At the request of Steve Dutic Out- Office Was contracted to perform the special inspection of epoxy bolt
holdowns. In addition, we were asked to re-evaluatc the need for the slicarwaUs and holdowns at the left
side of the new entryand to analyze the substitution of Simpson THU holdowns for the "I-ID8A'
holdowns.
E-12oxy Bolt Special Inspection
On December 8, 2004, our office visited the subject residence to observe the installation ofthe epoxy
anchor bolts and holdowns. For each installation a hole was drilled in the foundation to the specified depth
and diat-netcr. rhe hole was then blown out with compressed air, brushed out with a wire brush to remove
remaining debris, and then blown out again with COMPrCSSCLI air. Each hole was Filled approximately lialf
way full of Simpson 'SET -PAC' epoxy and then a clean threaded rod was pushed to the bottom usinga
turning motion. With every installation epoxy was seen oozing out of the hole.
Our officc observed the installation of every anchor bolt and holdown. It is out determination that all bolts
and holdowns were installed correctly, in conformance with the manufacturer's specifications and in the
locations specified in thestructural plans.

After re-evaluation of die loadingat the new entry addition we determined that neither the holdowns nor
the shearwall arc required. The entry addition provides no additional lateral wind load to the house and
provides a negligible amount of additional weight for seismic loading. The remaining holdowns called out
for the addition are sufficient to resist the entire added loading. Please refer to the attached drawings for
specific locations of the holdowns in question.
