Your land use application for the Dedonker Comprehensive Plan amendment located at 615 6
Avenue North is under review; however, review cannot continue until the following required
items are submitted:
1.Submit the enclosed declaration form certifying that the adjacent property owners list
provided with your application represents all properties within 300 feet of the subject
property. Note that this form must be signed and notarized.
2.Since the subject proposal is for a non-project action, please fill out and submit the
enclosed SEPA supplemental sheet for non-project actions.
Please submit a response to the above items at your earliest convenience. If you have any
