An environmental impact statement (EIS) must be prepared for all proposals with probable significant adverse impacts on the quality of the environment. The purpose of this checklist is to provide information to help you and the agency identify impacts from your proposal (and to reduce or avoid impacts from the proposal, if it can be done) and to help the agency decide whether an EIS is required. Instructions for Applicants: This enviromnental checklist asks you to describe some basic information about your proposal. Governmental agencies use this checklist to determine whether the environmental impacts of your proposal are significant, requiring preparation of an EIS. Answer the questions briefly, with the most precise information known, or give the best description you can. You must answer each question accurately and carefully, to the best of your knowledge. In most cases, you should be able to answer the questions from your own observations or project plans without the need to hire experts. If you really do not know the answer, or if a question does not apply to your proposal, write "do not ]snow" or does not apply". Complete answers to the questions now may avoid unnecessary delays later. Some questions ask about governmental regulations, such as zoning, shoreline, and landmark designations. Answer these questions if you can. If you have problems, the governmental agencies can assist you. The checklist questions apply to all parts of your proposal, even if you plan to do them over a period of time or on different parcels of land. Attach any additional information that will help describe your proposal or its environmental effects. The agency to which you submit this checklist may ask you to explain your answers or provide additional information reasonably related to determining if there may be significant adverse impact. Use of checklist for nonproject proposals: Complete this checklist for nonproject proposals, even though questions may be answered "does not apply." IN ADDITION, complete the SUPPLEMENTAL SHEET FOR NONPROJECT ACTIONS (part D). Brief, complete description of the proposal, including the proposed uses and the size of the project and site: The site is an existing 12 -unit residential condominium. There is an existing wireless communication facility on the property, consisting of rooftop antennas and equipment cabinets located in an outdoor, fenced compound. AT&T proposed to add three (3) new panel antennas to the roof of the building, and one new cabinet within their existing equipment compound. Location of the proposal, including street address, if any, and section, township, and range; legal description; site plan; vicinity map; and topographical map, if reasonably available: This facility is located at 1414 91h Ave N, Edmonds, Washington. Parcel is located in Section 13, Township 27, Range 3SE W.M. Vicinity Map and Legal Description are included on the cover sheet (T-1) of the plan set submitted with this application. B. ENVIRONMENTAL ELEMENTS 1. Earth a. General description of the site (underline): Flat, Rollin, Hilly, Steep Slopes, Mountainous. (Note: the site is 100% developed) b. What is the steepest slope on the site (approximate percent slope)? The site is 100% developed. There is an engineered slope and rockery at the NE corner of the site that is approximately 100%. c. What general types of soils are found on the site (for example clay, sand, gravel, peat, muck)? Specify the classification of agricultural soils and note any prime farmland. No soil testing has been performed on this site. The site is improved only by the existing building, parking lot and landscaped grounds. No changes in these structures are proposed, so no geotechnical report will be repaired. d. Are there any surface indications or a history of unstable soils in the immediate vicinity? If so, describe. No. e. Describe the purpose, type, and approximate quantities of any filling or grading proposed. Indicate the source of the fill. No grading or fill is proposed. All structures presently exist. f. Could erosion occur as a result of clearing, construction, or use? No. g. About what percent of the site will be covered with impervious surfaces after project construction (for example buildings or asphalt)? Approximately 75% of the site is currently impervious surface (building, parking, driveway, pool, deck and walkways). h. Describe the proposed measures to reduce or control erosion, or other impacts to the earth, if any. No clearing or grading is proposed. No additional measures are proposed. 2. Air a. If any, generally describe and give approximate quantities, if known. There will be minor emissions from motor vehicles associated with construction. There are no emissions associated with the project operation. b. Are there any off-site sources of emissions or odors that may affect your proposal? If so, generally describe. No. c. Describe proposed measures to reduce or control emissions or other impacts to air, if any. None are necessary. 