The work on this project was performed in conformance with the Permit Drawings prepared by Freiheit
& Ho Architects and CG Engineers dated 9/16/16 and approved by the CRY of Edmonds. Special
inspections required on this project were listed as adhesive anchors, concrete and structural steel on
SIA.
The permit documents only called for adhesive anchors at the new column base plates for the columns
that were replaced along the storefront. These adhesive anchors were re -engineered as mechanically
fastened, thus eliminating adhesive anchors from the project permit documents.
The adhesive anchors that were inspected by Mayes were the dowels that Aldrich and Associated
inserted into the building perimeter slab, where possible, as part of Aldrich's standard practice for new
slabs on grade, although not required by the permit documents. Additionally, because portions of the
slabs were to remain to be replaced in a future phase of the project, the dowels were not extended into
the slab on the north and east slabs. Dowels could not be inserted into 8" un-grouted CMU at the north
portion of the south wall. The dowels are not required for gravity slabs on grade, nor were required in
the permit documents, but since they are "adhesive anchors" they fall under the purview of Special
Inspections.
The permit documents call for the removal and replacement of existing slab on grade with new. The
exisfing slab was unreinfbrced. The new slab was a bidder design by contract as follows: 04" thick with
#4 bar@ 12"O.C. each way," The concrete mix design used was 5,000 psi concrete. The minimum
compressive requirement for interior concrete slabs on grade is 2,500 psi as per IBC 1904.2.2.
There was not any structural steel installed as part of this project.
All of the above work was observed to be performed in conformance with the permit documents, the
IBC and Aldrich and Associates enhanced standard practices. We have attached the pertinent permit
documents for your review and expect to receive a final inspection letter from your firm on behalf of the
Owner, BARC Properties LLC, so that we are granted occupancy by the City of Edmonds.

Received the attached letter from Aldrich and Associates Project Manager, George Bruce, regarding open items noted in
Mayes Testing Engineers inspection reports dated 12/23/16 and 12/28/16 regarding epoxy without special inspection, access
to approved construction drawings and concrete mix design approval. Per the attached letter, the work was performed per the
drawings submitted to the City of Edmonds and Aldrich and Associates enhanced standard practices. It is approved to issue
the Final Letter for proprietary anchor installation and reinforced concrete.
To the best of our knowledge, all items inspected today are in conformance with approved plans and specifications.
