Description of Violation: 1) Installed gas washer/dryer (reference permit #2006-1033) without
obtaining final Cityinspection Uproval and, 2) The permit expired on 3125/07.
Corrective Action Required: Call for final inspection by calling the Inspection Hotline at 425-771-
0220 extension 1333. Inspection hours are Mondgy to Friday from 7:30am to 11:30 am and 1:15pm to
3:45 m. Failure to call by the deadline below shall result in the necessi1y of obtaining a new permit with
new fees.
Correction is Required no later than: 5:00pm (time) April 11, 2007 (date)
If correction is not made by the date and time specified in the Order, a Notice of
Civil Violation shall be issued. The Notice of Civil Violation shall assess fines of
$100.00 per day, or portion of a day, during which the violation continues.

RE: Order to Correct Violation Notice Dated March 29, 2007 for Building C
To Whom It May Concern:
As a follow-up to the referenced Order to Correct Violation Notice, a permit was obtained for
installation of stacked washers and dryers in units 108, 208 and 308 in Building C. The governing
code authority (the Uniform Plumbing Code and International Mechanical Code) requires that
installation approval be granted by the City Inspector. This inspection assures the health, welfare and
safety of all building inhabitants as well as guests for the installation.
The City utilizes the Civil Penalties ordinance and fines as a last resort to gain code compliance. Once
a Notice of Civil violation is issued (note this is the next step in the process) fines of $100.00 per day
are imposed and continue to accrue at the same rate until the City approves the installations. Be
advised, once fines accrue to the amount of $5,000.00 the City shall utilize our collection agency for
the re-couping of fines and, pursuant to State RCW Law, the collection company collects fines at a rate
of 150% of the amount owed plus added administrative costs.
We strongly urge you to contact our City Inspector at 425-771-0220 extension 1333 to schedule for the
required inspections by April 27, 2007. This is the last correspondence you will receive in this matter,
failure to schedule an inspection by this date shall result in the issuance of Notice of Civil Violation
and fines of $100.00 per day shall accrue until this matter is resolved to the satisfaction of the City.
Lastly, the contractor of record for the permit "The Plumbing Physician" has informed the City that
they are not responsible for this work and have removed themselves as contractor for this project. You
are required to immediately inform the City of the name and license number of the plumber who
performed the work. The plumber must be State licensed and must also have a City business license.
