was provi,ded but
there is no fire door at all between the parking. garage and the lst
floor corridor serving exit #1 from the restaurant. This- is a great
hazard to the restaurant occupants.
Section 3303 and 3318 of the UBC require that the A-2.1 restaurant h.ave
a main exit capable of accomodating 1/2 the total occupant load. Thus,
the existing exits for the building would limit the total number on
the third floor to 300. This is the limit stated on Mr. Pantley's
Certificate of Occupancy, yet he has posted a load limit sign of 377
and states that he has 400 or more people at times on the third floor.
Other code violations exist also.
Restaurant exit #2 dumps into the parking garage. Plans called for a
walkway (never installed), but the code would require a fire resistive
corridor to the public way.
The first floor lease space appeared originally to be designed for offices
but.was changed to retail use. The tenants use the fire resistive exit
corridor as a mall and prop open the fire doors. If these doors must be
open, they should be on smoke detector actuated automatic closure devices.
The City Attorney feels that the City has a great liability exposure in this
matter. It is my recommendation that efforts be coordinated through the
attorney's office to secure compliance with the code as soon as possible.

1. Re -organize clothing in employees change room so
that the door will shut and latch.
2. Adjust door from kitchen to hall so it latches.
3. Clean sprinkler heads as needed in cooking line
area.
4. Re -place outlet cover plate at waitress station.
5. Replace outlet by hot fudge machine.
6. Remove table from exit corridor by piano.
7. Remove or move piano so that a 3 foot corridor is
maintained to the exit.
8. Remove and prevent storage of combustibles in the
second story mechanical room.
9. Properly complete the liquor storage room.
10. Adjust southeast exit door from the lounge so
that it latches.

11. Within 60 days remove the chairs stored in the
well and allow only non-combustible storage in
this area or protect it with automatic sprinklers.
12. Provide a cover plate for the electrical junction
box just inside the door of the basement.

14. Repair or replace the light fixture located in
the southernmost storage room. (Basement)
15. Remove all storage from the electrical room and
repair the door so it will latch. (Basement)
We are requesting that the above corrections be made. We
will reinspect after 30 days.

In August, 1977, this approval was ex-
tended to both wall and ceiling panels.
In addition, Mly panels have passed
a number of diversified supportive tests
conducted by Underwriters' Labora-
.tories, and Factory Mutual.
Ah'thizse ratings qualify Bally Prefabs

those applicable to masonry type struc-
tures. Most important, they provide a
vastly increased measure of safety, as
compared to conventional urethane, in
hospitals and nursing homes, schools
and colleges, hotels and motels, restau-
rants and clubs ... wherever masses of

Bally's 10091v "pour -type"
urethane has greater
stability than "froth -type"
-Two- types ofurethane insulation are
being used commercially in foamed -in -
place applications . . . a "pour -type"
and a "froth -type". The "pour -type"
urethane has great dimensional stabil-
ity and holds its precise dimensions
and shape. "Froth -type" urethane
lacks this stability, especially when ex-
posed to heat.
Bally "pour -type" urethane uses Freon-
11 with a boiling point of 76.6*F.
(24.7*C.) (normal room temperature)
as its expanding agent. Since this is a
low pressure gas, that is why Bally ure-
thane foamed panels always hold their
exact thickness and precise edges.
"!Froth -type" urethane requires Freon-
f2 as an expanding agent, in addition
to Freon-11. It is the low boiling point
of -23* F. ( 30,6*C.) of Freon- 12 that
causes the frothing action. Because of
its low boiling point, in comparison with
Freon- 11, � Freon-12 generates much
higher prewnres when exposed to high
temperatures. For example, the inher-
ent pressur ' e of Freon-12 at 150*F.
(65.6'C.) is greater than six times that
of Freon-11.
In addition to superior stability char-
acteristics. "pour -type" is also better
and more consistent in its insulating
value. it contains far fewer large cells
than "froth4pe", and the smaller the
cell size in urethane the better the
insulating value. This tendency of
"froth -type" to have large cells is ap-
propriately known in the industry as
"Swiss cheesing".

Here's an example that illustrates how
operating costs can be lower with Bally
prefabs.
A Bally 12' x 12' x 8'611 high Walk -In
oper�ting at minus 10 ` (-23.3-C.)
requires a 3 H.P. unit. in an 80*F.
(26.7-C.) ambient it runs an average
of 8.8 hours daily consuming 1340
KWH per month. Ai 54 per KWH, the
operating cost is $64.32.
A similar size walk-in insulated with.
fiberglass or comparable insulation
would operate an average of 13.7 hours
per day and consume 1795 KWH per
month. At 54 per KWH, monthly op-
erating costs would be $86.16.

Narrative
E17 responded with E16 and B16 to a reported SWFA at the Edmonds Bay Office Building.
Upon arrival alarm was sounding, no problem visible. E17 established command. Investigation
revealed that the dry system ha� gone wet. The alarm was reset, the subscriber notified and
arrangements for system restoration by a tech were being made. All units returned to service.

Narrative
E16 responded with other units to a SWFA at 51 W. Dayton. On scene, crew was assigned to
check the 3rd floor. Nothing was found. Crew then checked the basement storage area and
found a broken pvc pipe that was the drain for the sump pump. Water from thhe sump was
spraying all over when the pump engaged. Unknown if this was a cause of teh alarm or a
secondary problem. Room was turned over to E17 and B16 for further. E16 returned.

As per our conversation, here is the information regarding out inspection.
I spoke with Carol Floten at (503) 624-2797. Her fax number is (503) 624-7174.
During the inspection, we found the following:
0 Emergency Lighting not working on 2nd floor, and Stairwell exit sign and attached lights are not working.
0 Multiple Fire doors were jammed open and must be kept closed.
0 There was storage on hot water heater.
0 2 junction boxes in 2nd floor mechanical room need covers.
0 Fire Door between first floor mechanical room and adjacent stairwell was propped open.
Fire Sprinkler/Fire Alarm Inspected 12/05 and due this year.
Fire Extinguishers 4/06
Lock Box present with keys
