Comments:
-MTC on site as requested to visually inspect reinforcing steel at the following location(s):
1.) MTC observed the completion of the reinforcing steel for the detention vault 10" cast in place concrete walls per detail A on sheet S3.
MTC observed the wall reinforcing to be #5 reinforcing bars 10" on center each way.
-Reinforcing Steel Inspection:
1.) MTC visually inspected the reinforcing steel at the locations noted and for the exception of the items noted below, found it to comply
with the approved project plans and specifications. MTC observed the reinforcing steel size, grade, type, and placement.
*MTC observed that the #4 vault lid hook dowel bars at 12" on center had not been installed. The contractor did not have the bars on site
at the time of the inspection. MTC will verify that the bars are installed prior to concrete placement. MTC will verify final clearances prior
and during concrete placement. No exceptions taken.

Structural Steel, Job site
Arrived on site to perform continuous special inspection of CJP welds for TSS beam brace.
Location : along the north side of great room on west side of structure.
Details : a photo copy of a single detai 1 19. No shop drawings were made available.
Welder was Dennis McVay. WABO certification number W20753, expires Oct I st, 2017.
Inspected fit up alignment and groove depth and angle. Inspected root pass for size, defects, fusion and consistency. Inspected subsequent
passes for size, defects, and continuity.
Filler metal was 7018 sticks stored on site in a dry box.
Due to complications during prep, only the brace on the north side was completed. The root pass for the brace on the south side was
completed.
All work conformed to the provided detail and AWS D I. I standards.
