ONLINE APPLICANT ASSESSOR OTHER STATUS: ISSUED BLD20100314 CONDITIONS • The Building Official may impose conditions that address site work issues that includes: Limiting all excavation and drainage installation to the drier season (normally between May 1st and the end of September) and, securing City approval before entering a site with excavating, grading or clearing equipment to grade or to clear for any purposes, including the creation of access to the site. • Separate Permit Required For: Fire Sprinkler • 1) All recommendations of the geotechnical reports associated with the project must be followed. • 2) Work cannot begin until December 17, 2010. • Installer shall provide the manufacturer's installation, operating instructions, and a whole house ventilation systemoperation description. A label shall be affixed to the whole.house timer control that reads "Whole House Ventilation" (see operating instructions). • Special inspections have been called for on this project and are noted on the approved construction plans and building permit. It is the owner and/or contractors responsibility to assure that reports are provided to the City on a weekly basis. Be advised— if special inspection reports are not forthcoming, the Building Official may issue a "Stop Work" and no City inspections will be provided until such time as the reporting agency has complied and reports are reviewed and approved by the City. • Hose Bibbs (exterior faucets) are required to have a permanently affixed anti -siphon device installed. • In addition to the required pressure/relief valve, an approved listed expansion tank shall be installed on all hot water tanks. Per UPC 608. • Obtain Electrical Pen -nit from State Department of Labor & Industries. 425-290-1309 • Pursuant to UPC 605.2 a water service shutoffshall be installed on the water line as it enters the building. • Gas pipe test must be observed by City Building Inspector, affidavits shall not be accepted. • City approved plastic piping may be used in water service piping provided that where metal water service piping is used for electrical grounding purposes, replacement piping shall be of like materials (UPC 604.8). A state electrical permit and inspection is required if electrical grounding is altered, removed, improved, or added. Contact State Dept. of Labor & Industries Electrical Division at 425-290-1309. • Submit all special inspection reports to the City Building Inspector on a weekly basis. • As required by Ordinance #3632 the geotechnical engineer of record shall monitor this s ite during construction for compliance with the recommendations in the geotechnical report including: site excavation, shoring, soil support for foundation including piles, soil bearing capacity, subdrainage installation, soil compactions, and other geotechnical aspects of the construction. Specific recommendations contained in the approved geotechnical report must be implemented by the owner. The geotechnical engineer shall make written, dated reports on the progress of the construction and submit the report to the Building Official on a weekly basis until all site grading; drainage, foundation and associated ground work is complete. Omissions or deviations from the approved geotechinical report and/or approved plans or specifications shall be highlighted and irrunediately submitted in a seperate letter to the City for review. The City shall be advised in writing of work stoppages of more than one week. In addition to the geotechnical monitoring, special inspections based on.the provisions of IBC Section 1704 are required when specified on the approved plans. Other special inspections may also be required by the geotechnical engineer, architect, or structural engineer of record (refer to approved plan set). At the completion of final site grading and all permitted structures, a final geotechnical report, prepared by the geotechnical engineer shall be submitted to the Building Official. This report shall contain a statement that, based upon his/her professional opinion, site observations, and testing during the monitoring ofthe construction, the completed development substantially complies with the recommendations in the geotechnical report and with all geotechnical related permit requirements. Any deviations'or omissions in the report, plans, or specifications that occured during construction shall be addressed separately. Occupancy, final approval, or release ofthe bond for the project shall not be granted until the report has been reviewed and accepted by the Building Official. • Final approval on a project or final occupancy approval must be granted by the Building Official prior to use or occupancy of the building or structure. Check the job card for all required City inspections including final project approval and final occupancy inspections. • Provide combustion air per IMC Chapter 7 for conunercial and multi -family residential installations, and IRC Chapter 17 for one and two-family dwellings. • Any request for alternate design, modification, variance or other administrative deviation (hereinafter "variance") from adopted codes, ordinances or policies must be specifically requested in writing and be called out and identified. Processing fees for such request shall be