The subject application has been reviewed but cannot be completed until the following items have been
addressed/corrected. Please resubmit 3 sets of revised drawings to a Development Services Permit
Coordinator.
General:
1.Where will patients park if all parking stiping is being removed? How will handicap gain access to the
bldg from the east?

3.Existing main entrance off 210 shall remain commercial type. Existing parking lot to the west shall be
changed to a commercial entrance and the new parking lot shall have a commercial type entrance per City
standard detail E 2.27.
4.Annotate the parking stall depth (16.5’) on the drawing.
5.Be sure this site plan matches the civil site plan.

6.The existing commercial type entrance from 210 St is to remain as commercial. The new parking lot
shall have a commercial type entrance.
7.Entry width to parking lot shall be a minimum of 24’ wide. Indicate width on drawing.
8.Dimension the new parking lot and include stall width and depth and spacing between perpendicular
parking stalls.
9.Is the new rockery on the west side of the new parking lot really rockery or modular type block wall?

10.Where do the new wall drain connect to?
11.Provide an enlarged detail of the stair construction that leads from the new parking lot to the lower
existing parking.
12.Label the symbol for the new sewer ejector pump (located on the west side of the new addition).
13.Indicate the distance between the new curbing in the vicinity of the west entrance.
14.Provide a larger detail drawing of the steps to the new upper parking lot.

15.With the upper existing parking lot off 210 St SW being enlarged, the existing detention system needs to
be enlarged to accommodate the new impervious surface in that area.
16.Provide drawing a details for the new sewer ejector system. Label its location and indicate where it
connects to the building sewer line.
17.Sleeve the storm pipe that goes under the rockery.
18.There is insufficient cover for the proposed 8” pipe that goes through the new portico. Change the pipe
material from pvc to d.i.p.

19.The curb and gutter to be installed on 210 St shall be the type A standard 18” curb and gutter (detail
E2.8).
20.Ladder in the type II cb’s shall be polypropylene.
21.Change the driveway entrance from residential to commercial.
22.Indicate where the sidewalk ramp detail (as shown) will be located. The curb ramp at the parking lot
entrances shall match as noted the particular driveway entrance detail.
23.General notes;
a.note 1 and 12: Change 2002 to “current edition”
b.delete note 7
c.note 12: change ‘or as directed by the City of Edmonds’ to “and City of Edmonds Standards”

24. Comments, if any will be forwarded no later than Dec 13, 2006.

25. Provide a detailed cost break down (engineer’s estimate or contractor’s bid) for all site work both on site
and off site so the engineering inspection fee and bond amount can be determined.
