Please redline plans or submit two (2) sets of revised plans/documents (affected sheets only) with a written response to each of the items below to a Permit Coordinator. 1) On the site plan clearly label covered porches. 2) Show how bathroom island sinks will be vented through the roof or with a loop vent as permitted by UPC 909.0.Air admittance valves are not permitted unless first approved by the Building Official under and alternate method per UPC 301.2. 3) Plans show a in ground sunken hot tub to be done by others. A permit is required for in ground hot tubs. Please either completely remove hot tub from plans and apply for separate future permit or add all applicable information to these plans for review and approval included in this application. See enclosed hot tub handout for minimum submittal requirements. 4) Provide calculation for proposed 5 1/8" x 10 %z" GLB. 5) Specify size of existing window headers supporting new trusses and provide calculation to justify size.. 6) Expand existing roof framing plan to show that the upper floor walls being removed are non- bearing. 7) Expand floor framing plan to show how the existing floor framing is being supported (beams, posts, bearing walls, etc.) and how it ties into the new trusses. 8) Special inspections are required for the following. Complete the enclosed special inspection agreement signed by the owner, the general contractor and the special inspector and submit to the City for approval prior to issuance. State Licensed Engineer of WABO certified inspector. • Epoxy Holdowns • Field Welding Y CITY OF EDMONDS SPECIAL INSPECTION AND TESTING AGREEMENT The project at aj T � , �� issued under building permit number *-� _ 02 requires special inspection and/or testing per IBC Chapter 17. The complete list of special inspections is attached to this document. BEFORE A PERNIIT CAN BE ISSUED: The owner and contractor and special inspector shall complete this agreement and the attached structural test(s) and inspections schedule including the required acknowledgements. APPROVAL OF SPECIAL INSPECTORS: Each special inspector shall be approved by the Building Official prior to performing any duties or inspections. Each special inspector shall submit Statement of Qualifications to the Building Official for review. Special inspectors shall display identification when performing special inspections on site. Special inspection and testing shall meet the minimum requirements of IBC Chapter 17 and the following: A. Duties and Responsibilities of the Special Inspector 1. Observe Work The special inspector shall observe the site work for conformance with the approved (stamped) plans and specifications and applicable workmanship provisions of the IBC. Architect or Engineer reviewed shop drawings may be used only as an aid to inspection. Special Inspections are to be performed on a continuous basis—meaning that the special inspector is on site at all times observing the work requiring special inspection. Periodic inspections, if any, must have prior approval by the City based on a separate written plan reviewed and approved by the Building Official and the engineer or architect of record. 2. Report Non -conforming Items The special inspector shall bring non -conforming items to the immediate attention of the contractor and note all such items in the daily field report. Any item not resolved in a timely manner shall be immediate cause of the special inspector to notify the Building Official of the plan deviation, error, change or omission. It shall also be the duty of the special inspector to promptly notify the engineer or architect. 3. Complete Daily Reports Each special inspector shall complete and sign both the special inspection record and the daily report form for each day's inspection. These records shall remain at the jobsite with the contractor for review by the City Building Inspector. 4. Furnish Weekly Reports The special inspector or inspection agency shall furnish the City with weekly reports of tests and inspections. The project engineer or architect, and others as designated shall also be copied on reports. Weekly reports must include the following: • Description of daily inspections and tests made with applicable locations • List of all non -conforming items • Report on status of non -conforming items (resolved or unresolved) • Itemized changes authorized by the Architect, Engineer and City if not included in non- conformance items. 5. Furnish Final Construction Report The special inspector or inspection agency shall submit a final signed report to the City stating that all items requiring special inspection and testing were fulfilled and reported. And, to the best of L.ITEMPIBUILDING1SpeciallnspectionAgreementIBC.doe 71434 his/her knowledge the project is in conformance with the approved plans and specifications, approved change orders and the applicable workmanship provisions of the IBC. Items not in conformance or unresolved items or any discrepancies in inspection coverage, (i.e., missed inspections, periodic inspection when continuous inspections were required, etc.) shall be specifically itemized in this report. B. Contractor Responsibilities 1. Notify the Special Inspector It is the duty of the contractor to notify the special inspector when work is ready for special inspection. Note, the items listed on the attached schedule and as noted on the approved plans and specifications are required to have special inspections. Adequate notice shall be provided by the contractor so that the special inspector has time to become familiar with the project. 2. Provide Access to Approved Plans The contractor is responsible for providing the special inspector access to approved plans at -the jobsite. 3. Retain Special Inspection Records The contractor is responsible to retain at the jobsite all special inspection records submitted by the special inspector. These records are to be provided to the City building inspector upon request. C. City of Edmonds Building Department Responsibilities 1. Approve special inspectors or inspection agencies The building department shall approve all special inspectors and special inspection requirements. 