It is understood that some of the items noted below may already have been corrected at this time. A follow-up inspection shall be scheduled by the contractor when all items have been corrected or as needed to complete individual phases of work. 9/4/2018 — Original Inspection 9/17/2018 — Re -inspection BLD20170493 — Fill/Grade/Excavate (civil site and utility work): 1. Contractor to provide photos of water service line installation to engineering inspector. 9/17/2018 - Photos not received. 2. RPBA in hotbox and water quality reports to be inspected by City Water Quality Specialist — Jeff Koblyk — 425-771-0235. 9/17/2018 - Re -inspection and follow-up to be from Jeff Koblyk. 3. Trash enclosure - Drop pin holes not installed for use when gate is in open position and drop pin does not exist on north gate. 9/17/2018 - Correction not addressed. 4. Entire storm system to be cleaned. 9/17/2018 - Informed cleaning was completed — please email copy service performed. 5. Obtain Fire Department final. 9/17/2018 - In process. 6. Construction debris to be removed from site to ensure storm system remains clean and functioning properly. 9/17/2018 - Much of the site has been cleaned, few materials remain. Will inspect again prior to final. 7. Electronic asbuilt of civil construction improvements to be provided to the City. Asbuilt shall include complete civil plan sets. 9/17/2018 - Not yet received. ENG20170122 — Stormwater (stormwater detention vault 1. Entire storm system to be cleaned. 9/17/2018 - Informed cleaning was completed — please email copy service performed. 2. ok 3. ok ENG20170286 — Right -of -Wad 1. ok 2. Open trench along backside of sidewalk along Highway 99 needs to be backfilled. 9/17/2018 - Final restoration in process during inspection. Will inspect again prior to final. 3. The utility pole on 2121h St SW is to be relocated to the back of the sidewalk. Separate communications on this are taking place with Peter Chung and David Estes. If this item has not been resolved prior to final certificate of occupancy, a performance bond shall be held in place until the work is complete. With a performance bond in place, final C of O will not be held back for this item alone. 9/17/2018 - This issued has not yet been resolved. 4. Sidewalk surrounding PUD pole on 2121h St SW was not replaced with temp asphalt as requested. Concrete was placed, but was not poured with joints placed to meet City standards and the section immediately around the pole was not poured. In addition, the sidewalk was poured around the guy anchor within the pedestrian path of travel. Trash Enclosure: Drop pins required on both sides of gate. Sleeves to be placed in the asphalt for the pins to drop into when in the open position. 9/17/2018 - Correction not addressed. 2. ok 3. ok ENG20170333 — Side Sewer I . Oil/Water Separator in front of mechanic bays could not be opened due to asphalt debris stuck in the latch. Please remedy and ensure the separator has been cleaned and operating properly. 9/17/2018 — Hatch is now operable. Contractor to verify what white PVC pipe is coming into the south wall of the oil/water separator. 2. Install twist plug in sewer cleanout located in paved drive aisle near north end of mechanic bays. 9/17/2018 — A twist plug was added at this particular cleanout. It was discovered during the re -inspection that an additional sewer cleanout, just east of the oil/water separator, does not have a twist plug. Please correct.
