I arrived at the Thomas residence at 10:30 AM to observe the installation of Chance
Anchors. When I arrived, Davis
construction had already excavated seven holes below the foundation and had chipped away the overhanging foundation
in four ofthe holes. The excavated soil generally consisted ofan olive brown, silty fine sand to fine sandy silt with
organics, gravel and cobbles up to six inches in diameter, and is considered fill. The holes were spaced closer than the
seven feet shown on the plans. The spacing ranged from six to three and a half feet. Additionally, the anchors along the
north side of the house extended only 9.5 feet from the northwest comer to avoid a chinmey that was I I feet from comer
and not on the plans. In addition, it was noted that the length of the house's westem wall was only about 25 feet instead
ofthe 35 feet indicated on the plans. The attached sheet shows the house configuration and actual anchor layo I was
able to observe that anchors and extensions were ofthe sizes specified, and Chance was stamped on the anchorust* Their
motor for installing the anchors was a 2,500 foot-pound motor.
Shortly after my arrival, the southernmost anchor (A-]) was attempted. However, after penetrating about 4 feet below
grade, the anchor spun. There was no additional penetration even with a man standing on the motor. The adjacent
anchor (A-2) and the comer (A-4) were both attempted with similar results. Davis then proposed to cut the 8'�-10"-12"
lead anchor assembly between the 8" and 10" helices, and weld the 8" helix and square bar to a straight extension bar
producing a single 8" helix anchor. Davis also proposed to increase the pitch oftbe 8" helix by hammering it open
further to obtain better penetration into the subsoils. According to the chance anchor literature, a single 8" helix ins alled
to 2500 foot-pounds oftorque will have an ultimate capacity of at least 12.000 lbs. At their field -located spacings and
assumed wall load of 1,000 pounds per Iffical foot, the proposed modification to the anchors will have adequate bearing
capacity to support the house loads. We accepted the proposed modifications. I left the site at noon with the request that
1 be called when they were ready to proceed.
I returned around 2:30 PM, and southemmost two anchors (A- I and A-2) had already been installed. According to
Davis, they penetrated about six inches into the dense hard layer. I observed the installation oftwo more anchors to
refusal, (A-4 and A-5). Soils at the bottom oftbe foundations in the exposed holes consisted ofreddish brown, silty fine
sand with silt and gravel to about 4 feet below grade in the soutberninost excavations. The anchors went slightly deeper
as the north side ofthe house was approached. From A- I to A-7, the depths were approximately 5', 5', 5', T, T, 6', 7'
below L,
I left the site at about 3:45 PM,

I arrived at the Thomas Residence at 9:40 AM. Davis Construction bad already placed brackets on anchors A-1,through
A-6, and were preparing to place the bracket on A-7. Anchors A- 1, A-2, and A-3 were already bolted to the foundation.

Using the jacks, the load was transferred to the anchors via the brackets. Once the bracket bolts had been tightened, the

jacks were removed, and the rest of the brackets were attached to the foundation with bolts.
I left thejob site at 11:15 AM, after all ofthe brackets had been tightened down and bolted to the foundation. At the time

of my departure, Davis Construction was preparing to backfill the excavations.
