Provide written responses addressing each item and note where changes can be found on the plans. Please submit revised plans/documents to Marie Harrison, Permit Coordinator. NONSTRUCTURAL COMMENTS 1. General Site: a. Reference where 2/Al.1 is cut through the site. Could not find sections D2/A1.0 and A/Al.1 which are shown on Sheet A1.1. b. Accessible route of travel and parking: i. Clearly show the accessible path of travel from the public sidewalk (212th St. SW) to the main building entrance. Where the path crosses vehicular traffic, a cross walk is required. Show elevation points/slope along the path of travel, dimension width of path/striping (44" min.), white striping, curb ramps if applicable, etc. ii. Coordinate accessible parking and path of travel striping with civils, landscape, and site plans (and quadrant plans that show striping). iii. On Sheet C2 at the south lot, the existing CBMH near the current accessible path to the building entrance has a top elevation noted as 391. 10, which appears to be approximately 3 feet lower than the grade elevation at that area. Please verify and coordinate. This manhole cover is shown within a proposed accessible parking space which is not permitted to have a slope greater than 1:48 (ICC/ANSI Al 17.1 section 502.5). iv. Provide a cross walk (44" min width) connecting the accessible parking stall access aisles to the accessible building entrances, since they will be located within the drive aisle. v. Passenger drop off zones must meet ICC/ANSI Al 17.1 section 503. Show requirements on site, civil and landscape plans, including signage. vi. Show the accessible path of travel from the accessible parking stalls located at the covered parking area to the accessible building entrance, including elevation points, curb ramps, width, slope, etc. vii. Show locations of site accessible directional signage. c. What is the purpose of the temporary building shown on the site plan? What will be building be used for? How long will it be on site? Will it be a State approved black label commercial coach? d. What is the purpose of temporary ramp shown on the site plan? How long will be in place? Will it be part of the accessible path of travel? Provide construction details. C. On the Site Phasing Plan Sheet A0.3 and Phasing Floor Plans, show how the exit path of travel will be maintained out the lower east exit, since it appears that occupants will be directed through an area of construction. f. Similarly, on the Site Phasing Plan Sheet A0.3, how will the accessible path of travel be provided for exiting and entering from the accessible parking stalls on the east side of the property? It appears that it will direct occupants through an area of construction (2/A0.4). Also show directional signage. g. Cross-reference details 10 & I 1/A1.7 for the NW Site Stair on the site plan showing where they apply. h. Show the location of all new site retaining walls on the site plan, and cross-reference construction details and show where they apply. i. . Provide top and bottom wall elevations at 5 foot intervals for all proposed site retaining walls and rockeries. See City handout for Rockeries. j. Provide rockery construction details. See City handout for Rockerics. k. Provide detail of winder stairway from new northeast parking lot (see IBC 1009.3 for specific winder requirements) and cross reference it on the site plan. Handrails are required on both sides of the stairway including extensions. 2. NW Site Stair Details 1- & II/A1.7 (IBC 1009): i. Dimension the width of the stairway. ii. Dimension the height of the guard along the stairway and at the top of the stairway. iii. On I I/Al.7 clearly show the handrails extending continuous the full length of the stairway without a break or offset, including handrail extensions. IBC 1009.11 iv. Dimension the height of the handrails. v. Dimension handrail extensions at top and bottom. vi. The opening between the treads of the open risers cannot permit the passage of a 4" sphere (TBC 1009.3.2 exception 1). Revise details to clearly show this. vii. Dimension the landings at the top and bottom of the stairway. viii. The dimensions of the height of the stairway are illegible on MALT 3. On detail9/A1.7 dimension the top and bottom handrail extension from the tread nosing. Handrails are required on both sides of the stairway. 4. On detail A6/A1.7 an accessible parking sign is required at the head of each parking stall. Note the height of the sign above grade. (ICC/ANSI Al 17.1 Section 502.7) .5. Provide van accessible parking stall detail. 6. On Sheet A2.2 the column obstructs the landing at the top of the ramp. Show how this meets accessibility 7. Provide allowable area calculations on the coversheet of the plans. 8. Note number of stories on the coversheet. 9. On the coversheet note that the sprinkler system must meet NFPA-13. 10. A0.0 General notes: Page 2 of 5 a. Note 1- revise wind and seismic information to match Sheet S 1.0 information or remove. b. Note 8 - change UFC to IFC. c. Note 25 - add spray -applied fire proofing to list of required Special Inspections. 11. On Sheets AO.0 and A4.3 at the fire rated assemblies, note they are UL assemblies and provide the complete assembly on the plans so they are constructed and inspected properly in the field. 12. On dumpster enclosure details Sheet A1.7 note the height of the enclosure walls/fence. Provide construction detail for masonry wall. 13. Note the suite numbers for each tenant space on A1.2, and note which tenants are new vs. existing. 14. Provide manufacturer's information for the proposed overhead fire shutters at 1 -hour fire barrier between parking and office occupancies, including listed/tested assembly reference, fire rating, compliance to NFPA 252 or UL i OB. They must be automatic -closing per IBC 715.3.9, 1BC 715.3.7.2, and NFPA 80 and tied into the fire alarm system. 15. On D1/A7.1 clearly show that all pharmacy customer counters (drop off, cashier, etc.) are accessible per ICC/ANSI Al 17.1 section 904. 16. Wall types/construction details (in diamond symbol) are referenced on the floor plans but were not provided for our review. Please provide. 17. Provide restroom calculations per WAC 51-50 Chapter 29, including separate male/female facilities, and show accessible restrooms for each tenant space. 18. Provide accessible restroom details for 2308, 1133, 1116 (there is no exception for staff toilets) etc. 19. Clearly show the locations of safety glazing on the floor and window plans, including relites. 