This project is d for use or licensure and construction is not authorized to begin.
Authorization to begin construction may be issued once significant issues with the construction documents
have been resolved. The project will not be approved until the comments labeled as "not approved" have
been resolved by providing a written response to each of the comments. When referencing drawings or
attached materials, provide sheet or page #, revision # and date as applicable. Additional comments may
follow based on design team response and site inspections may be required to verify compliance. All
documents submitted will be retained in our office until all the construction documents have been received,
reviewed, and accepted. When the project is authorized to begin construction, a complete package of
accepted documents will be stamped and signed by the department. One copy will be returned to the facility
administrator to be kept on-site for the duration of construction and one copy will be retained for our records.

The preceding are preliminary comments provided as information and for use preparing the
construction documents. These preliminary comments may be revised during subsequent
submissions/conferences. Additional preliminary comments may be made during subsequent
submissions.
T1 Provide furniture plan showing exam table or chairs and fully dimension rooms and clearances.
Evaluate use of exam and procedure rooms and fully describe in the functional program.
T2 Provide life safety plans showing all rated partitions and general life safety information including
locations of exits, horizontal exits, travel distances and exit widths.
Review 2000 NFPA 101 Chapter 38 for life safety plan requirements for a Business occupancy.
T3 Provide two complete sets of mechanical, plumbing, and electrical plans. Plans shall be
coordinated between disciplines and must be stamped and signed by a Washington State licensed
architect or engineer complying with all applicable codes, regulations, and requirements. WAC

T4 Provide two sets of fire alarm and fire sprinkler drawings. WAC 246-320-505 & 2009 IFC
Section 903 & 907
T5 Provide a comprehensive functional program for each service within this project. This document
shall be prepared by the hospital staff and describe the functions and services affected by the
project scope. Generally, descriptions in the functional program should relate to the floor plan.
Include all procedures being performed, equipment used, and a narrative of support relationships.
Identify locations and types of sedation and medications used. If medication or sedation is not
used in this project, then state so in the functional program. WAC 246-320-505(2)(c)(i)(A) &

El The plans received 10/18/11 were incomplete, however adequate written information was
provided in the functional program and project manual to review the project.
Verification that all code requirements have been met can be achieved through a site visit
prior to project approval. WAC 246-320-505(2)(i)
2 ❑x Inconsistencies were found between the spec sheets, drawings, project manual and
functional program. Provide direction as to which information shall govern in these

7.1.3.2.
Exit enclosure shall provide a continuous protected path of travel to an exit discharge,
unless the stair meets the requirements of section 7.7.2. NFPA 101:7.1.3.2.2
Provide revised life safety plan showing location of rated walls, and provide rated door &
wall assembly. Doors to exit shall have closers.
4 ❑x Vertical openings, including elevator and mechanical shafts are required to have a 1 -hour
fire rated enclosure. NFPA 101:38.3.1.1 & 8.2.5.2

modification shall be submitted for review and approval prior to system installation.
Plans and specifications shall include, but not be limited to, a floor plan; location of all
alarm -initiating and alarm -signaling devices; alarm -control and trouble -signaling
equipment; annunciation; power connection; battery calculations; conductor type and
sizes; voltage drop calculations; name, address, and phone number of the agency
receiving off -premises transmission of alarm; and the manufacturer, model numbers, and
listing information for all equipment, devices, and materials. Incomplete plans and
specifications will be returned without review. Plans and specifications may be submitted
separately from construction documents during the construction of the project.

2" d floor. Identify the location of the biosafety cabinets, work areas and additional
information to clarify the use as described in the functional program. FGI 1.4-1.3.1
29 0 Not enough information was provided to review the pharmacy. Review FGI Section 2.2-
4.2 and provide detailed information on plans or in the functional program for all
requirements.
30 0 Mechanical plans appear to be incomplete. System must provide ventilation to all rooms
listed in 2010 FGI Table 7-1. Exam 131 does not have a supply or return in the room.
31 0 Provide mechanical schedule showing that the mechanical system meets the requirements
of 2010 FGI Table 7-1. Provide info on individual room pressurization, air changes per
hour, and outdoor ACH.
32 0 Provide cut sheet for plumbing fixture P4 and P413. If sinks are required for cleaning of
equipment and other lab needs, an additional sink shall be provided for handwashing. A
double basin sink may be provided in lieu of this requirement.
33 0 Plumbing fixture P2 has a non -gooseneck faucet. Verify there is adequate clearance for
washing hands. A gooseneck type faucet is recommended. Clarify whether there are
sensor faucets at all locations per the functional program.
34 D Per eyewash spec, the manufacturer states that the product may not meet ANSI Z358.1-
2009. This is required by the FGI and maybe required by L&I. Please clarify that this
equipment is desired by the facility. DOH does not regulate eyewash locations, and so
this equipment may be acceptable. FGI 3.1-8.4.3.7

the requirements of the applicable licensing regulations found in the Washington State Administrative Code and associated references. These
comments do not relieve the facility from the responsibility to meet the requirements of any other applicable federal, state or local regulations.
In the event of conflicts between other jurisdictions and these written comments, the most stringent shall apply.
