This letter outlines the revisions and corrections made per the City's review comment memo dated June
14, 2017. Below is a list of the comment response documents addressing the comments and requests:
GENERAL
1) Submit ROW Ecum, Applic-111m, form
Response: See attached ROW permit application.
2) SubmitSide Scwcr Permil Api)licatiml form
Response: See attached Side Sewer permit application.
3) A Stormwater Covenant is required to be recorded against the property based on the
proposed stonilwater facilities for the project. Please complete the covenant, including
notarization and any required exhibits and submit to the city. The document must be
complete, but will not be recorded prior to permit issuance. Prior to construction final
location of facilities are to be verified (by field inspection and asbuilt) and then the covenant
will be recorded.

SITE DEVELOPMENT PLAN
1) The note"the sewer line must be cut and capped a minimum of 20' from the existing and/or
proposed foundation, whichever is closest to the sewer stub" was not found. Please provide -
Response: Note added sheet 1 as requested.
2) The sewer cleanout at the property line will need a 12" cast iron lamphole cover with 1/2"
hexbolts. Update the note accordingly
Response: Okay, cost iron lamphole cover revised as noted.
3) The sewer cleinout at the house does not need a 12" cast iron laillphole cover with 1/2"
hexbolts.
Response: Okay, cast iron lamphole cover removedfrom onsite (house) SSCOs.
4) The sewer seii-ice connection between the house and the cleatiout at the property line is
currently shown at 24%. Please reduce the sewer slope to 20% or add a note that the sewer
line -,vill need to be anchored. Anchors need to be designed by a civil engineer and detailed
oil the plans.
Response: Side sewer depth at house lowered 18" to reduce slope below 20% (18.3916).