20. Columns cannot encroach into the clear width of accessible parking stalls and access aisles -- they must be unobstructed. Sheet A2.2 shows columns encroaching into the stalls. 21. Dimension the finished clear height of the covered parking area from all obstructions such as light fixtures, piping, beams, etc. (minimum 7'-0" required IBC 406.2.2). 22. Provide door schedule noting hardware, type and notes for each door. Sheet A9.1 does not provide a cross reference to each specific door. 23. Provide manufacturer's information for proposed power -operated doors showing compliance to IBC 1008.1.3.2 for swinging doors, and IBC 1008.1.3.3 for horizontal sliding doors. 24. The only exit door from the pharmacy is 1108A, a coiled security screen. Provide manufacturer's information showing it is openable from the inside without use of key, special knowledge or effort. IBC 1008.1.3.5 25. Provide occupant loads calculation for each tenant space. In the pharmacy, please clarify the extent of the retail area. 26. Verify that the common path of egress travel does not exceed 75 feet (IBC 1013.3 - See IBC 1002.1 for definition of common path of egress travel). 27. Provide detail. for latte stand counter to show that a portion is accessible per ICC/ANS1 Al 7.1 Section 904. 28. Provide details showing that the patient side of Reg. 1004 is accessible including the counter. 29. Show that a portion of the reception counter at Reception 1107 is accessible. Commonly there area 2 areas --- when a patient checks in and out. 30. If the opening next to door I I OOB is for customer service, it must be accessible. 31. Label the low partition area at waiting 1100. What is this area used for? Page 3 of 5 32. Sheet A4.9 is referenced on details on the floor plans but was not provided for our review. Please provide. 33. Sheet A8.1 was not provided for our review. 34. Provide stair construction details for Stair on 1/A2.3 35. Show how the accessible route of travel is provided (existing or will be upgraded) throughout the building in accordance with IBC 3409.5, 3409.6, & 3409.7 (see state amendments to these sections). 36. Dimension maneuvering distances at the following doors: 2310, 2336A, 2462A, 2469A, 2459A, 2470A, 2458A, 2471A, and 2422A. 37. Describe the use of space "coumadin" 2302. 38. Provide detail for reception counters 2305 and 2201 to show that a portion is accessible per ICCIANSI Al 17.1 Section 904. 39. Provide door reference number for cross -hallway door near exam 2469. 40. Some spaces not a "future" use. How will these spaces be used in the meantime? 41. Clearly shown beams that will required spray -applied fire proofing where supporting the fire barrier. 42. Assembly U434 calls for 718" thick stucco. The plans/details note 3/4" --- revise and add complete assembly to the plans so it is constructed properly in the field. 43. Is medical gas proposed for any of the tenant spaces? If so, a separate permit will be required and can be listed as a deferred submittal. 44. The Envelope Summary form submitted notes that Component Performance method of compliance was used for Energy Compliance. Provide Component Performance calculations for review and approval. 45. Special Inspections are required for the following: By Geotechnical Engineer of Record: a. Soil bearing verification b. Foundation drainage installation C. Placement of fill and compaction (if needed) d. Rockery construction, including drainage. By Approved Testing Lab or Structural Engineer: e. All items listed on Sheet S 1.1 f. Spray -applied fire proofing Complete the enclosed special inspection and testing agreements (one for geotechnical engineer of record and one for structural special inspector), have the owner, general contractor and special inspector sign the form(s) and return for Building Official approval prior to permit issuance. 46. Provide written contractor's acknowledgement statement for seismic -force -resisting system, signed by the general contractor (See IBC 1705.3) and cross reference the Quality Assurance Plan on Sheet 1.1. 47. Provide a letter from the geotechnical engineer of record that he has reviewed the plans and documents for the proposed project, including the April 1990 soils report by Rittenhouse- Zemen, and finds that they meet the recommendations in the report, or have additional recommendations. BEFORE A PERMIT CAN BE ISSUED: The owner and contractor and special inspector shall complete this agreement and the attached structural test(s) and inspections schedule including the required acknowledgements. APPROVAL OF SPECIAL INSPECTORS: Each special inspector shall be approved by the Building Official prior to performing any duties or inspections. Each special inspector shall submit Statement of Qualifications to the Building Official for review. Special inspectors shall display identification when performing special inspections on site. Special inspection and testing shall meet the minimum requirements of IBC Chapter 17 and the following: A. Duties and Responsibilities of the Special Inspector 1. Observe Work The special inspector shall observe the site work for conformance with the approved (stamped) plans and specifications and applicable workmanship provisions of the IBC. Architect or Engineer reviewed shop drawings may be used only as an aid to inspection. Special Inspections are to be performed on a continuous basis—meaning that the special inspector is on site at all times observing the work requiring special inspection. Periodic inspections, if any, must have prior approval by the City based on a separate written plan reviewed and approved by the Building Official and the engineer or architect of record. 2. Report Non -conforming Items The special inspector shall bring non -conforming items to the immediate attention of the contractor and note all such items in the daily field report. Any item not resolved in a timely manner shall be immediate cause of the special inspector to notify the Building Official of the plan deviation, error, change or omission. It shall also be the duty of the special inspector to promptly notify the engineer or architect. 3. Complete Daily Reports Each special inspector shall complete and sign both the special inspection record and the daily report form for each day's inspection. These records shall remain at the jobsite with the contractor for review by the City Building Inspector. 4. Furnish Weekly Reports The special inspector or inspection agency shall furnish the City with weekly reports of tests and inspections.
