Comments:
1) Sheet: Dtl
Location: Storm water vault footings
2) MTC at this time inspected the reinforcing steel for the footings noted above. Basic reinforcing was #4 verts at 12" on -center, and a mat
of 4 #4 horizontals long span and #4's at 24" short span. Slab dowels were #4's at 12" oc. All reinforcing was found to be of correct grade ,
size, location, spacing, lap, clearance and secure, and was in -compliance with approved plans and specs.

Comments:
-MTC on site as requested to visually inspect reinforcing steel at the following location(s):
1.) MTC observed the completion of the reinforcing steel for the detention vault 10" cast in place concrete walls per detail A on sheet S3.
MLTC observed the wall reinforcing to be #5 reinforcing bars 10" on center each way.
-Reinforcing Steel Inspection:
L) MTC visually inspected the reinforcing steel at [Ile locations noted and for the exception of' the items noted below, found it to comply
with the approved project plans and spec ifi cations. MTC observed the reinforcing steel size, grade, type, and placement,
'MTC observed that theq4 vault lid hook dowel bars at 12" on center had not been installed. The contractor did not have the bars on site
Lit the time of the inspection. MTC will verify that the bars are installed prior to concrete placement. MTC will verily final clearances prior
and during concrete placement. No exceptions taken.
