Ihe inspection was conducted on February 9,2001 by John Terrill, an AHERA certified Building inspector. This inspection was performed at the request of Mr. Alan Blodgett of Total Concept Services. inspection was to identify asbestos containing materials that may be The purpose of this present in this structure. This asbestos survey section is written to comply with the AHERA asbestos sampling procedure as stated in 40 CFR 763.86. This protocol is required for all asbestos surveys prior to a building demolition under the Puget Sound Clean Air Agency, (PSCAA), Regulation HL Article 4, revised February 1998. This asbestos survey also satisfies the "Good Faith" asbestos inspection requirements as stated in Washington Administrative Code (WAC) 296-62-07221(2), "identification". As stated in this regulation, the owner of a structure scheduled to be demolished must provide a contractor with a written report of the asbestos containing materials to be disturbed during demolition. The National Emission Standards for Hazardous Air Pollutants (NESHAP), 40 CFR Part 61 requires a survey by an acc redited asbestos inspector prior to dernolition.of a structure. The walk-through inspection include internal and external aspects of this structure. The locations and types of potential asbestos e, containing materials were noted. Sufficient bulk samples of building materials (vinyl floor til a requirements of the roofing, wall materials, etc.) were obtained of each material to satisfy thi 0" Asbestos Hazard Emergency Response Act (AHERA) rules and regulations. If samples are not homogeneous, then subsamples of the components were analyzed separately. All Bulk samples are analyzed using EPA Method 600/R-93/116 with the -30 -9%, 10%=5- following measurement uncertainties for reported % Asbestos (I%=0 /6, 5%=>l 15%,20%=I0-30%,50*/6=40-60%). For samples contaLnmg more than one separable layer of materials, the report will include findings for each layer (labeled Layer; 1, Layer: 2, etc.). The asbestos concentration in the sample is determined by visual estimation. Sprayed -on ceiling texturing material (popcorn): (material number T210215-1) There is approximately 400f t2 of asbestos -containing sprayed -on ceiling texturing material (popcorn) on the ceiling in the living room and the bedroom. 2. Sheet vinyl flooring - kitchen: (material number 1210215-2) There is approximately 60 ft'of sheet vinyl flooring with asbestos -containing fibrous backing in the kitchen. The substrate is wood. 3. Sheet vinyl flooring - bathrooni: (material number T210215-3) There is approximately 20 it' of sheet vinyl flooring with asbestos -containing fibrous backing in the bathroom, Tl-ds material is on top of another layer of sheet vinyl that does not is wood. contain asbestos. The substrate 4. Mud compound: (material numberI210215-4) There is an undetermined quantity of asbestos-contaft-dng mud compound associated with gypsum wall system. This material is less than 1 asbestos if it is considered as a component of the composited wall system. -12) 5. Asphaltic sealant: (material number M10215 There is approximately 5 ft' of asbestos containing asphaltic sealant around the wood stove chimney at the roof top.