established by Council and shall be paid upon submittal and are non-refundable. • Approval of any plat or plan containing provisions which do not comply with city code and for which'a variance has not been specifically identified, requested and considered by the appropriate city official in accordance with the appropriate provision of city code or state law does not approve any items not to code specification. • Sound/Noise originating from temporary construction sites as a result of construction activity are exempt from the noise limits of ECC Chapter 5.30 only during the hours of 7:00am to 6:00pm on weekdays and 10:00am and 6:00pm on Saturdays, excluding Sundays and Federal Holidays. At all other times the noise originating from construction sites/activities must comply with the noise limits of Chapter 5.30, unless a variance has been granted pursuant to ECC 5.30.120., • Applicant, on behalf of his or her spouse, heirs, assigns, and successors in interests, agrees to indemnify defend and hold harmless the City of Edmonds, Washington, its officials, employees, and agents from any and all claims ford amages of whatever nature, arising directly or indirectly from the issuance forth is permit. The landscaping installed on the site is significantly different from the originally approved landscape plan prepared by The Jay Group. Mr. Todd provided as built plans and the summary report by Nelson Geotechnical dated Feb 22nd. These documents were peer reviewed by the City's consultant Landau Associates. A copy of their review is attached. In the conclusion of the Landau review, they recommend that the original landscape design architect review the as built conditions, since the original geotechnical approvals had been based upon that design. I agree with this determination as being consistent with the original permit approval and the Edmonds earth subsidence and landslide hazard provision in ECDC .19.10. In my Feb 24th letter I stated that "If items requiring correction or modification are necessitated by the review these will need to be completed prior to the final certificate of occupancy." At this time I ask that you have the landscape architect provide his review and comments on the changes to his original design. Please provide a copy of the architect's comments and any recommendations. BLD20100314. This residence is located at 7510 - 162nd Street West in Edmonds. In the NGA final geotechnical inspection summary letter, NGA acknowledges differences in the as -built landscaping plan from the original landscaping plan included in the approved building permit submittals. Our review and comments are primarily concerned with this deviation from the landscaping plan and its potential impact to slope stability of the project site and surrounding properties. This geotechnical peer review was accomplished in accordance with Task Order No. 12-01 of Landau Associates' On -Call Geotechnical Engineering Services Agreement with the City of Edmonds (City). Documents we reviewed in preparation of this technical memorandum include the NGA summary letter, a brief letter of explanation on landscape changes (untitled, presumably provided by the owner), revegetation plans by The Jay Group (included in the July 29, 2010 permit re -submittal), and a landscape as -built survey of the project site (source unknown). These documents were provided to us by the City. We also reviewed previous documents in our files for this project site and conducted a brief visit to view site conditions from the City ROWs along 162nd Street and 75h Place West. Comments on NGA Special Inspection Summary Letter On page 3 of the NGA Special Inspections Summary Letter, the following statement is made: Permanent erosion control measures were observed on February 16, 2012 at the site and generally included mulch, grass, bark, and low vegetation. We did not observe any exposed areas or areas that we would consider at risk of significant soil erosion. We understand that the implemented vegetation plan varied somewhat from the proposed vegetation plan. 77te original 130 2nd Avenue South 9. It is our opinion that the long- term erosion control measures that have been implemented and the vegetation planted at the site are adequate. The large trees that were called for in the original revegetation plan could pose a risk to the slope and the residence as they mature. The current plan is more appropriate to the site conditions than the previous plans. We recommend that the sloping areas within the site that have received vegetation be monitored throughout the wet season and repairs be implemented immediately should any signs of erosion or plant die -off be observed. During our brief visit to the project site on February 27, 2012, we observed similar conditions as those reported by NGA and as shown on the landscape as -built survey. We concur that short-term erosion control is provided by the landscaping as implemented. However, we find that the as -built condition constitutes a significant deviation from the approved landscape plan included in the permit submittal documents. Furthermore, it is not apparent that the landscape architect has been made aware of the modifications executed during construction. Based upon the approved landscape plan, nine