Purpose of Checklist:
The State Environmental Policy Act (SEPA), chapter 43.21C RCW, requires all governmental agencies to consider
the environmental impacts of a proposal before making decisions. An environmental impact statement (EIS) must be
prepared for all proposals with probable significant adverse impacts on the quality of the environment. The purpose
of this checklist is to provide information to help you and the agency identify impacts from your proposal (and to
reduce or avoid impacts from the proposal, if it can be done) and to help the agency decide whether an EIS is
required.
Instructions for Applicants:
This enviromnental checklist asks you to describe some basic information about your proposal. Governmental
agencies use this checklist to determine whether the environmental impacts of your proposal are significant,
requiring preparation of an EIS. Answer the questions briefly, with the most precise information known, or give the
best description you can.
You must answer each question accurately and carefully, to the best of your knowledge. In most cases, you should
be able to answer the questions from your own observations or project plans without the need to hire experts. If you
really do not know the answer, or if a question does not apply to your proposal, write "do not ]snow" or does not
apply". Complete answers to the questions now may avoid unnecessary delays later.
Some questions ask about governmental regulations, such as zoning, shoreline, and landmark designations. Answer
these questions if you can. If you have problems, the governmental agencies can assist you.
The checklist questions apply to all parts of your proposal, even if you plan to do them over a period of time or on
different parcels of land. Attach any additional information that will help describe your proposal or its
environmental effects. The agency to which you submit this checklist may ask you to explain your answers or
provide additional information reasonably related to determining if there may be significant adverse impact.

Complete this checklist for nonproject proposals, even though questions may be answered "does not apply." IN
ADDITION, complete the SUPPLEMENTAL SHEET FOR NONPROJECT ACTIONS (part D).

The site is an existing 12 -unit residential condominium. There is an existing wireless
communication facility on the property, consisting of rooftop antennas and equipment cabinets
located in an outdoor, fenced compound. AT&T proposed to add three (3) new panel antennas to the
roof of the building, and one new cabinet within their existing equipment compound.

This facility is located at 1414 91h Ave N, Edmonds, Washington. Parcel is located in Section 13,
Township 27, Range 3SE W.M. Vicinity Map and Legal Description are included on the cover sheet
(T-1) of the plan set submitted with this application.
B. ENVIRONMENTAL ELEMENTS
1. Earth
a. General description of the site (underline):
Flat, Rollin, Hilly, Steep Slopes, Mountainous. (Note: the site is 100% developed)
b. What is the steepest slope on the site (approximate percent slope)?
The site is 100% developed. There is an engineered slope and rockery at the NE corner of the site
that is approximately 100%.
c. What general types of soils are found on the site (for example clay,
sand, gravel, peat, muck)? Specify the classification of agricultural
soils and note any prime farmland.
No soil testing has been performed on this site. The site is improved only
by the existing building, parking lot and landscaped grounds. No changes
in these structures are proposed, so no geotechnical report will be repaired.
d. Are there any surface indications or a history of unstable soils in
the immediate vicinity? If so, describe.
No.
e. Describe the purpose, type, and approximate quantities of any
filling or grading proposed. Indicate the source of the fill.
No grading or fill is proposed. All structures presently exist.
f. Could erosion occur as a result of clearing, construction, or use?
No.
g. About what percent of the site will be covered with impervious
surfaces after project construction (for example buildings or
asphalt)?
Approximately 75% of the site is currently impervious surface (building,
parking, driveway, pool, deck and walkways).
h. Describe the proposed measures to reduce or control erosion, or
other impacts to the earth, if any.
No clearing or grading is proposed. No additional measures are proposed.
2. Air
a. What types of emissions to the air would result from the proposal
(e.g. dust, automobile, odors, industrial, wood smoke) during

There will be minor emissions from motor vehicles associated with
construction. There are no emissions associated with the project operation.

3. Water

additional equipment cabinet will be installed within the existing

Deciduous trees: There are a few small deciduous trees on the site
near the pool.
Evergreen trees: There are a few small evergreen trees on the site
near the pool and on the NE corner of the site.
Shrubs: There are hedge rows along the southern and eastern edges of
the property to screen the outdoor areas from the traffic and existing
commercial uses to the east.
Grass: There are green lawn areas on the south and east sides of the
property.

screened from the street and abutting properties to the east and north by
shrubbery and small trees.

6. Energy and Natural Resources

be used to meet the completed project's energy needs? Describe
whether it will be used for heating, manufacturing, etc.
Electric power, required for the radio equipment, is already present on the
site, and sufficient to operate the additional equipment. No other energy is
required.
b. Would the project affect the potential use of solar energy by
adjacent properties? If so, explain.
No. The replacement antennas would have no shadow effect of the tower
structure.

7. Environmental Health
a. Are there any environmental health hazards, including exposure
to toxic chemicals, risk of fire and explosion, spills, or hazardous
waste that could occur as a result of this proposal? If so, describe.
No such hazards have been noted. All RF emissions will be within the
parameters established and governed by the Federal Communications
Commission.

None are necessary.

environmental health hazards.
Operation of the wireless facility is constantly monitored by AT&T
Mobility's Network Operation Centers. Any operational problems are
responded to on a priority basis.
b. Noise
1. What types of noise exist in the area which may affect your
project (for example: traffic, equipment operation, other)?
None. Noise will not affect this project.
2. What types and levels of noise would be created by or
associated with the project on a short-term or long-term basis
(for example: traffic, construction, operation, other)?
Noise levels on site will increase intermittently for one or two weeks
during the construction phase of the project. Construction will occur
during allowed time periods.
When completed, the sound levels will meet the 45 dBA limit at the
neighboring property line. The additional equipment cabinet will be
installed inside of the existing equipment compound. A noise report
prepared by a qualified third party will be submitted along with this
project permit application.
3. Describe proposed measures to reduce or control noise
impacts, if any.
The distance from any sensitive responders is sufficient to mitigate the
marginal noise produced by the equipment cabinets.

The Edmonds Comprehensive Plan and Zoning Code allow wireless
communication facilities with restrictions. Current code allows rooftop
wireless facilities that do not exceed 9' above the roof of an existing
structure that exceeds the current height limit of the zone. The existing
and proposed antennas will be flush -mounted to the existing chimneys, or
screened by an RF -transparent panel.
9. Housing
a. Approximately how many units would be provided, if any?

The screening panels will extend approximately 6'-4" above the existing
roof, or 46'-4" above grand level. The RF -transparent panels will be
molded and painted to resemble a chimney.

The new antennas to be attached to the existing chimneys will be painted
to match the color of the brick. The RF -transparent panels will be molded