3. Water a. Surface: 1. Is there any surface water body on or in the immediate vicinity of the site (including year-round and seasonal streams, saltwater, lakes, ponds, and wetlands)? If yes, describe type and provide names. If appropriate, state what stream or river it flows into. No. 2. Will the project require any work over, in, or adjacent to (within 200 feet) the described waters? If yes, please describe and attach available plans. No. 3. Estimate the amount of fill and dredge material that could be placed in or removed from surface water or wetlands and indicate the area of the site that would be affected. Indicate the source of fill materials. None. 4. Will the proposal require surface water withdrawals or diversion? Give general description, purpose, and approximate quantities, if known. No. 5. Does the proposal lie within a 100 year flood plain? If so, note location on the site plan. No. 6. Does the proposal involve discharges of waste materials to surface waters? No waste material will be discharged into the ground as a result of the proposed modification of the existing wireless communication facility. c. Water Runoff (including storm water) 1. Describe the source of runoff (including storm water) and method of collection and disposal, if any (including quantities if known). Where will this water flow? Will this water flow into other waters? If so, describe. No additional water run-off is expected from this project. The additional equipment cabinet will be installed within the existing equipment compound. 2. Could waste materials enter ground or surface waters? If so, generally describe. No. d. Describe proposed measures to reduce or control surface, ground, and runoff water impacts, if any. No erosion is anticipated as no grading or ground disturbance is required by this project. 4. Plants a. Types of vegetation found on site: Deciduous trees: There are a few small deciduous trees on the site near the pool. Evergreen trees: There are a few small evergreen trees on the site near the pool and on the NE corner of the site. Shrubs: There are hedge rows along the southern and eastern edges of the property to screen the outdoor areas from the traffic and existing commercial uses to the east. What kind and amount of vegetation will be removed or altered? None c. List threatened or endangered plant species or critical habitat known to be on or near the site. None known. d. Describe proposed landscaping, use of native plants, or other measures to preserve or enhance vegetation on site. No additional landscaping is proposed. The outdoor equipment area is screened from the street and abutting properties to the east and north by shrubbery and small trees. 5. Animals a. Underline any birds and animals which have been observed on or near the site or are known to be on or near the site: Invertebrates: None observed Fish: Not applicable Amphibians: None observed Reptiles: None observed Birds: None observed Mammals: None observed b. List any threatened or endangered animal species or critical habitat near the site. No threatened or endangered species are known to exist on this site. c. Is the site part of a migratory route? If so, explain. No. d. Proposed measures to preserve or enhance wildlife, if any. None proposed. 6. Energy and Natural Resources a. What kinds of energy (electric, natural gas, oil, wood, solar) will be used to meet the completed project's energy needs? Describe whether it will be used for heating, manufacturing, etc. Electric power, required for the radio equipment, is already present on the site, and sufficient to operate the additional equipment. No other energy is required. b. Would the project affect the potential use of solar energy by adjacent properties? If so, explain. What kinds of energy conservation features are included in the plans of this proposal? List other proposed measures to reduce or control energy impacts, if any. None proposed. 7. Environmental Health a. Are there any environmental health hazards, including exposure to toxic chemicals, risk of fire and explosion, spills, or hazardous waste that could occur as a result of this proposal? If so, describe. No such hazards have been noted. All RF emissions will be within the parameters established and governed by the Federal Communications Commission. 1. Describe special emergency services that might be required. None are necessary. 2. Describe proposed measures to reduce or control environmental health hazards. Operation of the wireless facility is constantly monitored by AT&T Mobility's Network Operation Centers. Any operational problems are responded to on a priority basis. b. Noise 1. What types of noise exist in the area which may affect your project (for example: traffic, equipment operation, other)? None. Noise will not affect this project. 2. What types and levels of noise would be created by or associated with the project on a short-term or long-term basis (for example: traffic, construction, operation, other)? Noise levels on site will increase intermittently for one or two weeks during the construction phase of the project. Construction will occur during allowed time periods. When completed, the sound levels will meet the 45 dBA limit at the neighboring property line. The additional equipment cabinet will be installed inside of the existing equipment compound. A noise report prepared by a qualified third party will be submitted along with this project permit application. 