significant trees were to be removed during construction and replaced by 27 deciduous, 3 -inch diameter saplings following construction, plus groundcover consisting of kinnikinnick and other low vegetation and bushes. Based upon a calculation methodology presented on Sheet L-2 of the original revegetation plan, the landscape architect specifies that trees should be replaced at a rate of three replacements per one removed significant tree. For the seven significant trees that were actually removed during construction, this methodology would require 21 replacement trees. The as -built condition retains two significant trees and provides two replacement trees, but otherwise substitutes all other replacement trees with additional kinnikinnick, bushes and shrubs. The actual grading accomplished on the site (and documented in the as -built drawing) is slightly different than that shown on the approved plans, with some small retaining walls having been eliminated or relocated. These modifications appear to be minor and in a general configuration consistent with the approved permit submittals; however, these changes are not acknowledged or evaluated in the NGA letter. Applicable Codes Several Edmonds Community Development Code (ECDC) provisions potentially pertain to the tree removal and replacement issues. ECDC 18.45 — Land Clearing and Tree Cutting Code calls of preservation of trees wherever possible, particularly within designated environmentally sensitive areas and sloping sites. The property is located within the designated North Edmonds Earth Subsidence and Landslide Hazard Area and is considered to be both an environmentally sensitive area and to satisfy the 3/12/2012 P:\074\164\FiteRoom\R\Lan&capeModificaaon&P=RwimTM.doe 2 Landau Associates slope criteria in the code. An application for a permit in accordance with ECDC 18.45 was required and the application must address "The manner in which the cleared areas on the property will be reclaimed with vegetation and the timetable for replanting" (ECDC 18.45.045.A.8). The landscape plan prepared by the landscape architect for the project (and approved as a part of the City permit process) addresses the replacement of removed trees. We are not aware of the source of the 3:1 replacement ratio called for by'the landscape architect, although a ratio of 2:1 is mentioned as a rate of replacement for hazard trees in Environmentally Critical Areas (Edmonds City Code, Chapter 23.40.220.7b.iv). A ratio of 3:1 is specified for unlawful clearing in Chapter 18.45.075.A. Neither of these chapters appear to be directly applicable—the clearing activity was lawful and part of an approved permit, and a "hazard tree" condition was not present. However, Chapter 18.45.045.D does provide that "Approved plans shall not be amended without written authorization from the city. The permit may be revoked or suspended by the city upon discovery that incorrect information was supplied violation of the provisions of this chapter." One goal of revegetation planting is typically to reduce the potential for erosion and shallow sliding of the site soils. Numerous Puget Sound -area slope bluff management publications and a majority of geotechnical engineers and engineering geologists acknowledge the role that well-managed trees play in providing erosion prevention/control and shallow stabilization of soils at sloped sites, especially as part of a multilayered vegetation structure also including lower -growing shrubs and groundcover. These functions are provided through several mechanisms, including reductions in soil moisture from deep root uptake and tree respiration, reduction of raindrop impact. from the canopy and from leaf litter, and the mechanical binding of soil within the tree's root zone. Tree roots typically extend much deeper and provide greater slope reinforcement than other vegetation. Deep root uptake and.tree respiration stopped after trees were cut at the project site. With time, the mechanical binding provided by the dead roots will degrade as they rot. This may take several years to more than a decade to occur; in the meantime, new trees planted now would grow to the extent that they would provide a slope stabilization benefit in the future. ECDC 23.80 — Geologically Hazardous Areas addresses development in landslide hazard areas and section 23.80.070.4 states "Unless otherwise provided or as part of an approved alteration, removal of vegetation from an erosion or landslide hazard area or related buffer shall be prohibited, " Section 23.80.070.3 states that "Development within an erosion or landslide hazard area and/or buffer shall be designed to meet the following basic requirements unless it can be demonstrated that an alternative design that deviates from one or more of these standards provides greater long-term slope stability while meeting all other provisions of this title. " 3/122012 P:\074\164\FileRoom\R\LandtcapeModificaaon&Peer RevimTM.doc 3 Landau Associates Since development of the property required removal of a number of trees within or adjacent to the proposed footprint of the residence, an effective means of addressing this issue was to develop a planting plan that included a significant number of replacement trees such that the vegetative