The following are our responses to the structural items of the City of Edmonds Plan Review
Comments dated March 15,2006.
1. The exception of IBC section 1614.3 applies specifically to the upgrades that are a part of
this project. The upgrades have been initiated solely for the purpose of increasing the
strength or stiffness of the existing seismic -force -resisting system. The calculations
included with the submittal meet the requirements of the engineering analysis and addr(
the items listed under the exception of IBC section 1614.3. Please note that this exceptior
appeared in Chapter 34 under previous editions of the code. Please also note that the int
of the IBC and IEBC (although not adopted) is to provide referent: documents for the des
of upgrades mandated by code for additions, changes in occupancy, etcetera. The inten
as evidenced by section 1614.3, is to allow voluntary seismic upgrades as long as it
complies with the items of the exception.

Reported to the site to inspect the added framing angles for buildings C and D perimeter roof
braces and A35 clips.
Fount the angles bolted up and tight. Found one area at building C, North side at few A35's were
missing. These were added while on site.
Also found one location on building D at approximately grids D6.5 line where on one side of
bracket to Glu-lam 3 of the 5 lag screws were missing. Advised contractor. Found all other items
per plans, details 3 on S31 and 7 on S3.2. Also reviewed RFI #4 that states nails instead of
screws as per plans can be used on A35's.
Noted during inspection listed above that many areas where in the original Construction the
anchor bolts (through bolts) from the top of the Glur-lams into masonry walls below were either
loose, nuts were 1-2" above top of Glu-lam surface and were not secure in Glu-lam to CMU walls.
Noted some of the cells where these anchor bolts were located were not filled with grout. This
item was not in today's scope of inspection or part of the on going seismic upgrade. However,
the structural engineer, Todd Wood with CPL, was advised as a courtesy and Todd stated that
this should not be a problem, as they did not account for any loading values of these anchors in
their design.
To the best of our knowledge, items inspected this date are in accordance with approved plans
and specifications.

Monitored the placement of Glacier mix #3357 (4000psi @ 28 days), which was placed and
mechanically consolidated into stair 4 treads ad landings, from the orchestra to the catwalk level.
Wire mesh and epoxy bars were present at the landings. A composite sample was taken from.
the point of placement and tested. Cast 4- 48" cylinders, to be laboratory cured for compressive
strength analysis.

On July 21, 2006 Mayes Testing Engineers performed load testing on epoxy -grouted 5/8"
threaded rods installed in the brick walls in Sherwood Elementary School located in Lynnwood,
WA. We understand that these rods were being tested because the engineer requested
inspection of four rods which were installed in hollow brick cells. The test load for the epoxy -
grouted bars is 850 pounds.
A calibrated hydraulic ram assembly consisting of a hollow plunger hydraulic ram, along with a
dial gauge and hydraulic pump was used for the test. The load was applied gradually via the
assembly until the 850-pound load was reached. In accordance with standard International Code
Council procedures for the testing of embedded anchors, the load was held for five minutes.
The four rods were tested on Building C. Two rods were tested along the north wall, and one
each were tested along the west wall and south wall. Along the north wall, the first bolt from the
NE corner successfully held the load, however, the third bolt from the NE corner reached the
load, but did not hold the load for five minutes. From a visual inspection, it appeared that not
enough epoxy was placed in the sleeve. Along the west wall, the eighth bolt from the SW comer
was tested and successfully held the load. Along the south wall, the ninth bolt from the SE corner
was tested and successfully held the load, but it was apparent that a microcrack was forming 'Iy
from the left side of the bolt. It was noted that this bolt was installed in the hole from a previous
failed bolt (tested 7118/2006).

grouted. Two of the bolts successfully held the load, however the third bolt failed under the
weight of the equipment before the test commenced.
For the second building, four bolts were tested, all of which were not installed in grouted
locations. All of these bolts successfully held the load.

Performed load testing of adhesive anchored 5/8" diameter threaded rods. CPL memorandum
dated June 26 specifies a total of four anchor tests for the entire site. Per telephone
conversations With B. McCrae (CPL), MTE will focus testing efforts on anchors installed in hollow
(versus grouted) masonry.
Coordinated with Peter (Western Ventures) to locate bolts installed in hollow block for which the
plates had not been lag -screwed to the glue -lams. A total of two locations meeting these
conditions were located in building B at approximate grid lines Ba/B17 and Ba(+)/BI5. Adhesive
anchor installations performed in Bldg C had not had sufficient curing time to allow for testing.
Performed load tested of in direct tension to 850 pounds -force as directed — both anchors
achieved and maintained the test load without incident.
In addition, noted several locations where 5/8-Inch diameter anchors were installed without the
use of a screen tube into 7/8-inch diameter holes drilled into grouted masonry. Epoxy anchor
installation can often be sensitive to oversized holes and inquired about this to Peter (WV) who
stated that an RFI had been submitted in this regard with disposition by CPL still pending.
Tested two anchors meeting this criteria at grid lines Bb(-)/BI5 and Bb/B1 I — both anchors
achieved and maintained the test load without incident.
Additional testing will need to be scheduled to satisfy the test quantity specified by CPL's June 26
memorandum.

Observed epoxying of 5/8" threaded anchors with 4" embedment into Bldg A masonry walls
approximately 8' above ground per detail 16IS3.1. Holes were predrilled, brushed and blown
clean previously. Hollow CMU's were drilled to 7/8" diameter for 5/8" diameter bolts with plastic
optimesh screens per Simpson recommendations in Simpson Catalogue page 45. Embedment
of 3-1/2" depth was approved per RFI No. 02 for substitution of 4" required by plans. Holes drilled
into solid grouted masonry were drilled using 7/8" bit. General notes state that 1/6' over drilling of
bolt size is the maximum. RFI No. 05 was written for approval from engineer to substitute 7/8" for
3/4" bit for hollow CMU. Asked Peter (Western Ventures) to get approval from engineer to use
7/8" for 5/8" rod, drilled into solid grout CMU. Approximately 90 bolts installed into solid grout and
24 bolts into sleeves in hollow CMU.
