Alternative Environmental Technologies (AET) conducted an asbestos survey of the subject site on 3118108 and follow-up sampling on 03/24/08. Mr. Anthony Ramirez was the site contact and provided access and authorized the survey. The purpose for the survey was to voluntarily comply with permit and notification requirements necessary for building demolition._ inspection: Brian Hunt, AET Project Manager, inspected the subject site and took bulk samples for analysis of suspect asbestos containing materials in the Taco Time Restaurant located at the subject address (See Photographs). Sampling & Analysis. Brian Hunt took a total of 4 samples from various building materials on 3118108. Since the roof sample was positive and suspected of being heterogeneous, two additional room samples were taken on 03/24/08. Modified AHERA methods were used including strict chain of custody procedures for labeling, recording, transporting and release of the samples to Seattle Asbestos Test for analysis. The samples were subsequently analyzed for asbestos using Polarized tight Microscopic (PLM) methods. AET building inspectors are AHERA certified_ The user requested five-day tum around for the initial analyses and rush analysis on the two additional samples. The materials sampled included, roofing material, a gypsum wallboard composite, storage room wall surfacing material & 2' x 4' suspended ceiling tile. Results: The results can be found attached with the chain of custody and analytical reports. The total materials sampled are listed below in Table 1. The only asbestos containing material reported by the laboratory for samples taken at the subject site was the built up asphalt roofing material above the drive through and over the storage area. Discussion: Any contractors working with the asbestos containing material should be provided a copy of this report prior to starting work activity. Asbestos -containing materials refers to any material containing more than 1% asbestos. 1. PSCAA Notification: The Puget Sound Clean Air Agency (€'SCAA, Regulation Ili, Article 4) requires notification for asbestos removal and demolition and dust control measures for building demolition and remodeling. Questions regarding PSCAA requirements should be directed to the asbestos staff at 800.552.3555, or for a notification form and regulations go online at www.pscleanair.org. Unless asbestos containing materials are going to be removed by the homeowner, certified asbestos workers are required for removal of asbestos containing materials as identified in this survey. Lead The: building was reported built prior to 1978. The paint on the buildings was not sampled for lead analysis and therefore painted surfaces should be assumed to contain lead_ In the state of Washington any level of lead is regulated. Therefore, contractors should be notified prior to demolition for worker protection and the use of lead safe work practices. 5. Biohazards` None observed. Limitations: Contractors will need to do their own asbestos containing material take offs for bid purposes. Work activities that create dust, regardless if conducted by the owner or contracted out, may require respiratory protection, training and if done by a contractor, air monitoring to demonstrate that there has been no worker exposure to asbestos or lead in construction during demolition activities. Although this survey is considered a complete inspection of all visible and accessible building materials, the survey did not involve destructive or invasive sampling techniques of all ceiling & wall spaces as the business was occupied and operational. Therefore, if any suspect asbestos containing materials are uncovered in hidden spaces during demolition they should be sampled to determine if they are regulated, prior to disturbance. The contacts and regulations provided herein should not be considered comprehensive and have been included for user review and use if there are any questions concerning worker safety and notification requirements. Summary: Asbestos containing materials were reported for the subject site for the roofing material over the Drive Through and the storage room. The sample of the main roof reportedly installed in 1995 was negative for asbestos. The material was determined to be in poor condition and it may become friable (turn to dust) upon removal with power tools. Asbestos is regulated and the information for completion of the required notification has been provided herein.