function of the trees previously on the property could be restored. This alternate design, when properly implemented, could be argued to provide greater long-term slope stability than conditions prior to construction. In our opinion, the landscaping plan that was submitted as a part of the permit application for development of the property and approved by the City effectively mitigated the removal of trees and other vegetation on the property that occurred during construction. CONCLUSION The actual planting and tree replacement on the property differs substantially from the approved landscape plan. In our opinion, the limited number of replacement trees actually planted does not meet the intent of the approved permit to provide a significant number of replacement trees that would provide greater long-term slope stability benefits as compared to previous conditions and that would effectively mitigate the removal of trees from the designated environmentally sensitive area. We recommend that the landscape architect review the as -built condition to determine appropriate modifications to the current as - built conditions in order to achieve the original design objective and to satisfy the intent of the ECDC and the approved development plans. The project generally consisted of constructing a two-story residence. This residence is part of a two -lot short plat and. is considered to be the northern residence of the two lots. We previously issued a geotechnical engineering evaluation for this project dated February 12, 2010. l The City of Edmonds required geotechnical inspections for earthwork construction at this project site as indicated in an undated letter. The geotechnical inspections included soil bearing verification, subsurface drainage installation, site monitoring during wet weather construction, temporary and permanent erosion control inspections, and foundation and site drain installation. We were retained by you to provide geotechnical engineering inspection and consultation services for the project. We monitored geotechnical -related construction activities at the project site on a part-time basis on and off from May 2, 2011 to February 16, 2012. Our observations, opinions, and recommendations regarding earthwork activities for the northern residence at this project site were included in Field Reports 9 through 15, which have been submitted to you. Special Inspections Summary Letter Gold Residence 7510 -162nd Street West Edmonds, Washington February 22, 2012 NGA File No. 8196B 11 Page 2 We evaluated the residence and garage foundation subgrade by observing the material exposed in the excavation and by probing with a '/z -inch diameter probe rod under moderate pressure. The soils exposed at the foundation subgrade consisted of native blue -gray silt with varying amounts of sand that we interpreted as native glacial soils. In general, the footing subgrade probed approximately 6 to '18 inches and was interpreted to be very soft to stiff. The foundation excavations were over -excavated a minimum of two feet below the proposed foundation subgrade and backfilled with 2- to 4 -inch rack spalls. The rock spalls were tamped in place with the trackhoe bucket and a bulldozer. In our opinion, the foundation subgrade was suitably prepared in accordance with the plans and our recommendations and should adequately support the planned loads. We observed the footing drains around the residence consisted of 4 -inch PVC perforated pipes embedded in washed rock and covered in filter fabric, extending around the perimeter of the structure per the project plans. We did not observe the installation of the drains behind the block retaining walls. The contractor informed us that the drains behind the block walls consisted of 6 -inch perforated rigid PVC pipe embedded in washed rock and covered in filter fabric. We observed that there were 4 -inch cleanouts located at the ends of the walls. The contractor informed us that the wall drains were tightlined to the catch basins as shown on the plans. In our opinion, based on our observations and the information that the contractor provided, the footing and wall drains appear to have been installed in accordance with our recommendations and the plans. In Field Report 13, we evaluated the surface of the slope above the northern residence to the east•and south with a V2 -inch diameter steel probe. Probing yielded results of up to three feet indicating very loose conditions. We also observed some tension cracks within the surface of the soils within the slope. We recommended at that time, that all of the loose material be removed from the sloping areas and be replaced with 2- to 4 -inch rock spalls. We also recommended that the rock spalls be placed on level benches and tamped in with the excavator bucket. We did.not observe any of these recommended repairs. The contractor informed us that the repairs, as described in our field report, were performed in accordance with our recommendations. We understand that the soft soils on the slope were removed and rock spalls were placed on the slope. Bark -and low-lying vegetation was then placed on the slope. We again probed the surface of the slope with results of 0.5 to 1.5 feet of penetration. It is our opinion that there is