2. Monitor special inspection and approve weekly reports Work requiring special inspection and the performance of special inspectors shall be monitored by the City Building Inspector. His/Her approval must be obtained prior to placement of concrete or other similar activities in addition to that of the special inspector. 3. Issue Certificate of Occupancy The Building Official may issue a Certificate of Occupancy after all weekly special inspection reports including the final report have been submitted and accepted. D. Owner Responsibilities The project owner or the engineer or architect of record acting as the owner's agent shall fund special inspection services. E. En 'neer or Architect of Record Responsibilities The engineer or architect of record shall include special inspection requirements on the plans and specifications. AREAS CHECKLIST (See Handout #P20) A Critical Areas Determination, issued iy the Planning Division; must be completed and on file with the City. Provide applicable information as indicated by the decision. 2. PLOT PLAN (See Xuachment A) Three (3) copies, scaled 1"=20', one (1) of the three (3) plot plans must be no larger than $%" x 14" with the following: a) Property lines, all existing structures within the property, distance from structures to property lines_ b) . SPA OR HOT TUB PLANS a) For spas/hot tubs on decks indicate the maximum weight (filled with water) per manufacturer's specifications. Provide deck framing and foundation plan, including beam calculations. b) Provide spa or hot tub dimensions, and number of gallons, provide piping layout. c) Indicate how spa or hot tub will be filled. d) Provide manufacturer's specifications on all equipment and plumbing details of water. supply and drainage. e) Specify model number and type 'of reduced pressure backflow assembly and/or hose bibb. f) Provide heat loss calcplations to show compliance to the current edition of the Washington State Energy Code. NOTE: Spas heated in excess of 90 degrees must have an insulated cover. 4. FENCE AND GATE DETAILS (See Attachment B and Handout #B31) As required by ECDC 19.00.005(C)(7) the spa or hot tub area must be completely surrounded by a substantial fence at least six (6) feet in height, unless a hard top is provided. No openings in the fence or gate shall be greater than two (2) inches and a self-closing, self latching gate must be provided with an inside lock inaccessible to children. The plot plan must clearly show the extent of the required fencing. NOTE: City Ordinance requires that the gate be securely locked when the spa or hot tub is. unattended and fences that exceed 6 feet in height must obtain a variance. S. CROSS CONNECTION CONTROL REQUIREMENTS Protection of the City's potable water system shall be provided by the homeowner. A reduced pressure backflow assembly on automatic fill, and/or chemical injected type systems - is requifed. Manual filled systems must have a non -removable type hose bib vacuum breaker installed on the supply faucet. Contact the City Water Department for a list of approved hose connections. All plumbing connections shall be inspected by the City Water Department Cross Connection Control Specialist. For. inspection call 425-771- 0235 extension 1644. POOL, DRAINAGE DETAIL No direct connection shall be made between any swimming pool, hot tub or spa to any storm drain, city sewer main, drainage system, seepage pit, underground leaching pit, or sub -soil drain. NOTE: it is unlawful pursuant to ECDC Chapter 19.00.005(C)(12)(a), to drain any swimming ,pool, hot tub or spa, through any City owned grinder pump without prior City approval. Also, it is unlawful to drain chlorinated water directly into the ground or into any City stormm drainage catch basin. Illegal discharge of chlorinated water is subject to State Department of Ecology mitigation and possible fines as well as city violation compliance fines and permits. 7. SNOHOMISH COUNTY HEALTH DEPARTMENT APPROVAL Snohomish County Health Department approval is required for all pools, hot tubs, spas etc. for other than single family residences. S. SEPARATE PERMITS & INSPECTION REQUIREMENTS a) A separate State Electrical permit is required for electrical work. For information and inspection contact the State Electrical Inspector at 425-290-1309. b) The City Water Department requires an initial cross connection control installation inspection to be performed by the City Cross Connection Control Specialist prior to final installation approval. Contact the inspector at 425-7710235 extension 1644. c) All backflow assemblies shall be tested by State Certified Backflow Assembly Testers (BAT) upon initial installation and then annually thereafter. Copies of all BAT test reports must be submitted to the City Water Department for review and approval_ Contact the Water Department at 771-0235 extension 1644 for more information. d) Hose BibbsNaccuum Breakers are required to be placed on garden hoses when used to fill any hot tub or spa. Contact the Water Department for a list of approved hose bibbs. The initial installation inspection is conducted by the City Cross Connection Control Specialist. e) If the hot tub or spa will be filled from a City Fire Hydrant a separate Hydrant Use permit is required_ Obtain permit from the Public Works Department. A water charge shalt be levied based on the number of gallons used. f) The Building Inspector shall be called for the following applicable inspections: erosion. control, setbacks, plumbing, sanitary extension and cleanout, unit equipment, gas piping, equipment shed location and final inspection. g) A separate mechanical equipment shed shall be required if the structure exceeds 120 square feet. (See -Handout #B2_ All structures regardless of size must meet minimum setback requirements of the applicable zoning district. Contact the City Planning Division for more specific information. h) A' separate fence permit is required if deferred. (See Handout 03I) i) A separate side sewer repair permit is required for the installation of the sanitary tee (pool clean -out) - (See Handout #E7'1) 9. OVERFLOW DRAINS When overflow drains are required/provided indicate on the plot plan where the line will be connected to the sanitary sewer. NOTE: THE PURPOSE OF THIS HANDOUT IS TO ASSIST THE PUBLIC IN COMPLYING WITH DETAILED PERMIT SUBMITTAL REQUIREMENTS.
