Suite 101 Seattle, WA 98134-1805 Re: Holy Rosary roof safety Dear Mike, DAVE EARLING MAYOR In response to your request, the City of Edmonds Building Division has reviewed the fall protection plan prepared by Clarence Atchison of Safety Training and Risk Reduction Inc. as an alternate method to the roof level guard requirements of IBC 1013.5. Based upon the document submitted, the city feels the intent of the code is met and hereby approves this request based upon the following factors: The installation of the roof hatch was not required by code but was installed voluntarily. Adding a permanent roof access hatch and ladder is .more stable and safe than a temporary ladder. The roof has been constricted with a permanent safety anchor system and will only be accessed according to the provisions of the approved fall protection plan. The anchors will be used by trained workers to attach lifelines, lanyards, and harnesses prior to the performance of any work. The roof hatch will be kept locked with the key held by a designated competent person. Box 206 760 ALOHA STREET EDMONDS, WASHINGTON 98020 Fall Protection Work Plan Holy Rosary Parish Mall 760 Aloha Street Edmonds WA. 98020 (425) 778-3122 FAx: (425) 672-4909 parislimail@holyrosaryedmonds.org The implementation and use of this plan must be under the supervisor of a Competent Person for any work on the roof area accessed by the Roof Access Ladder. Competent Person is one who is knowledgeable and has control and authority. Access to the roof using the access ladder and hatch must be limited to those persons that have been trained on this plan and have permission of the Competent Person to access the roof. The roof hatch will be locked to prevent unauthorized persons from having access to the roof. The key will be under the control of the Competent Person. Competent Person: Date The roof is a flat roof with a parapet that is not adequate for fall prevention. The Roof hatch is in close proximity to the roof edge/fall hazard. Any maintenance work needed will not be in close proximity to the roof hatch for the purpose of this plan. Any authorized worker needing to access the roof must be trained on this plan and the use of a full body harness, lifeline and lanyard system. The manufacturer's instruction for the safe use of this equipment will be used for the training. All training will be documented. The roof anchors installed on the roof will be used along with the Harness, lifeline and lanyard to form the fall protection system. Only one worker to an anchor. r:1111011111 it +- . ., protection system: A worker will need to use a backpack system to carry the fall protection equipment to the roof using the vertical access ladder. The worker must have both hands free for climbing. The worker must use the ladder hand hold extension device installed on the ladder when getting off the ladder or when getting back on the ladder. Once on the roof the worker will travel directly to the nearest anchor point and attach the lifeline to the anchor. The worker will put on the harness as trained and use the lanyard attached to the lifeline. The lifeline and lanyard will at all times limit the workers ability to approach the roof edge or to fall. A self -retracting life line may be used in lieu of a safety line and lanyard. Additional training is required for this equipment. Handling, storage and securing the fall protection system: Body harnesses, lifelines, and any rope grab or other attachment's will be stored in a clean dry storage area safe from damage or exposure to any chemicals or sunlight. When not in use, gear will be in secure storage to prevent theft. When equipment becomes wet during use, it will be hung to dry in a heated area prior to storage. A Celebration of k6th, a Commitment to a New Century. Overhead protection for workers who may be in the area below the roof edge should not be an issue for most work. In the event a hazard is created the area below will be roped off with danger tape or rope and signs until the hazard no longer exist. All tools and materials will be kept 6 feet back from the edge of the roof. Removal of injured workers: If a worker is injured while on the roof, first aid will be administered as needed and evaluation for further care if needed. If the person's condition appears serious, a call to 911 will be made for assistance. Tell the 911 operator the injured worker is on a roof. If a person has fallen and is suspended in harness, a man lift or ladder will be quickly brought to allow them to descend safely. If that person is unconscious or nonresponsive, the supervisor or designee will immediately call "911" for assistance in lowering them.