still a potential for minor sloughing events to occur on the slope above the residence. Such events could affect NELSON GEOTECHNICAL ASSOCIATES, INC. We recommend that this slope be monitored for signs of sloughing through the wet season and repairs be implemented immediately.. Erosion control measures were utilized at the project site. Temporary erosion control measures generally included a silt fence and plastic sheeting over the slopes of the excavation. We did not observe stormwater flowing uncontrolled off site during our visits. Based on our observations while on site, materials were not tracked into public right-of-way or onto neighboring properties during earthwork construction. It is our opinion that temporary erosion control measures both during dry season and the wet season were adequate. Permanent erosion control measures were observed on February 16, 2012 at the site and generally included mulch, grass, bark, and low vegetation. We did not observe any exposed areas or areas that we would consider at risk of significant soil erosion. We understand that the implemented vegetation plan varied somewhat from the proposed vegetation plan. The original plan called for large deciduous trees to be planted on the slope below the residence, while the current plan utilized mostly low-lying, deep-rooted vegetation. It is our opinion that the long-term erosion control measures that have been implemented and the vegetation planted at the site are adequate. The large trees that were called for in the original re - vegetation plan could pose risk to the slope and the residence as they mature. The current plan is more appropriate to the site conditions than the previous plans. We recommend that the sloping areas within the site that have received vegetation be monitored throughout the wet season and repairs be implemented immediately should any signs of erosion or plant die off be observed. We also observed structural fill compaction for a portion of the storm drain located within the roadway, The backfill consisted of recycled asphalt compacted in thin lifts. We evaluated compaction using a nuclear densometer and a %z -inch diameter steel probe rod. Probing generally yielded penetrations of one to two inches, indicating medium dense to dense conditions which we interpreted as well compacted. In- place density tests results indicated 95 percent relative compaction per ASTM D-1557, modified proctor. APPROVAL OF SPECIAL INSPECTORS: Each special inspector shag be approved by the Budding ` official prior to performing any duties or inspections. The company performing inspections must be WABO certified as well as the individual completing inspections. Special inspectors shall display identification when performing special inspections on the project site. OBSERVE 'WORK The special inspector shall observe the site work for conformance with the approved (stamped) plans and specifications and applicable workmanship provisions of the IBC Architect or Engineer reviewed shop drawings may be used only as an aid to inspection. Special inspections are. to be performed on a continuous basis --meaning that the special inspector is on site at all times observing the work requiring special inspection. Periodic inspections are permitted only as specifically noted in the approved project plans in accordance with IBC Chapter 17, and as approved by the Building Official. Continuous Inspection means the same inspector is on site day to day to observe the work requiring special inspection - Periodic Inspection means some inspections may be performed on a periodic basis to satisfy the requirements of continuous inspection, provided these periodic inspections are performed as outlined in the project plans and specifications, and approved by the Building Official. 2. REPORT NONCONF.ORNUNG ITEMS The special inspector shall bring non -conforming items to the iomediate attention of the ` contractor and note all such items in the daily field report Any item not resolved in a timely :r manner shall be immediate cause of the special inspector to notify the Building Official, by lo telephone at (425-771-0220) or in person at City Hall, of the plan deviation, error, change or omission. It shall also be the duty of the special inspector to promptly notify the engineer or architect of record. 3. DAILY REPORTS DEPT ON SITE WITH CONTRACTOR Each special inspector shaft complete and sign a daily report for each day's inspections. The daily reports shall remail at the jobsite with the contractor for review by the City Building Inspector. 