The purpose of this Visit was to review some welded connections now in
place, primarily In the service area.
We reviewed the connections of the exterior service entrance. Some
variations have been made but they are acceptable, and the total amount of
weld originally specified has been met, This area is substanitally different
fro�n the oeiginall documents, as noted in the Special inspectors Report. BY
this memo, we are approving the changes made in the field.
On the interior space� we looked at the catwalk access to the upper floor
service area. In this place as well, the weld IS In place somewhat differently
than detailed, but more than adequate with a doubling of weld thickness
along one edge rather than two.

Tom Maddock Performed visual welds inspection at cat walk steel frame at south side channel C 1 Ox25.3 to west
2/2/2005 face of existing tube steel 6x6 column at grid C.5/2 and east face of existing tube steel 6x6 at C.3/2-
Arrived at site with welds in place. Observed 1/4" welds at outside faces at top and bottom flanges
and full length vertical at web. Detail R on S2-4 calls for 3/16" all around, however inside faces at
flanges and web have access blocked by existing drywall/wall. These welded connections should be
reviewed by Structural Engineer prior to conformance and closure. Welds at bottom of four (4) Y2"
diameter pipe hangers to top of top flange at north side channel C10x2.53 were %" all around and
per detail X on S2-5.
Top of north side stairs stringer channel MC10x25 to east face of existing tube steel 6x6 column at
grids C.7/2.1 has outside faces at top and bottom flanges and web with %" welds (7 Y2" long vertical
at web) with inside faces' access blocked by existing wall. Engineer should review this welded
connection prior to closure and for conformance.

1 %4" diameter steel anchor rods to inside face to web and to flanges at W1 0 columns — bottom at
north fin wall frame at NF1/C.2, NF2/C.3, and NF3/C.4. These vertical welds were in accordance
with detail N on S2.4 except: Item 1: welds 16 W to 16 3/4" long with 18" detailed, plus torch cut
holes at column base plates.
Arrived at site and found all welds in place.
Observed 1/4" all around welds at two (2) 4" diameter pipe columns, bottom to base plates, at north
side of existing building at roof level at grids C.7/20' north of 1 and C.8/25� north of 1. These welds
wereper B & BWelding drawing 8. ltemlA: Observed top and cope cut atW12 beam top flange at
bottom to beam web and gusset at bottom of 4" diameter pipe diagonal brace to gusset plate welded
connections per Perbix Bykonen's SSK-20 dated 10/2/2004. Item 2: 1/4x7x8 1/2" plate/clip was torch
cut off and relocated 4 1/2" east and welded with %"x8 1/2" long fillet at one side, vertically, at skewed
clip to north face web of W1 2 beam at lower frame. Item 3: East end of W1 2 beam to existing east
tube steel (TS) 6x6 column connected as follows — bottom/east side of 3/8" plate by 1 1/2"x`14" strap to
top of existing TS 6x6 column has 6" long vertical W fillet with 3 W long vertical at west side of strap

long vertical with 2 1/2" bottom/return at clip to W12 beam web W fillet instead of 3 Id of 3 bolted
connection. Contractor added 3/8x8xY plate strap/washer through 2 of 3 washered nuts at this
bolted connection. Item 4: Existing west TS 6x6 column at top to W20 % beam's south end
connection as follows: - Y4" vertical fillet at east side of strap to top of existing west TS 6x6 column is
5 W long, 23/4" long vertical, 1/4" weld at west side/bottom of strap to TS column, 15"x4" clip to west
existing TS 6x6 column has vertical 114" fillet, 4" long, at top of clip to column, 2 1/2" gap at vertical
middle 4 W long weld at bottom of clip at south end of W20 % beam connection. Item 5: Plate clip
18x6 onto top of existing west TS 6x6 column with 2 1/2" long 1/4" weld and vertical W fillet 3 W long
with 4 W long middle gap with 3 1/2" long at bottom of clip to web/west face at W20 1/2south end
beam. Crew placed 15"xl 1/2" plate washer strap at east face of W20 % beam web to clip connection
with 5 round washers and nuts over 15" long plate at 5-A325 bolted connection.
Field connections (welded/bolted) differ from drawings, including items #1, 1 A. 2, 3. 4. & 5).
Recommend Structural Engineer review all field connections and provide written acceptance and/or
