The plans appear to meet the provisions of the State Board of Health, Primary and Secondary School Regulations (WAC 246-366), and are approved. Please note the following: 1. Any significant changes or additions to the plans must be approved by the Snohomish- Health District. 2. Prior to opening, verification is required which certifies that the ventilation equipment and features have been installed according to the approved plans, and that the actual background noise at any student location complies with WAC 246-366-110. 3. Prior to opening, a pre-occLipancy inspection of the school is required under WAC 246-366-040(2). Please contact this office at least 10 working days prior to schedule the inspection. A fee for this inspection is required per the current Health District fee schedule. 4. Health District plan review and approval of the proposed cafeteria kitchen facilities is required per SHD Sanitary Code Chapter 10 (WAC 246-215). Please contact the Food Safety Program at 425- 339-5250 regarding application submittal requirements. In addition, a site visit was conducted on June 2, 2015. The following comments pertain to the existing playground: The National Playground Safety Institute (NPSI) strongly urges all playgrounds to be compliant with national standards to ensure a safe play environment for all children. The safety concerns addressed below are based on two primary documents: 1. American Society for Testing and Materials (ASTM) F 1487-01: Standard Consumer Safety Performance Specification for Playground Equipment Use. 2, Consumer Product Safety Commission (CSPC) Handbook for Public Playground Safety. No. 325, April 2008. 3020 Rucker Avenue, Suite 104 0 Everett, WA 98201-3900 m tel: 425.339.5250 N fax: 425.339.5254 Taine Wilton June 3, 2015 Page 2 Subject: Plan Approval: Lynndale at Woodway Elementary School Remodel Site Address: 9521 240th St SW, Edmonds WA It is recommended that the following safety concerns are addressed before students occupy the school: • ASTM 8.13.7, CPSC 9.5 The rolling portion of the track ride was exposed, making a pinch, crush and shear hazard. There should be no accessible pinch, crush, or shearing points on playground equipment that could injure children or catch their clothing. • CPSC 4.2 The rubber guards on tire swings are showing signs of wear and should be maintained. • CPSC 4.2 Paint was noted to be flaking on older equipment and should be maintained. • CPSC 2.4 No protective surfacing is installed around equipment use zones of the large climbing tire. Earth surfaces have poor shock absorbing properties and are not recommended. Refer to CPSC Playground Safety Handbook for appropriate types and depths of protective surfacing. Note: All protective surfacing needs to extend 6 ft in all directions from play equipment. Never use less than 9 inches of loose -fill material such as mulch, wood chips or pea gravel.