4. WEEKLY REPORTS TO QTY The special inspector or inspection agency shall furnish the City with weekly reports of tests and inspections. The project engineer or architect; and others as designated shall also be copied on reports. Weekly reports must include the following. Description and date of daily inspections and tests made with applicable locations • List of all non -conforming items and locations • Report on status of non -conforming items (how each was resolved or unresolved) Field changes authorized by the Architect, Engineer or City Inspector if not included in non-conformance items. FINAL CONSTRUCTION REPORT The special inspector or inspection agency shall submit a final signed report or Certificate of Compliance to the City stating that all items requiring special Inspection and testing were fulflled and reported. And, to the best of his/her lmowledge the project is in conformance with the approved plans and specifications, approved change orders and the applicable workmanship provisions of the IBC. Items not in conformance, unresolved items or any discrepandes in Inspection coverage CU., missed inspections, periodic inspection when continuous inspections were required, etc.) shall be specifically addressed in this report. B. Contractor Resuousibatees 1. NOTIFY THE SPECIAL INSPECTOR OF ALL REQUIRED SPECIAL INSPECTIONS It is the duty of the contractor to notify the special inspector when work is ready for special inspection. Note, the items listed on the attached Special Inspection Information Form and as noted on the approved plans and specifications are required to have special Inspections. Adequate notice shall be provided by the contractor so that the special inspector has time to become familiar with the project. All work requiring special inspection must be approved prior to concealing or covering said work. 2. Ip'ROME ACCESS TO APPROVED PLANS The aintractor is responsible for providing the special inspector access to approved plans at the jobsft 3. RETAIN SPECIAL INSPECTION RECORDS. The contractor is responsible to retain at the jobsite all special inspection records submitted by the special inspector. Upon arrival, we met with Mike Todd. We observed that the silt fence had been installed along the western portion. of the site just to the east of the top of slope. The silt fence appeared to be trenched below the surface and backfilled with pea gravel on the upslope side of the silt fence. Upon arrival, we met with Mike Todd. We observed that the garage foundation for the southern residence had been excavated down to the proposed footing subgrade. The soil exposed in the subgrade consisted of blue gray silt with clay and fine sand deposits that we interpreted as native landslide grade by probing with a %Z -inch diameter probe rod under moderate deposits. We evaluated the sub pressure. The subgrade was penetrated approximately 6 to 24 inches. We interpreted it to be very soft to stiff. We recommended that they still overexcavate the foundation footing lines by two feet and backfill with 2- to 4 -inch rock spalls according to the approved plans. Mike agreed We also observed that during the excavation of the southern residence, a concrete slab and foundation re encountered near the footingsubgrade elevation along the western footing line. The senior were . engineer will return to observe the concrete and provide recommendations for repair. We also observed tall steep cuts on the order of approximately 4 to 8 feet tall along the southern and eastern sides of the residence excavation. Some sloughing of these slopes were observed while on site. ,. We recommended to Mike that these slopes should be sloped back to safe inclinations and covered with plastic sheeting to provide stable excavation cuts and safe worker access. We observed that material from the excavation was being stockpiled on site away from the slopes and being trucked off site. No material appeared to be tracked on to the roadway from the site. We have been providing monitoring services on a part-time basis only. Our findings and conclusions are based on construction activities observed while on site. It is our opinion that the subgrade, prepared as recommended above, should provide adequate support for the planned foundation loads. After the concrete has been removed and native soils exposed, we recommend that the excavation be backfilled with 4- to 8 -inch rock spalls to the top of the existing concrete and capped with approximately two feet of 2- to 4 -inch rock spalls. Mike agreed. We observed that material from the excavation was being stockpiled on site away from the slopes and being trucked off site. We observed that one sheet was installed on the eastern side of the garage cut within the southeast corner.. A one foot deep by two foot wide trench was excavated along• the toe of the cuts and the sheet was placed within this trench and rested against the slope face. Some sloughing and groundwater seepage was observed along the eastern and -south eastern sides of the cuts. PM Site Visit We observed that they had generally completed the installation of the metal plates along the excavation slopes. We observed that 7 -foot lengths of 2 -in pin piles were driven into the trench along the base of the slopes and along the top of the slopes. Approximately one foot of the pipe was left above the surface to provide support for the sheets. The sheets were placed against the piles and laid back onto the slope covering a majority of the exposed cut face. Chains were attached to the top of the plates and around the piles- located at the top of the slope. The trenches along the base of the plates were then backfilled with 2- to 4 -inch rock spalls. 