According to City records, Wash i ngton Energy Services obtained a permit on your behalf to install a

gas . fireplace insert in your living room (reference permit #2005-0134 copy enclosed for your

convenience). The Uniform Plumbing Code and International Mechanical Code regulates tl le

installation of gas piping and insert units and requires a gas piping test as well as a final installation

inspection in order to close out the permit and to be assured no life -safety issues exist.

Unfortunately, as of.this date there is no record of inspection request. My staff placed several phone

calls to Washington Energy Services and confirmed that the work is finished. We understand that it is
sometimes difficult to arrange a day and time for inspection, e specially when family members are
