Dear Ms. Piazza:
As a follow-up to City letter dated February 3, 2005 and the referenced Order to COJTCCt
Violation N*otice, the adopted building, pl . umbing, and mechanical codes for the State of
Washington require that you (as the property owner) obtain all necessary pen -nits and
inspections associated with installation of your gas insert. The City has repeatedly
attempted to contact you for the purpose of scheduling an inspection. You have also been
advised that the next step in the City's enforcement process is to issue a Notice of Civil
Violation and assess fines.
The City is hesitant to use the Notice and assess fines but if tile home owner ignores City
requests or simply chooses not to communicate, the City has no other option but to
continue the standard enforcement process (which is to issue a fornial Notice of Civil
Violation and impose fines of $100.00 per day). Please be advised once the Notice of
Violation is issued fines are immediately assessed and are in effect until the violation is
corrected.
Please contact the City inspection hotline (425-771-0220 extension 1333) by March 28,
2005 for the purpose of scheduling a Mutually agreeable day and time to perforril the
inspection. You may choose to have your installer oil site when this inspection is
performed in case corrections are noted that must be corrected. Also, the gas piping that
was installed for the insert must be placed under test, observable by the city inspector.
Failure to contact us by March 28, 2005.shall result in the issuance of the Notice of Civil
Violation and fines will immediately begin to accrue. If you have any questions please

Code Section Violated:. International Mechanical Code Section 107, Inspections and Testing.
Description of Violation:- 1) Failure to contact City Inspector for required gas piping test and appliance
installation inspection and,-2) Failure to request the inspection prior to the expiration date of the
mechanical pen -nit (reference #2004-0537).
Corrective Action Required: On 7/7/04 Washington Energy ;ervices submitted a mechanical permit
application for gas piping and installation of a fireplace insert on behalf of Helen Piazza. This pennit
was valid for 180 days and expired on Janua!y 7, 2005. The work has not.been inspected as required b
the International Mechanical Code and is presumed to be complete. The City, via letter, contacted the
owner, granted a pen -nit extension and provided a period of time to call for the required inspection. The
owner ignored the request for inspection. Because the pennit has expired, the corrective action
required is for the owner to remit a new application and mechanical Permit fees in the amount of
$85.00, upon receipt of the application and fee the City will process a,nd issue the per it. Once the
owner receives the new permit an inspection must be requested within 30 days date.
Correction is Required no later than:-- 5:00pm (time) March 11, 2005 -- (date)
If correction is not made by the date and time specified in the Order, a Notice of
Civil Violation shall be issued. The Notice of Civil Violation shall assess fines of
$100.00 Per day, or portion of a day, during which the violation continues.

specifically apply to your project but it does give you an idea about the kinds of things
that must be complete as well as some items that do not need to be installed such as:
paint, catpet, moldings, wallpaper, etc. Please be advised permits expire one year from
the date of issuance. if you will need more time to complete the work, permits can be
renewed prior to expiration for one half fees. IfthepernlitcxPifeswithutt Final
approval granted by the Building inspector, full fees are required in order to renew the
permit. This can be very costly to you and we strongly encourage you to check your
permit expiration date and 6all'for the required final inspection(s). Note: If more than
one department is required to final your project, this is noted on your Job Card.