2- to 4 -inch rock spalls were. then piled within the. southeastern corner of the excavation. We recommended that the remaining southern excavation cut for the residence be similarly supported with metal plates. We also recommended that the remaining exposed upper portions of the slope be covered with plastic sheeting. Mike agreed. We observed that were overexcavating the lower western portion of the southern residence approximately two feet below the planned foundation subgrade. The excavation exposed blue gray silt with clay and fine sand deposits that we interpreted as native landslide deposits. We evaluated the. subgrade by probing with a %2 -inch diameter probe rod under moderate pressure. The subgrade was penetrated approximately 6 to 18 inches. We interpreted it to be very soft to stiff. The overexcavation was beginning to be backfilled with approximately 2 feet of 2- to 4 -inch rock'spalls. The rock spalls were being tamped in place with a trackhoe bucket and track rolled in.place with.a bulldozer. We observed that the area where the old concrete slab was encountered was backfilled with 2 feet of 4 - to 8 -inch. rock spalls. The contractor informed us that they removed the concrete slab. They also informed. us that the foundation for the new residence is located outside of the encountered concrete foundation. We observed that they had generally completed the installation of the metal plates: along the southwestern excavation slopes. We observed that 7 -foot lengths of 2 -in pin piles were driven into the trench along the base of the slopes and along the top of the slopes. Approximately one foot of the pipe was left above the surface to provide support for the sheets. The sheets were placed against the piles and laid back onto the slope covering a majority of the exposed cut face. Chains were attached to the top of the plates and around the piles located at the top of the slope. The trenches along the base of the plates and behind the plates were then backfilled with 2- to 4 -inch rock spalls. The exposed portions of the slopes were covered with plastic sheeting. The excavation slopes appeared to be marginally stable at the time of our visit. However, we did observe a tension crack at the ground surface along the neighboring driveway to the south of the excavation. 'We informed Mike Todd and Vern about the crack and recommended that they closely monitor the stability of the cut slopes and move the pin piles anchoring the metal plates to the driveway area behind the tension crack. CIN OF EDMONDS We visited the site today at the request of Mike Todd with Todd Homes to observe excavation for the northern residence and overall site conditions. We met with Mike at the Gold Residence project located in 7510 162nd Street SW in Edmonds, WA. We observed the contractor was in the process of excavating the building pad for the northern residence. Tall cuts along the southern and eastern sides of the excavation have been created with inclinations of .over 1H:1 V. The cuts were up to 20 feet tall or taller. The material exposed within the lower portion of the cut consisted of gray medium stiff to stiff silt. Above the gray silt, we observed less competent light brown to brown silt with sand and sand lenses. We observed steady seepage at the bottom of the less competent material above the gray silt. We also understand that a portion of this material slid down the previous night, and the sloughed material had been cleaned up by the time we arrived at the site. We recommended to Mike that steel plates or some other type of temporary shoring be implemented along the tall cuts to maintain safe worker access. He indicated to us that he would rather wait until the excavation is complete and he will decide at that time how to approach this issue. We informed him that, in any case, safe worker access must be maintained at all times. He agreed. We also observed the condition along the southern side of the excavation for the southern residence. We observed that the tension cracks we previously noted on the neighboring property to the south have ( widened to several inches in some locations, and that the entire cut bank along that side is slowly drooping and moving towards the recently poured foundation. The metal stakes holding the tops of the plates are within the moving wedge of soil and thus are not providing any support for the plates. We informed Mike that this movement will likely continue, especially. during wet weather and will . eventually lead to a major failure if conditions are not addressed. We also recommended that for the very least more stakes be placed well beyond the failing soil mass and the tops of the plates be chained to the new stakes. We recommended to Mike that the safety of workers operating in that area be his number one priority and that the conditions be corrected as to ensure safe worker access at all times. He agreed. We also observed that sections of the recently poured foundations along ,the southern portion of the south residence has been undermined by on-going sloughing and seepage in that area. We observed a gap several inches in width extending underneath the footing. We recommended to Mike that concrete or grout be pumped underneath the footing to fill all noted gaps. He agreed. We evaluated the subgrade by probing with a Y2-inchdiameter probe rod under moderate pressure. The subgrade was penetrated approximately 6 to 18 inches. We interpreted it to be very soft to stiff. The contractor informed us that these areas were still approximately 