• Structural special inspection reports from MTC received 11/15/06 (last report received was
11113106)
• Deferred shop drawing submittals received 11/15/06
• FAX received 11/15/06 showing staked gridlines by surveyor
1. MTC Special Inspection Reports
a. Report C6037 cast date 7/25/06: Inspectors remarks state that the trash enclosure
walls were changed from CMU design to poured reinforced concrete walls and that
engineer's review is required. Submit revision for the concrete walls, stamped and
signed by the structural engineer (2 sets). Submit report from MTC that
reinforcement and concrete were inspected per the structural engineer's revised
details.
b. Reports F17272, F16550, & F19280: Hold down and anchor bolt issues must be
addressed by structural engineer of record and revisions submitted to the City for
approval.
c. Report F17286 dated 11111106: Submit letter from Structural engineer of record that
he has reviewed this MTC report and stressing log as requested by MTC.
d. Submit MTC special inspection reports showing these issues inspected and resolved.
2. Geo Group NW Special Inspection Reports:
a. The City has received special inspection reports 1 through 7 (last report dated
12119105) for shoring installation (pile and lagging installation) only. Provide all
remaining reports, including follow-up special inspection report for over excavation
noted in report #7, and reports for additional required special inspections (see
coversheet of approved plans) for site dewatering, subsurface drainage, placement of

fill and compaction, soil bearing verification, and general site monitoring during wet
weather construction. Please provide.
1 Shop Drawings:
a. The shop drawings from Post -Tensioning Institute are not stamped and signed by the
engineer (Diana j. Leonard, PE) and do not contain the shop drawing approval stamp
by the Structural Engineer of Record. Please provide.
b. Shop drawings were not received for the structural steel and embedded items. Please
provide.
c. Shop drawings for the prefab guardrails must be submitted to the City before
installation.
4. FAX showing staked gridlines by surveyor:
a. The staked gridline plan is inadequate to show that minimum required setbacks are
met. The plan must show the distances from the exterior wall of the building to each
property line at the closest point per the approved site plan, and must be stamped and
signed by the surveyor. It must also reference the project and address of the site.
(Building Inspector corrections dated 1117/06 and 11/15106).
b. The staked gridline plan is also inadequate to show that the building does not exceed
the maximum allowed height as shown on the approved site plan. In a letter or plan,
the surveyor must reference the datum/benchmark on which the height calculations
were based (shown on the approved site plan.), and the actual elevation of the highest
portion of the building (based on that datum). If there is an elevator penthouse or any
other structure above the main roof line, the elevations of these structures must also
be noted. The surveyor must stamp/sign the letter/plan.
