This survey purpose is to identify asbestos containing materials, prior to demolition. FROM PUBLIC RECORD: 8710184th St S W is a house in Edmonds, WA 98026. This 2,060 square foot house sits on a 0.85 acre lot and features 4 bedrooms and 1 bathroom. This property was built in 1952. NARRATIVE OF FINDINGS BASIC CONSTRUCTION: The house is a single -story on a post and beam foundation. The siding is wood. The roofmg is composition. The composition roofing materials were sampled. INTERIOR CONSTRUCTION, FINISHES, AND FLOORINGS: The interior is drywall and wood. The drywall and taping compounds were sampled. The vinyl and file flooring materials with associated backing and mastics were sampled. The masonry file flooring with relating mortar/mastic materials were sampled. The misc. mastic materials were sampled. The "popcorn" ceiling materials were sampled. INSULATION: The insulation materials were sampled. ELECTRICAL SYSTEM: The electrical wiring insulation did not appear to be the kind to be ACM. The power may have been "on" at the time of the inspection, thus sampling was not safe. Any suspect electrical wiring insulation or any suspect TSI (Thermal System Insulation) found in, around or behind any located fuse or breaker boxes should be considered to be ACM (Asbestos Containing Material), unless determined otherwise by laboratory analysis, continue to page 2 E3 o Page 2 of 10 B2170416 April 30, 2017 Asbesto-Test, Inc. 425-489-4040 House and CarporilShed * 871018,0 S"t. Sl ,'tbnon s'A 98026 HEATING AND VENTILATION SYSTEM: There is a gas forced air furnace in the house. There was no visible relating suspect ACM located except the flue mortar packing materials that were sampled. THERE MAY BE ADDITIONAL ACM TSI PRESENT (ASSOCIATED WITH THE FURNACE AND/OR DUCTING) IN THE STRUCTURE THAT IS CONCEALED FROM VIEW (i.e. within the walls, ceilings, and/or floors, inside additional ducting areas, or within the furnace unit itself). ALL TSI THAT MAY BE LOCATED THAT IS NOT VISIBLY FIBERGLASS AND/OR FOAM MATERIAL IS PRESUMED ASBESTOS CONTAINING. Note: It was not possible to dismantle the furnace and ducting to locate and/or evaluate any additional suspect ACM which may be concealed, inside the unit, or associated in hidden areas. It is our opinion any further handling of the furnace system should be coordinated by the project manager and/or abatement contractor. If any additional relating suspect asbestos containing materials are located prior to and/or during any demolition or renovation, any and all additional related suspect materials should be considered to be ACM (Asbestos Containing Material), unless determined to be otherwise by laboratory analysis. The brick and mortar materials on the front of the fireplace and hearth were sampled. MISC.: The window glazing materials were sampled. ADDITIONAL STRUCTURES ON SITE INCLUDED IN SURVEY: There is a wood frame carport/shed. The siding is wood. The siding vapor barrier materials were sampled. The roofing is composition. The composition roofing materials and sealants were sampled. # of structures included in survey: one house; one carport/shed Requested by: Michael Gallavan PJA N,1 +tl.
