During review of the documents submitted in response to field corrections, we have the following
comments:
1. Listed and Tested Fire Stopping Assemblies (item 7 of 4/18/07 corrections). On 6/26/08 the
City received only the specification sheet for Firetemp CE by Johns Manville. The listed and
tested assemblies were not received. Firetemp CE is only a product noted within an assembly.
Johns Manville has numerous listed and tested assemblies on their website. Your contractor can
go to this website, download the "download binder" (see attached copy of website page) and
select a UL or WH listed and tested assembly for each type and size of penetration of fire rated
walls, floors and ceilings to submit for City review. If there are penetrations that are not covered
in the listing on the JM website, contact them directly for help. Fire stopping assemblies has
been discussed with your contractor both on the phone and in the field (as well as with the
plumbing contractor at the counter).
2. Geotechnical Special Inspection Reports (items 1 & 10 of 4/18/07 corrections): On 6/12/08 the
City received special inspection reports from Earth Consultants for soil bearing verification. We
are still awaiting special inspection reports for grading, temporary shoring, placement of
fill and compaction.
3. Structural Special Inspection Reports (items 1 & 11 of 4/18/07 corrections): On 6112108 the
City received special inspection reports from Otto Rosenau & Assoc. for spray -applied fire
proofing (dated 2/15/07), welding and high-strength bolting for structural steel construction
(dated 10125106), and concrete/rebar inspection for footings only (dated 3/28/06). We are still
awaiting special inspection reports for epoxy anchors, foundation wall rebar and concrete,
and for spray -applied fire proofing applied due to framing changes made after 2115/07.
Special inspection is not required for masonry since this was changed to solid concrete under the
structural revision.
4. Shop drawings (item 1 & 13 of 4/18107 corrections). On 6/12/08 we received a shop drawing
for reinforcement only (dated by engineer on 2/22/06). We are still awaiting shop drawings
for prefab. 1 -joists and roof trusses, open web steel joists/girders, and structural steel
fabrication.
Additional follow-up items:
• Refer to e-mail dated 6/20/08 from Jeannine Graf, Building Official regarding the structural
.revisions (item 2 of 3/26/08 corrections), architectural revisions (item 4 of 3/26/08
corrections -- also plan review corrections attached dated 3/31/08), and structural engineer's
inspection report (item 3 of 3/26/08 corrections).
• Plumbing Inspection: Once all items in City notice dated 4/2/08 have been corrected, call
the Building Inspection Hotline and schedule an inspection.
• Mechanical Inspection: Once all rough -in mechanical systems have been installed, call the
Building Inspection Hotline and schedule an inspection. During a phone conversation on
6/20108 with John Holte, he mentioned that he wanted to change the location of the garage
exhaust shaft. I recommended that he discuss this with the mechanical engineer first since
the code is very restrictive regarding the outlet location. I also told him that revised
mechanical, architectural and possibly structural plans would be required, and possibly ADB
review would be required if it affected the exterior of the building. Revised plans must be
stamped and signed by the design professional of record (one copy of each must be wet
signed).
• Framing Inspection: Before framing can be inspected by the City:
o All outstanding correction items must be satisfactorily addressed.
o Rough -in of plumbing and mechanical systems must be complete and signed off by
the City.
o Cover approval by the State Electrical Inspector must be obtained.
o Cover approval by the Fire Marshal must be obtained.
o Structural revisions must picked up and outstanding fees paid.

Project: Architectural Revision for New Mixed Use Building
Project Address: 155 3rd Ave South
During review of the revised architectural plan for the above noted project, it was found that the
following information, clarifications or changes are needed. Provide written responses to each
comment and where changes can be found on the plans. Please resubmit to Marie Harrison, Permit
Coordinator.
1. During the original plan review for this project, the design professional of record chose to use
IBC 1025.1 exception 6 for emergency egress from sleeping rooms. This requires a 3068 side
hinged exit door to be provided from the bedroom to the exit balcony (as shown on the original
approved plans). The sliding door how shown is not permitted and must be changed back to a
3068 side hinged exit door, unless the design professional of record determines another
approved method of emergency egress from the sleeping room.
2. Reference door type #2 on the floor plan for both bedrooms.
3. The revised plans must be stamped and signed (one copy wet signed) by the design professional
of record, Chris Covington, P.E.