2 to 4 ft higher than the proposed bottom of rock spall trenches. He also informed us that they would be continuing to excavate the foundation area down to allow placement of the rock spalls. We also observed tall steep cuts on the order of approximately 15 to 20 feet tall along the southern and eastern sides of the northern residence excavation. These slopes were generally sloped back away from the residence excavation at an approximate gradient of 0.75:Horizontal to 1:Vertical (0.75H:1V). Some sloughing and minor groundwater seepage was observed within the slope cuts during our site visit. We informed Mike Todd that these slopes were very tall and steep and could experience instability if not addressed. We recommended that these cuts be sloped back to a safe inclination or benched to provide ( safe worker access. We also recommended that the slope cuts be covered with plastic. Mike informed us that he would monitor the slope cuts overnight and address stability measures tomorrow if needed. We observed that the southern residence foundation has been poured. We also observed indication of further movement at the ground surface along the top of the slope cut within the neighboring southern property. The senior engineer with NGA will visit .the site tomorrow to provide additional recommendations for this area. We observed that material from the excavation was being stockpiled on site away from the slopes and being trucked off site. No material appeared to be tracked on to the roadway from the site. Upon arrival, we observed that for the majority of the sloping potions of the site had been covered with plastic sheeting and were anchored down with sand bangs. However, we did observe that some of the plastic sheeting had been blown away likely by the wind which exposed some bare soils within the sloping areas of the site. We recommend that the plastic sheeting in these areas be placed back to cover the exposed portions of the slope and additional sandbags be placed to minimize the potential of the sheeting moving again. Upon arrival, we met with Mike Todd and Patrick Lawler with the City of Edmonds. We understand that the wet season grading period began October 1" for the City of Edmonds and the City has requested recommendations regarding site erosion control measures. We walked the site and observed that the slopes along the downhill side of the northern and southern residences had bee covered with plastic and a silt fence was also located along the downhill side of the plastic. For the most part the remaining portions of the site remained uncovered and bare soil was exposed. We recommended that all of the sloping portions of the northern and southern lots be covered with either plastic sheeting or erosion control material such as jute netting of a synthetic straw mat. These materials should also be adequately anchored to the surface of the slopes. We were informed by Mike that the eastern and southern slopes around the northern residence will begin to be landscaped tomorrow. He also informed us that he intends to place landscaping plants and bark along the surface of the slope. We evaluated the surface of the slope with a 0.5 -inch diameter steel probe. Some areas of the slope above the residence probed .approximately 4 to 5 -inches indicating suitable conditions. However, some areas probed up to 3 feet indicating very loose conditions. We understand that these areas were likely filled in when the block walls were constructed. We also observed tension cracks within the surface of the slope. We recommend that all of the loose material be removed from the sloping areas and be replaced with 2- to 4 -inch rock spalls. The rock spalls should be placed on level benches and tamped into the slope with an excavator bucket. The surface of the slope around the northern residence should then be covered with erosion control matting and vegetation. We have been providing monitoring services on a part-time basis only. Our findings and conclusions are based on construction activities observed while on site. We recommend that we return to'the site after erosion control measures have been completed and slope repairs begin. We were informed and observed that the entire trench backfill consisted of recycled asphalt and compacted using a trackhoe with hoepack attachment in 1 -foot lifts. We evaluated compaction using a nuclear densometer and a Y2 -inch diameter steel probe rod. Probing generally yielded penetrations of 1 to 2 inches, indicating medium dense to dense conditions which we :1 interpreted as well compacted. In-place density tests results indicated 95 percent relative compaction per - ASTM D-1557, modified proctor. Test results and locations are provided in the attached pages. We have been providing monitoring services on a part-time basis only. Our findings and conclusions are based on construction activities observed while on site. Based on our part-time observations in the field, communications with the contractor, our understanding of this site and project plans, and provided that the contractor used consistent materials and methods while we were present and not present at the site, it is our opinion that.the backfill is being compacted to a firm condition and should support the planned loads.