3. Describe proposed measures to reduce or control noise impacts, if any. The distance from any sensitive responders is sufficient to mitigate the marginal noise produced by the equipment cabinets. g. The primary use of the property is a twelve -unit residential condominium. Properties to the north, south and east are developed by residential uses — single and multi -family dwellings. The property to the east is commercial b. Has the site been used for agriculture? If so, describe. No. c. Describe any structures on the site. The four-story condominium is the primary structure on the site. d. Will any structures be demolished? If so, what? No. e. What is the current zoning classification of the site? The site is currently zoned BN. L What is the current comprehensive plan designation of the site? Neighborhood Commercial g. If applicable, what is the current shoreline master program designation of the site? Not applicable to this project. It. Has any part of the site been classified as an "environmentally sensitive" area? If so, specify. The applicant has not found any evidence that this site has been classified as environmentally sensitive. i. Approximately how many people would reside or work in the completed project? 20. j. Approximately how many people would the completed project displace? None. k. Describe proposed measures to avoid or reduce displacement impacts, if any. Not applicable. The Edmonds Comprehensive Plan and Zoning Code allow wireless communication facilities with restrictions. Current code allows rooftop wireless facilities that do not exceed 9' above the roof of an existing structure that exceeds the current height limit of the zone. The existing and proposed antennas will be flush -mounted to the existing chimneys, or screened by an RF -transparent panel. 9. Housing a. Approximately how many units would be provided, if any? Indicate whether high, middle, or low-income housing. None. b. Approximately how many units, if any, would be eliminated? Indicate whether high, middle, or low-income housing. None. c. Describe proposed measures to reduce or control housing impacts, if any. No measures are proposed to control housing impacts. 10. Aesthetics a. What is the tallest height of any of the proposed structure(s), not including antennas? What is the principal exterior building material(s) proposed? The screening panels will extend approximately 6'-4" above the existing roof, or 46'-4" above grand level. The RF -transparent panels will be molded and painted to resemble a chimney. b. What views in the immediate vicinity would be altered or obstructed? None. c. Describe proposed measures to reduce aesthetic impacts, if any. The new antennas to be attached to the existing chimneys will be painted to match the color of the brick. The RF -transparent panels will be molded and painted to match the existing chimneys. No marker or hazard warning lighting is proposed or required for this project. There is a small work light within the equipment compound for use by technicians if emergency repairs are required at night. b. Could light or glare from the finished project be a safety hazard or interfere with views? No. c. What existing off-site sources of light or glare may affect your proposal? Existing light and glare will not have any effect on this proposal. d. Describe the proposed measures to reduce or control light and glare impacts, if any. The work light fixture is shaded to direct into the work area and not disturb neighboring properties. 12. Recreation a. What designated and informal recreational opportunities are in the immediate vicinity? Maplewood Park is located approximately'/2 mile east of the site on Puget Drive (Hwy 524). b. Would the proposed project displace any existing recreational uses? If so, describe. No. c. Describe proposed measures to reduce or control impacts on recreation, including recreational opportunities to be provided by the project or applicant. No measures are proposed to control impacts on recreational opportunities. 13. Historic and Cultural Preservation a. Are there any places or objects listed on or eligible for national, state, or local preservation registers known to be on or next to the site? If so, generally describe. The applicant has not proposed any measures to reduce or control impacts on historical or cultural aspects. 14. Transportation a. Identify public streets and highways serving the site, and describe proposed access to the existing street system. Show on site plans, if any. The property is located on the northeast corner of Puget Drive and 9t" Ave. Access to the property is from a shared driveway on 9t" Avenue. b. Is the site currently served by public transit? If not, what is the approximate distance to the nearest transit stop? Yes. Community Transit Route 116 Edmonds. Stops are located on adjacent Puget Drive and 91" Ave N. c. How many parking spaces would the completed project have? How many would the project eliminate? This proposal does not create or eliminate any off-street parking. d. Will the proposal require any new roads or streets, or improvements to existing roads or streets, not including driveways? If so, generally describe. This proposal will not require any improvements to existing roadways. e. Will the project use (or occur in the immediate vicinity of) water, rail, or air transportation? If so, generally describe. No, this project will not occur in the vicinity of water, rail, or air transportation. L How many vehicular trips per day would be generated by the completed project? If known, indicate when peak volumes would occur. This site would be visited approximately one time per month by a technician for service and repair activities. g. Describe proposed measures to reduce or control transportation impacts, if any. None are necessary.
