Purpose of Checklist:
The State Environmental Policy Act (SEPA), chapter 43.21C RCW, requires all governmental agencies to consider the
environmental impacts of a proposal before making decisions. An environmental impact statement (EIS) must be prepared for all
proposals with probable significant adverse impacts on the quality of the environment. The purpose of this checklist is to provide
information to help you and the agency identify impacts from your proposal (and to reduce or avoid impacts from the proposal, if it
can be done) and to help the agency decide whether an EIS is required.
Instructions for Applicants:
This environmental checklist asks you to describe some basic information about your proposal. Governmental agencies use this
checklist to determine whether the environmental impacts of your proposal are significant, requiring preparation of an EIS. Answer
the questions briefly, with the most precise information known, or give the best description you can.
You must answer each question accurately and carefully, to the best of your knowledge. In most cases, you should be able to answer
the questions from your own observations or project plans without the need to hire experts. If you really do not know the answer, or
if a question does not apply to your proposal, write "do not know" or does not apply". complete answers to the questions now may

SUBJECT: DOOLITTLE COMPLAINT ON ALLEY CONDITION
On October 6th Bill Stroud and I met with Mr. Doolittle and Ms.
Von Endt in the alley for a discussion. I explained that the
condition of the alley was not bad enough to spend a large amount of
money at this time, but that it definitely was in need of minor
repairs. Ms. Von Endt wanted to repair her driveway and wanted,to
know then the City would schedule repair of the alley. This was
scheduled on October 8th. Next year we will make additional repairs to
the east end of the alley. I have written a letter to the residents.
I thought I had given the letter to Bill Stroud back during the
first part of April so that he could address the problem, apparently it
got misplaced as Bill states he never received it. We have a new
practice now where Chris keeps the original and suspenses it for follow
up, this appears to be working.

I apologize for not answering your letter which you had written in
March. I thought I had given the letter to our Street Supervisor, Bill
Stroud, so he could follow up on the needed repairs. I did met with
Mr. Doolittle and Ms. Von Endt on October 6th to discuss the
alley maintenance.
The following are answers to your questions in your letter dated
March 31, 1986.
1. Any major work to the alley would be done by the residents
upon approval by the City Engineer.
2. Paving by means of asphalt would be paid by the residents.
3. Recontouring would be considered major work as referred to
in #1.
We will maintain what exists, as previously discussed in meeting
of October 6th. Next year we will remove the grass on the east end of
the property at 714 Laurel St. and reseal coat that section. The
section which has never had any surface would have to be paid for by
the adjacent residents. This cost would be between $1.00 and $1.50 per
square yard. We will check the section with the large crushed rock and
see how much seal coat exists under the grass and rock. Any existing
seal coat will be replaced by the City in the 1987 Seal Coat Program.

Dear Larry:
I would like to bring the enclosed letter to your attention.
It has been almost six months and we have not had any formal
reply. Ms. Von Endt did talk with a gentleman in the Public
Works Department, but he had no knowledge of the letter. He.
told her that the City did not maintain alleys, only streets.
In the past, The City of Edmonds has maintained the alley in
question. The City surfaced it with oil and gravel during
the Summer of 1970, and again about 1978, as part of Street
Maintenance Plan. From these past actions it would appear
that the gentleman Ms. Von Endt spoke with was in error.
The use (i.e. traffic) of the alley has increased considerably,
as has the surface water problem. Thus, our request is for
more extensive repair than just resurfacing. At the very
least, however, it should at least be re -oiled and graveled.
In the interest of the community, we hope the proper corrective
action will be taken so that the run-off of dirt and gravel
onto 7th Avenue during heavy rains will be eliminated.

RE: The Alley that. runs East of Seventh Avenue South,
between (and Parallel to) Laurel and Pine Streets.
It has been approximately eight years since the City applied
oil and gravel to the above described alley, and it is in need of
repair. During heavy rains, water springs from under the surface
in several locations. Water also erodes the surface at some
locations carrying away material that we as individuals have put
in place to correct the general deterioration. Improved water
control and resurfacing are needed. The alley carries. considerable
traffic since i-t is the primary access to five of the seven homes
that border it.
We the undersigned request, that as a part of -the City of
Edmonds Street Maintenance Plan, thEt the above named alley be:
1. Recontoured so that the surface will be concave,
rather than "crowned", thus, enabling it to carry
the surface water. A "drain", while not essential,
should be installed where it intersects with 7th-
Ave. S. so that the water will be carried by the
existing system.
2. Resurfaced. Preferably paved with asphalt to
enhance the water carrying capability and improve
the durability, in contrast to oil and gravel.
3. That the recontouring and resurfacing occur so as
match and adjoin our existing driveways.

SUBJECT: REPORT ON ALLEY CONDITION DESCRIBED IN LETTER OF
DECEMBER 18, 1978
As indicated in the attached Aareement, the alley was improved
in 1970 by the application of a seal coat (oil coat with -ravel
topping). This improvement was accomplished jointly by the
Public Works Department doing the grading and by the abutting
residents paying a contractor for the application of the seal
coat." The seal coat surfacing has a limited life expectancy
normally two to five years under light loading conditions. The
residents indicated in their letter that until recently the
surface appeared in satisfactory condition. However, the Street
Division indicates there was evidence of deterioration occurring
prior to the work by the contractor.
Subsequent to the house construction, the Community Development
Director's letter of September, 1977, stated it was the responsi-
bility of the builder (developer) to repair any damage. Since
this time there has been three changes in inspectors in both -11he
Public Works Department and Building Division. Accordingly, it
appears that some continuity in follow up was overlooked. Our
newly established procedures should prevent this occurrence in
the future.
The contractor for the developer or owner has indicated the willing-
ness to contribute to the resurfacing. The original surface job
cost the residents $83.00. The total job today will cost $160.00.
Therefore, it is my recommendation that the Public j,!orks Department
accomplish a new seal coat in the Summer of 1979, with the contractor
contributing; $100.00 and the residents contributing $60.00. I feel
this is fair and equitable and request your concurrence.

Subj: Restoration of alley that is between, and parallel to,
Laurel and Pine Streets running East from Seventh
Avenue South.
1. Prior to the building of the home at 909 Seventh Avenue
South, the subject named alley had an oil and gravel surface
which was hardened and satisfactory to the adjoining property
owners. The excavation equipment and heavy trucks employed in
the construction process broke through and "tore -up" the
hardened surface leaving an "unpaved" mostly dirt condition.
2. Since early this calander year, all of the undersigned have
contacted the Edmonds Public Works Department on one or more
occasions regarding the condition of the alley. Mr. Bobby Mills
came out to view the situation approximately 8-months ago. We
have been assured that the contractor, Mr. Spencer H. Horning,
would be made to restore the alley to its original condition.
This has not occurred, despite the statements in the attached.
3. Recent attempts to contact Mr. Mills, by most of the
undersigned, have been futile. Initial telephone calls are
either not returned, or there is no follow-up call to report the
results of promised investigation into what is being done.
4. It has now been over one year since the heavy construction
was completed, and we are tired of living with an alley that is
of a lesser quality than we originally paid for. We sincerely
request that the Edmonds City Council take appropriate action
to correct this unsatisfactory condition. Thank you for your
attention in this matter.
