Remarks:
As MTC arrived on site, all epoxy work had been completed, and anchors were completely installed. Contractor placed H I LTJ Anchor
Rod (threaded) HAS-R 3/8"A-5-1/8" bolts with 1-11LT1 Composite sleeves. The contractor infonned MTC that rods were installed per
detail N on page P6.00 of approved plans (HW Engineering). MTC informed that holes were drilled to 5/8" diameter. 3-1/2" deep. and
were brushed and blown clean prior to epoxy placement. The contractor showed MTC the brush. air punip, and empty epoxy tubes.
However, IVITC could not verify, these clairns, as work was completed upon arrival.
** MTC has spoken with the EOR and lie has re run the calculations, and wavirig the requirement for special inspection oil tile HY70
epoxy operation.
