MTC was on site as requested to inspect the epoxing of anchors. and perform the pull tests on four
sacrificial anchors.
The four sacrificial anchors were on bldg C at the roof area 2 feet below the diaphrarn sheathing.
Two anchors were perpendicular to the joist line and two anchors were parallel. These anchors were
tested at 850 pounds for 5 min. All four anchors were able to hold 850 pounds for 5 min.
Please refer to report # E 1500 dated 7/26/06 for epoxy inspection daia.

MTC was on -site as reguested to reinspect the 5 welded areas that were rejected on 9/29/06. IVITC
visually reinspected the welds. To the best of my knowledge, the welding of structural steel is
in -compliance with AWS W.I. The engineer's review is required for the following issues.
1) No fabrication shop inspection was completed.
2) The issue of a lap and fillet welds verses the plans calling for a butt weld (Item #27 on engineers
site visit report from 9/26/06).
3) Approval of the changes to avoid pipe made to the 3x nailer and angle per detail 11 on S3.2
(Item 2.2 on 9/26/06 engineers report).

MTC was on -site as requested for final inspection. MTC did not receive notification of structural
steel being welded at the fabrication shop. MTC was at the fabrication shop for Woodway Elementary
and verified welder's certification, procedures, and filler metal being used. MTC visually inspected
the welds on -site today. Some of the welds were completed on -site. MTC visually observed
rejectable defects on some of the on -site welds per AWS D1. 1. These welds will need to be repaired
and reinspected. The grid line locations of the [e
jectable welds are Bldg B (BB, 85), (13F, B2) on
S2.2a and (BF,B2) on S2.3a, Bldg C, (CE, C4) and (CE, C5).
MTC also was able to verify the following issues on the engineers site visit #2. Repot dated 9/26/06.
Item 2.1 was completed, Item 2.4 was completed per the recommendation on the report. Item 2.5
was completed. Item 2.6 was completed, ltem..#2.7 was inspected. Due to the plans calling out a
butt weld connection, the lap and fillet weld connection will need to be rewiewed by the engineer.
MTC verified everything that was possible on the list. The contractor needs to supply the sketches
for item 2.2
