MTC was requested by the engineer of record to report in more detail the epoxying of the LTT1 9
Simpson Hardware for buildings A through F at Woodway Elementa!y. MTC visually inspected this
epoxy on 7/13/06. MTC inspected this work per detail 8 on S3.1. To the best of my knowledge, this
work was completed in compliance with the approved plans, specifications, and ICC report #5279.
MTC observed the masonnt bit to the 7/8 inch diameter. The screens used were 3 1/2 inch
optimesh by Simpson Hardware. The epoxy used was Simpson Set 22. Please refer to report dated
7/13/06 for the batch numbers and expiration dates of the epoxy.

Comments: Also observed plates,on bldg B &.0 under slab. These had been placed with 5 expansion anchors on the_
plate on bidg C and on plates on bIdS B. Each plate had 2 expansion anchors out of 4 on each 2late were verbally
approved by engineer. The thread rods on bidg B & C on top could be only achieve 4-5" inbedment rather then 5" as call
for on detail 6 on 3-3 due to dimension of the brick being 5!'thick. All worK inspected was performed in a workman like
mannor and wasperformed in acco ance with project plans, specification and IBC.
