The purpose of the site visit was to observe the anchor bolt placement to assure —

it was in conformance with our initial damage assesement report's repair
recommendations. D
The following items summarize our observations: m rn

1. A new pressure treated 2x sill plate had been installed. The diameter and
spacing of the new anchor bolts meets the requirements listed in the initial M to

2. The contractor used wedge anchors in lieu of epoxy anchors, which is an
acceptable substitution. The contractor informed us the typical M
embedment depth was 7'/2". We could not confirm this since the anchor
bolts were already installed.
3. The contractor used round washers in lieu. of the2"x211x3/,t' square i
washers as called out in our assessment report. The washers must haveCA
