To continue review of your proposal, the following information must be submitted; 1) A Street Use permit application (Section 3 A); 2) A Right of Way permit application per agreement (Section 3 A), including a pedestrian control plan for the area below the installation area; 3) A Certificate of Insurance (Section 18); 4) Fees: Street Use Permit $85 + 15 city surcharge = $ 100 (Section 3 B) Right of Way Permit $240 + city surcharge = $ 260 (Section 3 B) Franchise Annual Fee $2,000 (Section 15) Please let me know if you have any questions.
